pub: 22 - Oct 2024 Abstract Olfaction may play a restricted role in human behavior, yet paradoxically, its absence in anosmia is associated with diverse deleterious outcomes, culminating in reduced life expectancy. The mammalian nose serves two purposes: olfaction and breathing. Because respiratory patterns are impacted by odors, we hypothesized that nasal respiratory airflow may be altered in anosmia. We apply a wearable device that precisely logs nasal airflow for 24-hour-long sessions in participants with isolated congenital anosmia and controls. We observe significantly altered patterns of respiratory nasal airflow in anosmia in wake and in sleep. These differences allow classification of anosmia at 83% accuracy using the respiratory trace alone. Patterns of respiratory airflow have pronounced impact on health, emotion and cognition. We therefore suggest that a portion of the deleterious outcomes associated with anosmia may be attributed to altered patterns of respiratory nasal airflow rather than a direct result of lost odor perception per se. Insights on Objective Chronic Cough Assessment Cough bouts, bursts, fits, epochs, spasms... - all suggest multiple expiratory cough efforts clustered together. How should we define multiple coughs to gain meaningful insights? 1. Research Findings: cough researchers at The University of Manchester analyzed data from 47 chronic cough patients to investigate clinically relevant cough bouts (≥2 coughs). Using various time windows (0.5s to 3.0s), they found that a 3.0-second window most accurately reflects patient-perceived severity (evaluated by visual analogue scale). The figure below shows a detailed comparison of correlation coefficients and total bout numbers across different time intervals. Takeaway: longer time windows may better capture the true impact of coughing on patients' lives. 2. CoughPro Application: at Hyfe, we have developed a consumer wellness grade application for smartphones (CoughPro), used by over 100,000 registered users globally. Based on user interviews, feedback, and analysis, we currently defined cough bursts as any group of ≥4 coughs without a gap of more than 5 seconds between them. Our CoughMonitor Suite - a purpose-built smartwatch system, a companion app for smartphones, and an investigator's dashboard - allows our investigators to experiment with cough statistics and investigate daytime vs. nighttime coughing, coughing episodes (e.g., bursts), time intervals without cough, and more; correlating longitudinal cough data with other objective and subjective data streams is also super interesting! As cough gains attention as a powerful biomarker in longitudinal patient monitoring, and with continuous passive privacy-preserving monitoring being feasible, academics, drug developers, and R&D teams in consumer-grade companies are exploring various cough metrics to better correlate with subjective perceptions of severity, intensity, and quality of life impact. There are many 'known unknowns' and likely even more 'unknown unknowns' in this field.
