This is great! Filled it and really looking forward to seeing the report :) Had a suggestion Divyesh Panchal, I believe you should add the option of 'Not Daily' in how often you snack field. That's ideally what I would have selected but since the lowest option was 1-2 times a day, that's what I went for. I believe there would be a bunch of people who don't snack everyday but rather in certain situations (which can also be a field actually) like at the movies, on a long drive, after skipping a meal due to work/ meetings, having munchies with drinks and so on. Even if people don't snack everyday these types of situations exist for everyone a few times a week and thats when they snack binge
