Job Title: Locum Consultant Paediatrician with an interest in


Ambulatory Paediatrics and Named Doctor for Child
Safeguarding

Full Time 10 PAs

Due to shift working and rota patterns this post is not

supports some 320,000 people in the surrounding area including the boroughs of Kingston,

top six London Trusts, Kingston is the only District General Hospital: the others are teaching

centre of London with good transport links into the capital and in a beautiful and historic part of
the country. It lies very close to Richmond Park and is close to Hampton Court Palace and

Paediatrics and Neonates at Kingston Hospital

We strive to achieve this through the quality communication of our excellent multidisciplinary
team and through the use of our facilities as follows:

Neonatal Unit 18 Cots; 2 ITU, 4 HDU. Supporting one of London’s largest maternity units,
with around 5800 deliveries per year, the maternity unit is consistently
highly rated by users and has been rated as the best maternity service in
London by CQC.

Dolphin Ward Children’s Day and Ambulatory care ward

Dedicated Children’s Outpatient Department

We run a Paediatric Assessment Unit (PAU) model of care on a fully refurbished suite within
Dolphin ward, with four beds and three cubicles.

We also provide the following specialist care:


 PONT – Community Nursing Team
 Specialist Diabetes Team
 Oncology Shared Care - working closely with specialist Centres of Excellence e.g. The
Royal Marsden, Great Ormond Street - and with a very large caseload of around 70
patients on active treatment.
 Specialist Outpatient Clinics run in partnership with Centres of Excellence:
o Renal Clinic, quarterly - The Evelina Children’s Hospital
o Cardiac Clinic including physiologist ECHO clinic, monthly - The Evelina Children’s
Hospital
o Endocrinology, quarterly - Great Ormond Street Hospital
o Gastroenterology, every 2 months – Chelsea and Westminster Hospital

The introduction of a PAU resulted in increasing the Paediatric Consultant workforce at Kingston
to a total of 13.8 WTE Consultants.

With the department providing care through an ambulatory model using a Paediatric Assessment
Unit (PAU), this post is instrumental in running this service. Assessment of patients referred by
A&E as well as attendance of GP referrals directly to the PAU, if appropriate to do so, is
undertaken. The PAU provides rapid access to senior paediatric opinion in a bespoke, child
friendly environment.

In accordance with Working Together to Safeguard Children – a guide to inter-agency working to


safeguard and promote the welfare of children, HM Government 2006, each NHS health trust is
required to have a named doctor for safeguarding children with protected time for the role. The
Named Doctor will work closely with the other team members and with the PCT Designated
Doctor and Designated Nurse in supporting all activities necessary to ensure that the Trust
meets its responsibilities to safeguard and promote the welfare of children.

Paediatric Outreach Nursing Team

 General Paediatrics

The General Paediatric ward has 24 beds plus 2 HDU beds. The Paediatric Department admits
approximately 2800 children per annum to its in-patient ward (Sunshine) and sees an equal
number on its ambulatory care unit (Dolphin Ward). The Children’s A&E department sees
approximately 27,000 attendees aged < 16 years. The Paediatric Department works closely with
the A&E Department; the Children’s A&E is staffed by at least 2 paediatric nurses 24/7. The
paediatricians automatically see all children aged < 6 months of age.

There is a daily consultant-led handover and ward round on the in-patient ward. The junior STs
are responsible for obtaining investigation results, presenting patients on the ward round and
managing the clinical care of the children as directed by the consultant. Multi-professional notes
are used and regular reviews of the standard of written documentation are performed.

 Neonates

The neonatal unit has 2 designated ITU cots, 4 HDU and 12 SCBU cots. There is an 8 bedded
transitional care ward. Working as part of the South London Neonatal Network, the unit provides
intensive care for most local babies born at a gestation above 26 weeks. The neonatal unit is
able to provide full intensive therapy including HFOV and cooling for HIE.

The large Maternity Unit delivers approximately 5800 babies per annum.

 Children’s Outpatient Service

The Children’s Outpatient Service currently has a dedicated children’s outpatient department with
6 clinic rooms. In the year 2016/2017, we saw approx 14,000 patients in medical and
multidisciplinary clinics here. We are working towards greater integrated services with our
primary care colleagues, and also run clinics from the Raynes Park Health centre.

 Support Services

The paediatric department is fully supported with appropriate 24 hour access to:
o Pathology service investigations and advice
o Radiological investigations – x-ray, ultrasound, CT, MRI and nuclear medicine –
including 24 hour advice and interpretation. There are 4 Consultants with a
specialist interest in Paediatric Radiology available for advice during normal
working hours, and who provide joint sessions with the paediatric department
every 2 weeks.

Professional Structures
All consultants are members of the Medical Staff Committee. There is a Local Negotiating
Committee representing the British Medical Association, which reports to the Medical Staff
Committee.

KEY RESPONSIBILITIES/OUTCOMES:

General

 Provide clinical leadership

simple care pathways
 To promote close working links with all providers of services for children, particularly
within local primary care and the South West London Network
 To manage a flexible approach to service provision, adapting practice and provision to
meet changing needs
 To work with consultant colleagues, other medical colleagues and junior doctors,
professional and nursing colleagues and local managers to ensure the efficient and
effective running of the service

General Clinical

 To work alongside other consultant paediatricians and other care professionals to provide
an up to date general and neonatal paediatric service
 To lead and deliver care on the Paediatric Assessment Unit (PAU)
 Support junior doctors and nursing colleagues in their roles on the PAU
 To participate in the attending system of care rota for consultants on Sunshine (general
paediatric) ward, 1 or 2 attending weeks per year
 To hold 2 clinics and 5 virtual consultations per week during clinic weeks
 Once sufficiently in post, to provide leadership and guidance for staff in an area of clinical
interest as lead for the department

Clinical Governance

 To comply with the Trust’s clinical governance policy and participate in relevant
multidisciplinary and uni-professional clinical audit projects. The department is actively
involved in both national and local level audit and all Consultants are expected to
participate in at least 1 audit annually.
 To undertake the clinical risk management and clinical governance activities of the
department and trust

Continuing professional development

 To ensure a register of continuing professional development is maintained, such as that


through the Royal College of Paediatrics and Child Health
 We have departmental links in research projects and opportunities to enhance
involvement could be a possibility if desired
 Involvement in departmental educational programme facilitating internal CME/CPD
 Participate in external CPD

Training and Teaching

 To contribute to the in-house training programme for junior doctors


 To contribute to the training of nurses and therapists or other staff in the trust, as well as
health visitors, school nurses, paediatric home care nurses, practice nurses or therapists
from the local CCGs that require input from a consultant paediatrician for training
 To contribute to the department’s training of medical students and physician’s assistants

Management, Service planning and Development

 To take an active role in the working of the paediatric service line and in the wider division
 To collaborate with colleagues to ensure services develop to comply with current and
future national guidelines
 To participate in the development and planning to maintain a PAU model of care
provision

 To support the trust and CCGs in the strategic development of integrated children’s
services including delivery of service within the community and supporting primary care
colleagues
 The appointee will observe the trust’s policies and procedures and will follow standing
order and standing financial instructions of the trust

Named Doctor for Safeguarding Child Safeguarding

 Take the medical lead and promote good professional practice for safeguarding children
within the Trust and in the locality in which the post holder is based.
 Ensure appropriate safeguarding children clinical lead arrangements for adult and older
people’s services, forensic services and specialist addictions services.
 Attend and contribute to the Trust’s Safeguarding Children Committee.
 Contribute to the Trust’s annual Safeguarding Children report, with the other named
doctors, on safeguarding issues for the medical directorate.
 Be a source of advice and expertise for fellow professionals and other agencies at point
of need on safeguarding children issues.
 Initiate new ways to raise awareness of safeguarding children issues with medical
colleagues.
 Contribute to, or carry out, LSCB Serious Case Reviews, in conjunction with the Trust’s
Associate Director and a Named Nurse/Professional, except for cases where there has
been substantial personal involvement.
 Participate in or lead internal SI panel investigations that involve child protection issues.

Job plan for Consultant in Ambulatory Paediatrics

1. Job content

All Consultants, including Ambulatory Consultants, contribute to an attending system of care for
continuity of patient care. This post will cover 1 or 2 attending weeks on the General Paediatric
Ward per year, attending 1 in 40 weeks as part of an integrated ambulatory and attending
consultant working pattern. There are no attending duties on the NNU associated with this post,
but some cover of the NNU is required within the rota and an attending role on NNU could be
incorporated if this is an area the successful candidate wishes to pursue (as other post holders
have done). The post holders will therefore be required to ensure neonatal skills are maintained
at a sufficient level to provide this cover – it is anticipated that most of this will be met through
internal departmental activity.

This post will be part of the rolling 5 week rota providing ambulatory and urgent care, with cover
for the PAU, including out-of-hour’s care, for 2 of those weeks. The working pattern and job plan
may be subject to minor changes to accommodate individuals’ and service needs.
There is no night time resident on-call commitment.

Attending weeks involve the Consultant delivery of acute inpatient care including the Consultant
of the week twice daily ward rounds on these 7 days. The only out-of-hours, on-call-off-site work
that is required of this post is the on-call for Friday, Saturday and Sunday nights, 1700-0800
each day of the attending weekend.

of-hours commitment for general paediatric attending weeks.

Consultant’s Job Plan


Job planning will be based on a partnership approach. The clinical manager will prepare a draft
job plan, which will then be discussed and agreed with the consultant. Job plans will list all the
NHS duties of the consultant, the number of programmed activities for which the consultant is
contracted and paid, the consultant’s objectives and agreed supporting resources. The duties
and responsibilities set out in a Job Plan will include, as appropriate:

- Direct Clinical Care duties including on-call work


- Supporting Professional Activities
- Additional NHS responsibilities
- External Duties

Educational Facilities
The Postgraduate Medical Centre has been recently refurbished and there is an excellent
multidisciplinary library. There is an active Postgraduate programme for junior staff co-ordinated
by the Clinical and College Tutors with regular meetings, seminars, x-ray sessions and numerous
tutorials. Medical students are attached to the medical teams from Imperial College and St
George’s Hospital Medical School.

Continuing Medical Education


The appointee will be required to meet, as a minimum, the requirements of his/her Royal College
in respect of continuing medical education. Collaborative research with other clinical colleagues
is encouraged by the Trust.

Protecting Patients Guiding Doctors


The Trust supports fully the General Medical Council’s performance procedures designed to
protect patients and guide doctors. In the context of a doctor’s fitness to practice, the Trust
requires all doctors to follow the GMC’s principles of Good Medical Practice, which encourages
and promotes effective self-regulation.

Secretarial / Administrative support


The post holder will have access to secretarial support and a personal desk in a shared office at
Kingston Hospital. A PC will be provided.

Health Clearance
Health clearance is required for this appointment. Applicants must complete a medical
questionnaire, return it to the Occupational Health and Wellbeing Service and, if required,
undergo a medical examination before appointment.

Disclosure and Barring Service (DBS)


A DBS will be required before appointment for all posts with access to children or vulnerable
adults.

Confidentiality and Disclosure of Information


In the course of your normal work with the Trust you will come into possession of confidential
information concerning patients, the Trust and its staff. This information should always be
treated according to the Trust’s rules on confidentiality. Any inappropriate disclosure may be
subject to the Trust's disciplinary procedures.

Staff may on occasion have genuine concerns about healthcare matters and consequently the
Trust endorses the principle that these must be raised in a responsible and appropriate manner,
and if necessary using the Trust’s ‘Raising Concerns (Whistleblowing)’ policy.

The post holder is responsible for ensuring that he/she maintains the integrity and quality of both
computerised and manual data.

Infection Control
All staff must at all times be aware of their responsibilities for ensuring infection control and to

The Trust is unable to accept responsibility for articles of personal property lost or damaged on
its premises whether by burglary, fire, theft or otherwise and staff are advised to insure against
all risks.

Safeguarding children and vulnerable adults


Kingston Hospital NHS Trust is committed to safeguarding children and vulnerable adults at risk
of abuse. If the post is one that involves access to children and vulnerable adults during the

course of their normal duties, an enhanced Criminal Records (CRB) check will be required. All
employees have a responsibility for safeguarding children and vulnerable adults in the course of
their daily duties and for ensuring that they are aware of the specific duties relating to their role .

Main Conditions of Service


The appointment will be on the basis of the new consultant contract laid down by the Hospital
Medical and Dental Staff Terms and Conditions of Service and General Whitley Council (2003).
Local arrangements have been reached and implemented by the Trust through a formal
negotiating process. Any subsequent changes will be notified to the successful candidate and
thereafter form part of his/her contract of employment. The arrangement of duties will be such as
may be agreed from time to time between the employing Trust and the person appointed.

It is the duty of a consultant to:

 Keep patients (and/or their carers if appropriate) informed about their condition
 Involve patients (and/or carers if appropriate) in decision making about their treatment
 Maintain professional standards and obligations as set out from time to time by the General
Medical Council (GMC) and comply in particular with the GMC’s guidance on ‘Good Medical
Practice’ as amended or substituted from time to time.

Dental Council (GDC) (Dental consultants only)

A consultant is responsible for carrying out any work related to and reasonably incidental to the
duties set out in their Job Plan such as:

 The keeping of records and the provision of reports


 The proper delegation of tasks
 Maintaining skills and knowledge

Consultants shall be expected in the normal run of their duties to deputise for absent consultant
or associate specialist colleagues so far as is practicable, even if on occasions this would involve
interchange of staff within the same employing organisation. This does not include deputising
where an associate specialist colleague is on a rota with doctors in training. When deputising is
not practicable, the employing organisation (and not the consultant) shall be responsible for the

need to the employer’s notice. The employing organisation shall assess the number of
Programmes Activities required.

Appraisal
Appraisal is mandatory. A number of consultants in each department have received training in
this process. One of these consultants will be identified as the successful candidate’s appraiser.
Training for appraisees will be provided. The appraisal process is in accordance with GMC
guidelines using GMC documentation.

Medical Clearance
Prior to employment we require a satisfactory report from our Occupational Health Department.

To enable the Trust to comply with Department of Health Guidelines for "Protecting Health Care
Workers and Patients from Hepatitis B", it will be necessary to provide evidence of Hepatitis B
status to the Occupational Health Department, in order for them to ensure that those involved in
"exposure prone invasive procedures" fit the criteria within these guidelines and to enable them
to offer continued protection to all medical staff.

Salary
The salary scale will be that applying to Hospital Medical and Dental Staff.
