1. Job Outline
We are looking for an enthusiastic and dynamic Medicine Doctor to work at the Bedford
Hospital Site . You must be dedicated to providing high quality emergency care,
maintaining clinical standards, and continually looking to drive positive change by
developing the service.
This is a Trust Doctor post and therefore does not have recognised training approval.
However, every effort will be made to provide teaching and training according to the post-
holder’s needs. Study leave and budget will be made available in line with trust
regulations. The successful candidates will be able to discuss their career aims and
progression with the clinical supervisor and ES. They will be encouraged to keep a record
of their competencies and training and encouraged to apply for entry to formal Core or
Specialty Training Program (as appropriate).

The successful candidate will work as part of a multidisciplinary team comprising of both
senior and junior medical staff, nurses and other allied health professionals. Duties will
normally be comparable to those undertaken by a core surgical trainee (CT1/2). However,
in exceptional cases you may be required to vary these at the direction of the Supervising
Consultant.

Your job will include a combination of direct clinical care, clinical education and personal
development activities designed to support your career progression alongside delivery of
the department’s service objectives.

2. Duties and Responsibilities

Routine and emergency patients must be seen and admitted expeditiously.


Ward rounds and visits to individual patients as well as discussion with their relatives may
be necessary.
Keeping medical records as per GMC Good Medical practice is utmost; these must be
dated and initialed, together with any other related administrative duties including
discharge and clinic letters.
The patient’s discharge summary paperwork is to be completed prior to the patients
discharge. Clinic letters are dictated within clinic and typed by the secretaries.
Supervision of the Foundation Year 1 doctors when performing clinical duties.
Duties in occasional emergencies and unforeseen circumstances at the request of the
appropriate Consultant(s), in consultation where possible with colleagues both senior and
junior. Additional commitments arising under this paragraph are exceptional. In particular,
the doctor will not be required to undertake work of this kind for prolonged periods or on a
regular basis.

3. On Call Duties

This post will include oncalls including Long days, Nights and weekends.

4. Department of:

Medicine
The Department of Medicine has approximately 80 beds and there is a 9-bedded Coronary
Care Unit. Patients requiring intensive therapy, artificial respiration, or dialysis are normally
admitted to the ITU remaining under the care of their Consultant and his team, although
routine care is the responsibility of the ITU team.
The Medical Department operates an age related admission policy, the Physicians for the
Elderly admitting all patients over 75 years of age. Most acute admissions are directly
admitted into a 26-bed medical admissions unit.

The Medical Directorate consists of nine General Physicians, two in each, of Diabetes and
Endocrinology, Gastroenterology and Respiratory Medicine with three cardiologists. There
are four Care of the Elderly Consultants. There are four Integrated Medical Teams, each
consisting of 2 General Physician Consultants and 1 Care of the Elderly Consultant.
Middle-Grade services are provided by 8 Medical SpRs, 2 Care of the Elderly SpRs and 4
Staff Grade Posts. There are 16 ST/FY2’s and 5 HO’s in the Integrated Medical
Department. The SpR on call rota has recently expanded to ten with split weeks of nights

Patients under the care of the Neurologist, Haematologist, or Oncologists, who require in-
patient treatment are admitted under the care of the physicians and he and his team
undertake the care of the patient whilst in hospital, in consultation with the relevant specialist.
This provides good training in these specialties and gives good opportunity for establishing a
firm basis for a subsequent career in general medicine.

Obstetrics & Gynaecology


The Department has 36 obstetrics and 16 gynaecology beds plus 6 beds in the day care
ward and in addition there are 14 special care cots in use in the Neonatal Unit. There are
over 2,700 deliveries a year. Fetal monitoring and diagnostic ultra-sound scanning are in
everyday use and epidural analgesia for labour is available around the clock. The busy
Gynaecology Department carries out about 450 major and 2,300 minor operations a year.
Designated outpatient clinics are run for colposcopy and large loop diathermy,
hysteroscopy, urodynamics, infertility, high risk obstetrics and diabetes in pregnancy.
Minimal Access surgery, including hysterectomy and endometrial ablation are also
performed. It is expected that Specialist Registrars will become competent at all standard
gynaecological surgery.

Paediatrics

Bedford Hospital Department of Paediatrics has undergone a significant and exciting service
redesign and is providing a consultant delivered model of service to the children of Bedford.

A team of 8 consultants provide a minimum of 14 hrs per day of consultant delivered care.
Bedford Hospital Trust and its partners are developing an innovative and thoroughly modern
approach to delivering acute paediatrics. The unit has one of the shortest inpatient stay for
children

Children’s Assessment unit is currently situated within Riverbank day unit and receives
emergency GP referral’s, A and E referrals and children with open access to the CAU (those
just discharged and some children with long term conditions.

Paediatric Ambulatory Care


The development of a robust paediatric Ambulatory Care system helps provide planned
attendance for children with long term conditions for reviews, phlebotomy service and some
procedures preventing an admission. It also provides the Hospital at Home Nursing Team.
Neonatal Services
There are 12 cots in the level 1 neonatal unit with highly skilled and dedicated staff lead by a
consultant with an interest in neonatology. The modern unit is very well equipped and has 1
Drager and 2 SLE’s ventilators, head ultrasound facilities together with blood gas and
bilirubinometer equipment. There is a team of community nurses to assist early discharge of
babies from the neonatal unit.
The Neonatal unit worked as at level 2 unit until the network re-designation as level 1. It
continues to have a potential to work at level 2.

Dedicated Paediatric Out Patient Department


This is an excellent facility. Few other DGHs have separate outpatient facilities specifically for
children. It is also used to provide specialty clinics in collaboration with teams from tertiary
care centres including cardiology, cystic fibrosis, genetics and cleft lip & palate services.

ENT
The Department has variable number of beds on the specialist surgery ward. Beds are
also available on the children’s ward and day case unit. There are about 1200 ENT
admissions each year, the majority of which are elective, but a significant amount of
inpatient and outpatient work comes from the busy Accident and Emergency department.
A full cross section of ENT disease is seen and treated, backed up by a full pathology and
imaging service which includes a CT Scanner and MRI Scanner. The three consultants
have interests in otology, head and neck surgery and endoscopic surgery of the sinuses,
and the clinical fellow will receive training in all fields. The department has regular audit
meetings and the juniors are assigned projects which they are expected to present. The
clinical fellow will be involved in both formal and informal teaching of nurses and FY2/STr.

Emergency Medicine
The Department is busy, seeing approximately 67,000 patients per annum

The Department is a designated Trauma Unit and operates a Trauma team 24/7. The
postholder will be expected to lead the Trauma team with other middle grade/senior staff.

There is an active teaching programme for Senior House Officers and visiting Medical
Students. The Consultants are involved in training Paramedics. The Department has its
own Seminar Room with up to date education and audio visual aids.

General Surgery
The department of general surgery is busy and dynamic with strong emphasis on training
along with providing a first class service to our patients. Eleven consultant surgeons work
on firms with specialist interests in Vascular, Breast, Endocrine, Upper gastrointestinal and
Colorectal Surgery. In addition all surgeons undertake a range of general surgical
procedures including laparoscopic repair of incisional and inguinal hernias (TEPP and
TAPP) and laparoscopic cholecystectomy. Laparoscopic colorectal resection for the
treatment of benign and malignant disease of the colon, is now well established within the
colorectal department.
Vascular surgery is one of the core services provided by Bedford Hospital. Six vascular
surgeons provide round the clock cover to the population of Bedfordshire (Bedford, Milton
Keynes Luton and Dunstable). We offer experience in the full range of vascular
procedures including Endovascular Aneurysm Repair (EVAR) and endovenous treatment
for varicose veins (radiofrequency ablation and ultrasound-guided foam sclerotherapy).
We have a full time Clinical Vascular Scientist providing a non-invasive diagnostic service.

Approximately 50% of admissions to the unit are emergencies and this enables the trainee
to gain experience of management of the acute abdomen. The proximity to major roads
means that we receive a fair share of patients following road traffic accidents allowing
experience in the management of patients who have sustained multiple trauma.

The Surgical department has a compliment of 85 beds.

An audit facilitator is seconded to the Surgical Unit from the Hospital's audit department
and trainees are encouraged to take part in audit projects and to develop clinical research
interests.

Out Patient Clinics are conducted in parallel with the Consultant and under his
supervision. Clinics are divided into General Surgery and special interest clinics according
to the firm on which the trainee is working.
Endoscopy - There is a separate dedicated endoscopy unit for both upper and lower GI
endoscopy.
Day Surgery - There is a dedicated day ward within the hospital and the trainee will be
introduced to the special needs and techniques involved in dealing with day patients. At
present some twenty five per cent of elective surgery within the General Surgery
directorate is carried out on a day case basis.
Operating Theatres - The trainee will work in theatre assisting and being directly
supervised by the Consultant with whom he is working.
Wards - Each surgical team operates from its own ward, for both cold and emergency
admissions.

The Trust

It is an exciting time to be joining Bedfordshire Hospitals NHS Foundation Trust which


resulted from the merger of Luton and Dunstable University Hospital and Bedford Hospital
NHS Trusts in April 2020. Both hospitals have retained their identity with individual hospital
names and key services remaining on both sites and be managed as single teams across
the whole Bedfordshire catchment area.

A merger was the logical next step in collaborative working after the two hospitals worked
closely together for a number of years in various clinical services such as:
 Neonatal Intensive Care
 Vascular surgery
 Oral and Maxillofacial (OMF)
 Ear Nose and Throat (ENT) and
 Stroke services
 Pathology services since June 2020.

Our Trust has a strong portfolio of complementary specialist services: bariatric surgery,
vascular, head and neck, OMFS, NICU, HASU

In 2022, the Trust was rated GOOD by CQC.

Innovation and technology:


We have state-of-the art facilities placing us at the heart of cutting edge health care. The
Trust continues to be committed to delivering the best patient care using the best clinical
knowledge and technology available.

We have embraced the latest in Robotic Assisted Surgery with the arrival of two da Vinci
Xi robots, one at Bedford Hospital and one at Luton & Dunstable University Hospital
(L&D), which will bring considerable benefits for both patients and staff.

Unlike in many other hospitals in the UK, the two robots will be used across a range of
different specialties, treating patients with conditions ranging from bowel and renal cancer
to endometriosis, gynaecology, bariatric and biliary surgery, and complex head and neck
conditions.

Redevelopment projects:
Both sites have been undergoing a range of redevelopment projects.

Bedford:
We have completed one of our major redevelopment projects at Bedford hospital, to
expand our Outpatients facilities within Cauldwell Centre. The expansion included an
additional 34 rooms, which increased the overall number of Outpatients rooms from 19 to
53.

Within the Emergency Department work has been carried out to deliver nine additional
beds, two extra paediatric cubicles, a two-storey extension, two larger adult waiting areas
and new reception. The previous space was converted to provide an isolation room,
treatment rooms, mental health room and dedicated secure paediatric ED.

In addition, a brand new state of the art CT scanner has been installed within the
Emergency Department to allow for rapid diagnostics and improvement to patient
experience and care.

To allow the Trust to further develop and improve the Bedford site, we have also upgraded
our electrical infrastructure to provide additional capacity and resilience across the site.

L&D:
Work to create an acute services block and new ward block is currently underway to house
maternity, neonatology, theatres and critical care services. This work is planned to
complete towards the end of 2024, with services moving in the following year.

Within the Emergency Department work has been carried out to deliver a brand new
entrance and waiting area, a dedicated paediatric area and 15 new major cubicles. The
final phase of works will see the expansion of the department where there will be a further
increase in capacity, including assessment and triage rooms and sensitively designed
mental health care rooms.

In addition, a brand new state of the art CT scanner has been installed within the
Emergency Department to allow for rapid diagnostics and improvement to patient
experience and care.

An Energy Centre has also been built on site to deliver a substantial reduction in energy
consumption, supporting the Trust’s drive to Net Zero Carbon, and will increase resilience
across the site.

This will support with the further expansion of works and developments for the future.

About Bedfordshire

6. General Information

Medical Education

Medical education is led by the Directors of Medical Education, Dr Tejal Shah, Director for
the Bedford site and Professor Ritwik Banerjee, Director for the Luton site. They are
supported by a cross site Medical Education Manager and a site-specific team of
administrators.

Medical Education centres are based at both Luton & Dunstable and Bedford Hospital
sites and offer a comprehensive range of education and training events for
undergraduates, postgraduate professionals and support staff to help drive forward and
encourage life-long learning in the NHS. Both sites offer good library and study facilities,
including PC/internet support for staff and students, as well as classrooms, lecture
theatres and high tech simulation facilities.

The Danielle Freedman Library at the Luton site and The Bevan Library at the Bedford site
offer access to a range of printed and electronic materials, including books, articles,
journals, and research publications. The Library staff can help you with evidence searching

to support your decision making and offer training to help you develop your own searching
skills.

We are committed to developing our teaching and academic activities across the Trust and

Revalidation – Annual Appraisal review

The Responsible Officer (RO) for medical revalidation is Mr Paul Tisi, Medical Director who is
fully supported by two Associate Medical Directors for Revalidation and Appraisal (Dr Penny
McNamara and Miss Neela Mukhopadhaya), a team of appraisers (including three Senior
Appraisers) and a Revalidation Support Officer, with systems in place to ensure and support
annual appraisal and 3600 MSF in line with GMC and NHS-England requirements. The RO
and Chair are required to confirm to NHS-England annually that Medical Appraisal and
Revalidation are adequately supported.

Terms and Conditions

The post will be subject to the Terms and Conditions – Junior Doctors (England) 2002.

If the consultant appointed chooses to contract on a whole-time basis and wishes to


undertake private practice, this private work must be carried out in accordance with the
Code of Conduct for Private Practice.

All Consultants have a continuing responsibility for the care of patients in their charge and
for the proper functioning of their department, and are required to undertake the
administrative duties associated with that responsibility.

Pension

Employees will automatically be enrolled within the NHS pension scheme and will be
required to opt out on appointment if they do not wish to pay into this scheme.

Smoke Free

The Trust implements a Smoke Free policy that applies to all staff. Staff are not allowed to
smoke while wearing a recognisable Trust uniform or visible trust identification badge, and

not allowed to smoke anywhere on hospital grounds. Staff are not allowed to take
additional breaks in order to smoke. They may smoke during designated breaks but only

Data quality

The information that you record as part of your duties at the Trust must be ‘fit for purpose’,
reliable and easily accessed by appropriate/authorised personnel. To achieve this
standard the information must be: accurate, legible (if hand written), recorded in a timely
manner, kept up-to-date, appropriately filed.

All staff must monitor and take responsibility for data quality throughout the areas of the
system used locally, all users maintain timely input, and ensuring that data is checked with
the patient, and staff (in relation to their staff record), whenever possible, and to support
initiatives to improve data quality.

Recorded information includes: patient information entered in case notes and entered on
any computerised care records system, financial information, health and safety information
e.g. incident reporting and investigation, personnel information recorded in personnel files
etc.

Failure to adhere to these principles will be regarded as a performance issue and will
result in disciplinary action.

Payment of Salary

Salary is paid direct into your Bank or Building Society on the 26 th (Bedford) and 27th
(Luton) of each month.

Variation
