Project: Optimizing HIV Investment for Impact

Job Title: Community Prevention Officer

Job Summary:
This position is responsible for assisting in the delivery of community mobilization, PMTCT services, RH,
HCT Services, improving retention, intra/inter-facility referrals in supported health facilities overseen by
the organization in order to achieve organizational goals. Also, he/she would oversight Case Management
and Key Population activities.

The GF’ Optimizing Investment for Impact in HIV and TB in Nigeria will help reduce the HIV and TB burden
in Nigeria where PHRI will be implementing the grant in 5 states.

Relationship: National Office team and Zonal officers.

Prime Health Response Initiative Formerly known as Hygeia Foundation is implementing Global Fund
to fight AIDS, TB and Malaria, (GFATM) Project under the New Funding Model implementation in 5
states as Sub-recipient (SR) under the Family Health International (FHI360) who is the Principal
Recipient (PR) for HIV/AIDS component of the Global Fund.

PHRI is charged with the responsibility to provide Treatment Adherence, Care and Support services
to clients in all Global Fund supported facilities in 5 States which are; Osun, Ondo, Oyo, Ekiti and Ogun
states.

Job Description:
• Lead in the community mobilization activities in the zone by providing technical support to the
supported ART facilities, CBOs, NGOs whose volunteering services will lead to demand creation
for the program in the zone.
• Provide technical support and closely monitor all PMTCT/ RH services in the PHCs and the
community surrounding them leading to service uptake.
• Ensure the facilities register all pregnant women both within and outside the facility for ANC and
provide testing and counselling.
• Work closely with the adherence counselling unit to ensure that patients eligible for ART/PMTCT
are properly prepared and followed up.
• Also ensure all babies born to the clients get EID testing and positive cases referred for ART
immediately.
• Assist to collect, collate and disseminate quality data to the M&E unit to ensure appropriate data
analysis. Help revert back to the facility in case of data verification.
• Attend state led meetings on behalf of PHRI and any other meeting that is relevant to the
program. Work closely with the State HIV officers in areas of relevant meetings with stakeholders.
• Liaise with the STO –PCT and STO-Clinical in dealing with difficult cases and situations
• Ensure complete client referrals to and from the facilities; Make sure commodities are made
available in all supported facilities
• Work with the zonal team to facilitate referrals and adherence services to all positive clients are
ongoing in all facilities including the SHCs; link all PLWHA to other support services within the
communities such as OVC, nutrition, legal services.
• Prepare and submit reports such as the weekly, monthly and quarterly reports, with others such
as trip and field reports
• You will report to the Zonal Manager for all programmatic and administrative purposes. However,
technical direction will be given by the different technical units in the national office especially
the community services unit manager.
• Perform any other official duties that may be assigned from time to time by the zonal team lead.

Key Performance Indicators/ Performance Goals:


• Ensure tabular review and analyses of the community and prevention unit performance weekly
• Ensure Case Managers update the defaulter and tracking lists per ART site and KP Friendly sites
weekly
• Conduct linkage and referral analyses in collaboration with the M/E and Technical unit monthly
• Seek local/international funding opportunities to support program activities.

Qualification
• BSc. in any basic degree, HND, Registered Nurse, or related social science degree.
• 5 years’ postgraduate with relevant working experience in HIV management.
• Good knowledge of HIV AIDS
• Good understanding of the HIV/AIDS Psychosocial issues
• Strong Interpersonal/ communication skills
• Good Counseling skills
• Must have good understanding of Ms packages
• A good community referral network knowledge
