Sharing my thoughts on how UC Davis Health is working to decrease costs by reducing unwarranted variations in patient care. Leveraging artificial intelligence to provide accurate and meaningful data to illustrate patients’ problems to clinicians will be key. As I noted in this article below: “AI has the power to aggregate vast amounts of data and demonstrate the impact of different care pathways, informing nurses and physicians about the optimal way to deliver care for each patient. With AI providing constant surveillance of practice variations, maybe it won’t take many years for improved care algorithms to make their way into daily practice. Newest episode of #HealthAndVeritas with guest, Mitesh Rao, MD, CEO of OMNY Health talking about democratizing access to healthcare data and improving health through more reliable information. Harlan Krumholz talks about "The business case for quality: estimating lives saved and harms avoided in a value-based purchasing model" an article in Health Affairs Scholar that returns us to the uncomfortable domain of asking why our "system" doesn't do more to save lives and improve health outcomes. I talk about Marijuana. I am concerned that we are watching a slow moving catastrophe due to the desire to decriminalize and reduce controls over Cannabis and THC-containing products (which is good) while paying too little attention to quality and regulations. There appears to be a "Superstar Effect" in healthcare and biomedical AI research right now. A small percentage of publications are truly novel methods contributions that introduce new capabilities or behaviors; whereas a majority of contemporary research publications are focused on implementation science to assess the real-world utility and performance of new methods. This dynamic seems different from prior eras, when the translational infrastructure for implementing novel AI methods was largely unavailable. While methods research often captures more attention and accolades, both types of research are critical – novel methods without implementation science lacks translatability; implementation science without novel methods lacks novelty. If our goal is to invent and deploy transformative AI methods in real-world healthcare and life sciences contexts, we ought to continue encouraging and rewarding stellar implementation science work alongside breakthrough methods development.

📣 AI in Healthcare Survey Results 📣 " . . . The survey [from Wolters Kluwer] of 100 U.S. #physicians found that 40% are prepared to use #generativeAI during #patient interactions at the #pointofcare. Additionally, 68% of physicians said they have had a change of opinion over the last year and are now inclined to believe that generative AI would be beneficial for #healthcare." We need adopt AI in ways that minimize risk and keep the clinician in the loop as the final arbiter of care. As adoption becomes more prevalent (as evidenced by the 68% surveyed changing their opinions), AI will be integral to effective #carepathways, improved #patientoutcomes, and greater #clinician #jobsatisfaction.

Deloitte Health - 2024 MedTech Digital Innovation Report a valuable report to validate the focus and value of digital innovation to MedTech Notes: ✍ 100 MedTech executives in the States, Europe, and Asia joined the survey. ✍ 3 themes: 1️⃣ measuring digital innovation maturity 2️⃣ modernizing operations, products, and customer experiences 3️⃣ adopting digital business models I'm highly interested in the values of MedTech in healthcare (theme 2&3) and address the highlights as follows. 👑 The top 5 use cases: 1️⃣ digital clinical trial (74%) 2️⃣ digital therapeutics (74%) 3️⃣ smart factories (89%) - automation 4️⃣ patient health and experience (68%) 5️⃣ customer service augmentation (66%) 👑 The top 5 values: 1️⃣ better patient/customer experience (76%) 2️⃣ greater connectivity of devices (eg. remote monitoring) (57%) 3️⃣ new product development (55%) 4️⃣ new digitally-enabled "as a service" business models (53%) 5️⃣ more accurate and predictive analysis of post-launch real-world data (43%) 💡 Digital health - 46% of leaders address digital health as their most important area of technical focus, including delivering new products, therapeutics, or services to engage patients. Only 16% of responders said that they are contemplating B2C approaches in addition to the traditional B2B model. 💡 Business models: ☝ 21% of leaders have made significant investments into growing other services: SaaS, HaaS, DaaS, and subscription. ☝ 29% of responders mentioned that they leveraged traditional channels to sell products rather than pursuing new business models. 💡 XaaS (Everything as a service) model: What it is: Bundle products such as hardware, devices, equipment, and consumables, providing value-added services. Example: Connected medical devices continuously collect and interpret data, predict events and behaviors, and recommend personalized next-best actions, incorporating real-time insights and predictions. As promised here are my thoughts from Day 2 at the FDA's Digital Health Advisory Committee meeting on "Total Product Lifecycle Considerations for Generative-AI Enabled Devices." I have pretty much focused my thoughts on the talk and remarks presented by Dr. Keith J. Dreyer as he summarized it all very eloquently. He was truly inspiring! Here are my top 5 takeaways: 1️⃣ Continuous Clinical Validation is a Must: AI devices, especially in radiology, need ongoing validation to ensure they remain accurate and effective in real-world settings. 2️⃣ Regulating Generative AI for Patient Safety: There's a pressing need for clear regulations to guide the safe deployment of GenAI in healthcare, focusing on transparency and human oversight. 3️⃣ Expanding the FDA's Role: Keith emphasized that the FDA should look beyond manufacturers to include device users for better monitoring. As he put it, "We have to keep the data pristine to make sure these devices are working, independent of what the manufacturer makes." 4️⃣ Cost-Effective Monitoring Solutions: Recognizing that monitoring and validation can be costly, we need innovative solutions that make this feasible for all healthcare settings, big or small. 5️⃣ AI's Potential to Boost Efficiency: Generative AI could greatly enhance efficiency in radiology. Keith noted, "If there was a way to improve our efficiency through AI, and I think Gen AI has a way to be able to do that, then you're going to see availability, much broader availability of even specialists, to do more work and to work beyond the scope of what they can do today." "The preservation of individual autonomy and privacy is the essence of freedom." In the rapidly evolving world of healthcare, AI has emerged as a transformative force, promising to enhance patient outcomes, streamline processes, and democratize access to care. There’s still much work to be done before we truly get to that place – but as we embrace these technological advancements, we must also confront a critical underlying debate: the balance between autonomy and control. Isn’t that what it’s really about as we consider deployment of such tools? AI in healthcare offers remarkable potential for empowerment—enabling patients to receive personalized care, facilitating earlier and more accurate diagnoses, making it easier for them to get a clinical care while at the same time,  also providing healthcare professionals with cutting-edge tools to make informed decisions. If done properly, it’ss a shift that could democratize access to quality care, breaking down traditional barriers and expanding reach to underserved populations. However, this empowerment comes with a trade-off. The more AI integrates into healthcare, the more data it collects—raising significant concerns about privacy, consent, and control. Who owns this data? How do we ensure it’s used ethically, without infringing on the autonomy of individuals? The challenge lies in striking a balance. We must harness AI’s capabilities while safeguarding the fundamental rights of patients. We need to explicitly address who’s in control? This requires thoughtful discussion around regulation, ethics, and the preservation of personal autonomy as well as professional education. The future of AI in healthcare will depend on how well we manage this delicate balance. Collaborative Care not Hybrid or In-Person Care Virtual First is the framework to transform patient experience and outcomes using data-driven innovation to reimagine health and healthcare operations. This will impact staffing, facilities, technologies, and policy as collaborative care changes the frequency, nature, and value of care interactions. VF goes beyond optimizing encounters or hybrid care; it's about utilizing the most appropriate care mode to address needs at any point in the patient’s health journey. Health is not a single event or a series of disconnected occurrences. Traditionally, healthcare has been plagued by silos within and across systems, specialties, and care modes, resulting in fragmented workflows, communication, and data accessibility. Even within offices, experiences can be episodic, lacking continuity, leading to redundant questions and unnecessary steps. By leveraging data and insights, Virtual First supports seamless care continuity through ongoing collaboration, regardless of care mode. Virtual touchpoints are standard and expected. Good system design incorporates and utilizes virtual tools effectively for the patient, clinical staff, and payers. Virtual First encourages ongoing collaboration to support daily health decisions and needs, shifting focus away from the limited time people spend as "patients." The goal is not merely to offer convenience but to improve health and healthcare delivery by using the most appropriate mode of care, provided by the right person, at the right time, for the right need. Rather than debating the value of virtual vs. hybrid vs. in-person, Virtual First emphasizes collaborative care. Data supports care delivery by any mode at each stage of the health journey. Collaboration can occur between patient-provider, among providers, healthcare team-patient, with caregivers and others. Technology and data allow patient and clinical needs to start to be addressed at the earliest opportunity. Each collaboration informs the next most appropriate step, moving towards a more proactive and person-centric model. By design, Virtual First enables nudges, reminders, and communications as part of a trusted health relationship, not an unfamiliar service. The same virtual tools can be used for chronic disease management, acute care, mental health, preventive initiatives, and risk prevention without data silos or redundancy. Over time, the data collected will support the refinement of evidence-based practices, moving beyond current underperforming care models. Let's not simply reengineer current processes or create new silos. By embracing Virtual First, we can reinvent healthcare to not only be more responsive and efficient, but to address the increasing challenges of access, outcomes, equity, and cost. #VirtualFirst #ReInvent

A new Epic, Mayo Clinic, and Abridge integration is set to develop a new resource for nurses leveraging genAI, to improve workflow and reduce admin burden. 👩‍⚕️ "Abridge's AI-powered platform transforms patient-clinician conversations into real-time structured clinical notes, addressing more than 50 specialities." 🗣 Emily Barey, vice president of nursing at Epic, said the collaboration "holds the potential to rethink how nurses document in an electronic health record today by bringing the voice of the patient and the nurse directly into the record." 🔀 Abridge's solutions, according to this article, are already at many prominent AMCs.
