Junior Clinical Fellows in Adult Critical Care (Full time appointments)

Cardiothoracic ICU, General ICU and Neuro ICU

April 2019

Key working relationships: Critical care colleagues, surgical and medical referring teams and
anaesthesia colleagues; the emergency department.

Role of the Department: Provision of critical care services to patients on the general intensive
care unit, the cardiothoracic intensive care unit and the neuro intensive care unit.

Job Summary: St George’s Hospital is the primary teaching hospital in south-west London and
provides all of the tertiary specialities. Clinical fellows are required to work as part of the critical
care medical and multidisciplinary team. Candidates will be given the opportunity to rotate
between the Cardiothoracic ICU, Neuro ICU and the General ICU. The highest scoring
candidates at interview will be offered their first choice of unit. The duration of the posts is 6
months with the possibility of extension. We can accommodate experienced trainees / CFs
looking to broaden their experience (including subspecialty specific) and novice trainees / CFs
with no prior ICM experience. Duties and responsibilities will be commensurate with the individual
candidate’s experience, but are overseen by consultant intensivists at all times. The experience
gained will be beneficial to trainees wishing to pursue a wide variety of career aims.
Successful candidates often extend their stay beyond 6 months to gain further experience within
the directorate.
The rotas on each unit vary, but all attract a 1A banding (50%). Samples rotas template can be

St George's Hospital is a major acute hospital that offers very specialist care for complex injuries
and illnesses, including trauma, neurology, cardiac care, renal transplantation, cancer care and
stroke. It has a helipad which opened in spring 2014 and the work of its emergency department
can currently be seen on Channel 4’s ‘24 Hours in A&E’ programme. The hospital also provides
all the usual care you would expect from a local NHS district general hospital.
We also provide services at Queen Mary’s Hospital in Roehampton. These include outpatient
rapid diagnostic and treatment facilities, a minor injuries unit, limb fitting services and 70 elderly,
intermediate care and rehabilitation beds.

As well as hospital services, we provide a wide variety of specialist, maternity and community
services to children, adults, older people and people with learning disabilities. These services are
provided from St John’s Therapy Centre, 11 health centres and clinics, schools, nursing homes
and people’s own homes. The trust also manages the healthcare service at HMP Wandsworth

Background

General Intensive Care

GICU is an 18 + 3 bed facility admitting over 1500 adult general medical, surgical and trauma
patients per year. We have full respiratory, cardiovascular and renal support. The post holder will
assist with the management of patients with severe polytrauma, post-operative high risk surgical
cases and patients with severe medical conditions requiring organ support. The General ICU
team is usually the first point of contact for patients that may intensive care admission on wards or
in the emergency department. Under the supervision of more senior colleagues, the successful
candidate will be able to develop the necessary skills to see patients referrals and evaluate
where, how and when to admit a deteriorating patient and how to stabilise and manage the
patient in ICU. There is continuous support from a Consultant available 24/7. General ICU is a
busy unit with plenty of research and educational activities. Members of our team hold, or have
held, senior positions in international and national Societies and successful candidates will be
given the opportunity to participate in many educational and research activities.

Cardiothoracic Intensive Care:


CTICU is a 21 bed unit which accepts around 1500 admissions per year and provides care to a
diverse group of patients.

Successful candidates will assist with management of these patients including generic intensive
care support, advanced respiratory support, advanced cardiovascular monitoring (including
pulmonary artery floatation catheters), intra-aortic balloon counter-pulsation, trans-thoracic and
trans-oesophageal echocardiography and renal replacement therapy. The fellow will always be
working under the supervision of a cardiothoracic intensive care consultant. Other members of
the CTICU medical team include anaesthetic specialist trainees, respiratory physicians and
clinical fellows. During quiet periods on the unit, there is the possibility to attend theatres to gain
exposure to cardiothoracic surgery/anaesthesia and transoesophageal echocardiography. Senior
ICM trainees have the opportunity to spend some of their day shifts pursuing their own specialist
interests (eg broadening theatre experience, research, teaching etc).

Neuro Intensive Care

NICU is a 12 +3 bed unit. Successful candidates will assist with the management of patients with
severe traumatic brain injury, spinal cord injury, subarachnoid haemorrhage, elective and
emergency post-operative neurosurgical cases and patients with neurological disease requiring
advanced organ monitoring and support. There may be opportunities to observe neurosurgery or
neuroradiology theatres lists or undertake neuroscience related research projects.

Non-clinical activities

The St George’s intensive care units conduct a comprehensive teaching programme for trainee
staff, both generic and specialty specific. In addition, dependent on workload, there may by
opportunities to attend the multiple other teaching opportunities provided by a large teaching
hospital/tertiary referral centre. Clinical Fellows are expected to actively participate in research
and audit as well as attend multi-disciplinary and clinical governance meetings. Participation in
specific research projects will be encouraged and facilitated. Successful applicants will be
expected to present at departmental and national meetings.

 The post holder will be required to participate in clinical governance activities within the
service centre.
