21. Top Four Customers (buyers)
Personal documents:
a. Photocopy of the Computerised National Identity Card. The Relationship Manager shall obtain this copy, check with the original and document
the evidence of checking/verification on the copy of the CNIC.
b. One recent passport size photograph of the customers (i.e. individual, proprietor and all partners/directors).
c. Copy of any latest paid utility bill received at the residential address.
d. ‘Customer’s Basic Fact Sheet’ under the seal and signature of the customer as per approved format of the State Bank of Pakistan.
i. the director, major shareholder, holding 5% or more of the share capital of the Bank or
iii. an employee or
d) Partnership Mandate.
i) Copy of title documents and/or rent agreement of business premises where property is rented.
m) In other cases, financial statements of last year at least and preferably for the last three years signed by the customer. The same should be in
compliance with prevalent and relevant PRs.
q) List of directors with specimen signature of directors (in case of limited companies).
b) Certificate from the existing banker indicating the constitution of firm, name of the proprietors/partners and the date since the account is
being maintained.
a) Copy of complete set of title documents/other supporting documents of property to be placed with the Bank as security.
d) For already constructed property, non-encumbrance certificate / search certificate / NOC / permission to mortgage (whichever is applicable) to
be obtained. In case of newly constructed property, any of these documents should be obtained after 90 days of the completion of
construction.

• understand and undertake to pay the application fees/charges (non-refundable) and all other charges relating to external agencies for
service rendered
• clearly understand that I/we are liable to pay periodic rental by the due date, and failure to do so will lead to charity as per my undertaking.
• hereby give irrevocable undertaking to Bank Alfalah Limited-Islamic Banking Group to debit my/our account from time to time to pay off
charges relating to Takaful insurance, business and financial appraisal, excise duty, outstanding amount or any other charges applicable
• declare that all the information provided above is correct and complete to the best of my/our knowledge and belief. I/We also
authorise Bank Alfalah Limited-Islamic Banking Group or its duly appointed agent to contact anyone to obtain reference and to verify the
information provided by me/us in this application
• agree that the financing received will be used for activities which are deemed allowed by the Government of Pakistan and
Bank Alfalah Limited-Islamic Banking Group as per the environmental laws, rules and regulations
