Title: Junior Clinical Fellow in General Medicine

Professionally accountable to: Medical Director via Clinical and Divisional Directors

Managerially accountable to: Supervising Consultant – Dr Shridhar Aithal

Base: Ysbyty Ystrad Fawr, Ystrad Mynach, SE Wales


(On-calls at Grange University Hospital, Cwmbran, SE Wales)

Other hospitals at which duties are performed: In view of the fact that the organisation is currently undertaking a review of
its services and the locations at which they are undertaken, it is important to be aware that work patterns may change and
the place(s) of duties modified.

The Clinical Strategy of the Health Board is under constant development and the job holder may be required to undertake
different duties agreed at the time of appointment.

Purpose of the Post and General Responsibilities

The post involves working at the Medical admission unit at Ysbyty Ystrad Fawr site (Normal working day – 9 am to 5 pm) as
your base site and also doing on-call at The Grange University Hospital (GUH). The post-holder will be trained in the
assessment & management of medical, psychological and social health of frail older patients.

We have on average about 30 acute admissions per day through the MAU and about 30-40% of the patients are older and
frailer patients. The medical assessment unit is comprised of 9 beds, which post holder will be covering when not on – call.

The post holder will be expected to participate in the on call which is currently one in 14 based at GUH site. This one in 14
on-call will attract a banding supplement (40%) and it means in a block of 14 weeks, you will be one week of long days
(8.30 am to 9 pm), one week on nights (8.30 pm to 9 am) and one week of twilight (2 pm to Mid-night) – spread
throughout 14 weeks period in block shifts. The post-holder will be working as junior doctor on call at FY2 level. There are
additional 3 more tiers of on-call doctors – SHO level, junior medical registrar grade and senior registrar grade. There will
be additional three on-call doctors covering wards at GUH on Saturdays and Sundays.

The expectation of the role is to provide junior grade support as part of a multi-disciplinary team in a 9 bedded Medical
assessment unit YYF Monday to Friday when not GIM on-calls at GUH. The post-holder will support consultant led ward
rounds in the acute medical unit (MAU – YYF). There is a very close cooperation with the acute medical team (on call) and
other two teams covering wards. He/she will be expected to pull patients from GUH after discussing with senior registrar/
Consultant / ANP.

The post-holder will be responsible to Dr Shridhar Aithal who will be her/his overall named Clinical Supervisor (CS) and will
be expected to work closely also with other medical teams based at Ysbyty Ystrad Fawr Hospital and GUH.

The post holder will also be expected to take part in the ambulatory care clinics attached to MAU under direct supervision
of senior clinicians based on the clinical requirement and to meet the expected progression of the post holder.

There is an active postgraduate teaching programme and clinical audit is a priority for the department/ unit.

The appointee will become a valuable member of the acute medical directorate and will be responsible to the consultant
physician for whom he/she is working. The principal regular duties include: -
Clinical

(1) The main responsibilities are for the effective and appropriate medical care of patients admitted under the care
of the Consultant Physician in MAU.
(2) The duties and responsibilities will be appropriate to the grade and speciality with appropriate consultant
supervision.
(3) This will include the assessment, monitoring and initiating treatment, with colleagues and under consultant
supervision.
(4) Attend consultant and multidisciplinary ward rounds, and supervise relevant junior medical students who may
rotate through the directorate.
(5) Should undertake regular unsupervised reviews and present cases to seniors.
(6) Complete discharge summaries in a timely fashion
(7) To attend Ambulatory care clinic as part of continuing professional development.
(8) Research and service outcome evaluation will be encouraged as part of this job.

Specific Responsibilities of the Post

Qualifications
Eligible to hold full registration and a licence  
to practice with the GMC/GDC
- Valid Certified Basic Life Support Skills (or  
equivalent international qualifications) OR
- Valid Certified Advanced Life Support Skills
(or equivalent international qualifications)
– mandatory for all Cardiac Arrest Teams
(Paeds, Neonatal, Adult Services)
Experience
Evidence of working effectively as a part of a   
multidisciplinary team

Previous experience of working in the NHS or   
equivalent - 12 months
12 months experience after full GMC   
registration or equivalent and evidence of
achievement of Foundation competences

The Specialty

Senior and Consultant Members of the Service Group

The medical unit comprises of 9 consultant physicians based at YYF. Visiting Consultants provide, Acute medicine, Respiratory, Cardiology,
Rheumatology and Dermatology cover. The medical teams comprise of SpR’s, Clinical fellows, specialty doctors and junior doctors (FY and
GPVTS trainees). This post will be attached to General Medical team within the medical directorate (YYF).

He/She will be based in the Medical Assessment Unit when on call, and will be supervised by Acute Physicians or consultant on call who
will provide close hands on supervision. The consultants provide a daily on-call system.
