Oklahoma is one step away from becoming the next state to grant full practice authority to nurse practitioners! Senate Bill 458 has passed the Oklahoma Legislature and is now waiting for Gov. Kevin Stitt's signature before it becomes law. Meanwhile, Pennsylvania is part of the minority of U.S. states that do not allow nurse practitioners to practice to the full extent of their scope and training. "We continue to monitor, support, and raise awareness of why full practice authority is important to modernize the licensure laws to allow nurse practitioners to care for and provide resources for their patients," said Stephen Ferrara, DNP and President of AANP. "We know it will increase access to care." Dr. Ferrara recently spoke about the top goals, priorities, and challenges affecting NPs in the upcoming year. "Some of the opposition to full practice authority is the mistaken idea that it will allow NPs to do more than they are licensed to do. However, in states that require a regulated contract between an NP and MD—states without FPA—NPs are unable to provide care or prescribe medications without maintaining a physician contract," Dr. Ferrara continued. "Retiring that unnecessary bureaucratic requirement is an issue that appeals to a broad coalition of organizations. The state Senate passed a bill allowing licensed practical nurses (LPNs) in hospice settings to make death pronouncements. This bill, introduced by Sen. Lynda Schlegel Culver (R-27), expands the current law in Pennsylvania. RNs, medical doctors, physician assistants, and coroners were previously the only authorized professionals for such pronouncements. In most of the country, nurse practitioners are able to deliver medical services for which they are licensed as independent providers. But in states like Pennsylvania, outdated and restrictive laws require NPs to be supervised by — and surrender part of their incomes to — physicians in order to do the work they've been trained and certified to do. To measure the effect these laws have on patients, we can look at nearby states that have already passed full practice authority legislation. Researchers found that counties in restrictive practice states bordering full practice authority states experienced a 20% decline in the number of NPs. Neighboring New York, Maryland, and Delaware already offer NPs full practice authority. It's clear that in order to keep talented NPs caring for patients here in Pennsylvania, we need to make PA an attractive state for nurses and NPs to work. The U.S. will face a shortage of up to 86,000 physicians by 2036. Primary care and family medicine are already experiencing the impact of this shortage as fewer physicians are going into primary care. At the same time, the U.S. population is increasing and growing older as people continue to live longer. Between 2013-2019, health care visits delivered by nurse practitioners and physician assistants in a year increased from 14.0% to 25.6%, according to an analysis from Harvard Medical School researchers. Nurse practitioners and physician assistants now account for a quarter of health care visits!
