PATIENT CARE COMPETENCIES

A nurse must maintain current knowledge and skills in theory and practice based on the best
evidence available therefore the checklist for core competencies of the head nursing performance
evaluation must be strictly followed such as for patient care. First, we must monitor the patient’s
vital signs regularly, correctly and accurately next nurses must determine the number of patients,
the geographical locations and their conditions. We also use critical thinking strategies in
decision making based on the existing situation in the ward and nurses must be legible, accurate
and updated documentation of patient’s care in the chart so that the we can identify the priority
needs of our patient’s rendering care. We also must give accurate direction, instruction and
supervision to nursing students in the units and in charge of inventory of stocks, supplies and
equipment’s in the ward and checks the availability of medications and fluids of the patients, diet
list, laboratory request and Kardex, execute legal orders of physician accurately and timely. Also
implements nursing care and procedures such as vital signs, administration of medications,
changing of intravenous fluids and lab results. Must always observe cleanliness and orderliness
of the unit, endorse the ward/station and patients’ appropriate personnel, cooperate in the
reinforcement of hospital policies, utilizes the nursing process as framework for a systematic and
comprehensive nursing care, accomplishes a discharge planning and provide discharges
instruction which is highly based on institutional policies and prepares accurate documentation
for efficient communication. For health education we must assesses the learning of the patient
and the family and develop a health education plan based on assessed and anticipated needs so
that we can evaluate the outcomes of health education. These are essential to patient care
competencies that we must use so that we can give a better care to a patient.

EMPOWERMENT COMPETENCIES

Being empowered means that you have the ability to effectively motivate and mobilize yourself
and others to accomplish positive outcomes in your practice and work environment. When you're
committed to enhancing your knowledge base and incorporating evidence-based practice
research findings, you become a positive change agent and can serve as a role model for your
peers, supervisors, and the community. The final outcomes of nursing empowerment are
increased workplace satisfaction, enhanced nursing autonomy, and improved patient outcomes.
For ethico-moral responsibilities we have to respect the rights of the clients and we should have a
commitment, concern and dedication to punctuality and adheres to policy and we must consider
the culture and religions of the patient rendering nursing care. For the legal responsibilities we
must practice in accordance with the nursing law and other relevant legislation including
contracts, informed consent. And for personal and professional development we must have
positive attitude towards change and criticism and must always show professional image of the
nurse that complies with the rules and regulations of the course concerning uniform and
paraphernalia and adheres to clinical requirements like nursing care plans, quizzes, journals,
attendance and case studies.

ENABLING COMPETENCIES

For enabling competencies nurses must identifies appropriate quality improvement


methodologies for the clinical problems, detects variation in specific parameters like vital signs
of the client from day to day and report significant changes in clients’ condition/environment to
improve stay in the hospital and also solicits feedbacks from client and significant others
regarding care rendered and must share with the team relevant information regarding clients’
condition and significant changes in clients’ environment, and encourages the client to verbalize
relevant changes in his/her condition. And also, must performs daily check of clients’ records or
conditions and document and record all nursing care and actions implemented so we can report
to appropriate persons significant variances or changes and occurrences immediately and
documents and reports observed variances regarding client care. Must also mean that we should
give an objective and accurate report on what was observed rather than an interpretation of the
event and provides appropriate suggestions on corrective and preventive measures and
communicates solutions with appropriate groups
