One of my team wrote this blog post for Complete Care at Home. I thought I'd share it here! The bottom line is - people are happier, healthier, and live longer when they get to stay home. And Complete Care can make that happen in Santa Barbara County, and beyond! https://lnkd.in/gcBubxaE

The mission of public health is to promote safer, healthier and better-quality lives, prevent disease and injury, and ensure all individuals have the right to access all the facts, information, and knowledge available to make healthy choices for themselves, their families, and their community. In partnership with communities across the United States and around the world, the CDC Foundation works to improve health outcomes by developing effective programs and tackling systems. Health is more than the absence of disease. It is not just about surviving. It’s about thriving. This Juneteenth, let us celebrate the resilience in our communities and all reinvigorate our commitment to everyone living their healthiest life. Together, we can reinvent our systems and practices so everyone can reach a better quality of life. #Juneteenth Image Credit: National Museum of African American History and Culture. Don't be fooled. Value-based care won't come to you! You need to go to value-based care! How? By weaving together the principles of data, population health management, operations, finance, and change management. Yes, it's challenging, but it's worth it. If you're at the annual #FACHC conference in late July, Carrie T. and I will show you the how, and hear from Syntax Health and Austin Helton about the #VBC #contracting, #data and the impressive results we've achieved. #VBC #ValueBasedCare #valuebasedhealthcare #data #PopulationHealthManagement #pophealth #healthcarefianance #change #changemanagement

How can we help people live healthier lives? We have to think about whole health, which is influenced by the communities where we live, work, and play. That’s why we created the Community Connected Care program to assess people’s social needs and connect them to much-needed resources. Today, on International What Matters Most to You? Day, let's reflect on the power of shifting our approach to care. Moving from asking "What's the matter with you?" to "What matters to you?" opens doors to transformative conversations between caregivers and individuals. Prioritizing meaningful dialogues can lead to shared understanding and support, ensuring that everyone's values are respected throughout their care journey. 🏥⚕️👩🏻⚕️🩵#WhatMattersMost #HealthcareConversations #InternationalDay
Today, we celebrate "What Matters to You?" Day (WMTY) 2024. Inspired by a 2012 New England Journal of Medicine article titled "Shared Decision-Making: The Pinnacle of Patient-Centered Care," written by Michael Barry and Susan Edgman-Levitan, WMTY is intended to shift the power to the person who knows best about the help or support they need, whether it be a person with a medical issue or the clinicians or staff providing care. This international day of celebration aims to encourage and support more meaningful conversations between caregivers who provide health and social care (including family caregivers) and those they are or will care for in the future. Smart move - this should be the key question that guides us all in interactions whether it's a patient, client, staff member or customer. To create person centred health responses understanding what matters to the individual is KEY. #whatmatterstoyou #whatmatters #understanding #personcentred #humancentreddesign #socialcare
