This is an interesting article because EHR companies, like Epic and Oracle's Cerner, have made siloing data a major part of their business model. Overall, I like EPIC’s ability to review records from other EPIC shops. It also has a nod to interoperability with its “Care Everywhere“ feature. However, this is often a disorganized and outdated collection of information. Using it requires a ninja level of both tech expertise and medical knowledge possessed mainly by younger physicians and chief residents. I am a fan (and advisor) of companies like Zus Health focused on user-friendly, comprehensive, and up-to-date interoperable health care data tools.

Frameworks for procurement, integration, monitoring, and evaluation of artificial intelligence tools in clinical settings Here is a great resource by Mark Sendak and team just published in PLOS Digital health: https://lnkd.in/eeM743wG The use of artificial intelligence (AI) tools has seen exponential growth in multiple industries, over the past few years. Despite this, the implementation of these tools in healthcare settings is lagging with less than 600 AI tools approved by the United States Food and Drug Administration. In this systematic review, the authors organize and synthesize data and themes from published literature regarding key aspects of AI tool implementation; namely procurement, integration, monitoring and evaluation and map the extracted themes on to the Plan-Do-Study-Act framework. They found that currently the majority of literature on AI implementation in healthcare settings focuses on the “Plan” and “Study” domains with considerably less emphasis on the “Do” and “Act” domains. This is perhaps the reason why experts currently cite the implementation of AI tools in healthcare settings as challenging. Furthermore, the current AI healthcare landscape has poor representation from low and lower-middle-income countries. To ensure, the healthcare industry is able to implement AI tool into clinical workforce, across a variety of settings globally, we call for diverse and inclusive collaborations, coupled with further research targeted on the under-investigated stages of AI implementation. Really exciting news out of the Oracle Healthcare Summit in the US this week. Oracle Healthcare is promising a new #EMR “built from the ground up” to be released in 2025 and embedding #AI across the entire clinical workflow. Seema Verma told the conference that the new platform was not “a refurbished EHR”, saying that “in this day and age, you can’t leverage modern technology by bolting new innovation to something built in the 1990s”. This is spot on and what many in the #digitalhealth community have been saying for some time. We need a #nextgenEMR designed to embed AI across the whole workflow with modern tools for automation, documentation, and tasking that uses conversational AI to navigate and create tasks and documentation, to drive down the #clinicianburnout. 100% agree with her statement: “One of today’s most important and widely used healthcare technologies, the EHR, has not lived up to its promise.'' This is a must-read article on the future of #EMRs. I feel like disruption is long overdue and an exciting future is on the horizon for healthcare. Here are a few excerpts: "Health care has long been Big Technology’s great white whale. Apple, Amazon and Google parent Alphabet Inc. — all #innovators that have been unquestionably dominant in their respective main businesses — have each spent billions trying to disrupt the industry, only to scale back their ambitions when faced with the complexity and power of the established US medical system." While Oracle’s long-term vision for modernizing #HealthInformationTechnology is compelling, and has the potential to positively affect patient care, the company has faced significant setbacks — much like its technology peers. Oracle has ribbed Epic for being too expensive and running on a database it calls archaic and insecure. Epic, in particular, likes to brag about retention. One of the most popular presentation slides at the company is blank and titled “Customers Which Have Switched From Epic to Cerner,” “Since Oracle’s acquisition of Cerner Corporation, it’s been heads down to create this vision... Pleased to keynote the "Advancing Health Devices and Services in the Connected Home" event in DC sponsored by Consumer Technology Association and Connectivity Standards Alliance. Great event with folks from FDA Office of the National Coordinator for Health Information Technology (ONC), industry, and academia discussing this opportunity. I focused on how the home can be a catalyst for health & healing specifically through technological innovation. Healthcare these days is complex and can no longer consist of simply a doctor with a small medical bag visiting your home (e.g. Marcus Welby). However, the home can still be a principal site of care through careful process redesign, engaged clinicians, as well as current/emerging tech innovation--where point of need diagnostics/therapeutics and appropriate monitoring are delivered and reach the highest standard of care. Some of this tech already exists but we are only at the beginning of this opportunity to re-think the delivery side of healthcare through technological innovation. Interesting perspective on #TechnologicalDebt in health care by Dave Lamar focused on the provider market, but we seem the same issues within state depts and payers. While good data governance practices are key, there must be a better way to solve this common - if not universal - challenge in health care, if only by offloading some of the administrative responsibilities that require the individual management and administration of no-value-add administrative functions. #healthdata

Great post! My thoughts on the impact: Canada’s current push for national standards in data interoperability is promising, but the reality is complex. While the idea of seamless data sharing across provinces and territories sounds ideal, it’s challenged by Canada’s fragmented healthcare system, where each region operates under its own policies. Standardization could streamline data access, but if driven by large EHR vendors, it risks becoming too restrictive, potentially centralizing control and limiting true accessibility. In the end, while EHR systems often talk about interoperability, they mostly focus on data within their own platforms. This approach risks creating silos between hospital and community care, limiting the vision of a connected health network. True interoperability needs not just technical standards but also a commitment to open access that fosters an innovative and competitive market. We need drivers and incentives that constantly push us to strive for better care—not another solution that holds us hostage to outdated systems we’ve over-committed to.

Since pharmacy claims are paid claims at time of service, it is easier to use this data to identify behavioral insights and patterns to engage members. What I’ve seen working at Arine is that medical trend consistently goes down. AI is not only optimizing this it is also increasing outcomes faster.
