S&P: Medicare Advantage's growth a bad omen for hospitals - n8: and Many Health Systems Continue to Participate Under the Terms Dictated by the MA plans, Enabling this Abuse “Even beyond rates, MA adds more administrative and liquidity hurdles for providers. S&P pointed to MA plans’ “key strategy” of prior authorization requirements, which force providers to absorb associated refiling costs and sometimes keep patients locked in a longer hospital stay. That burden is exacerbated by insurers’ generally “high” denial rates, S&P wrote, which have led to instances where smaller providers without the resources to fight claim denials terminate their MA plan contracts and become more vulnerable to closure…. The report also notes that many MA plans have longer pay cycles than traditional Medicare. Less Care at Higher Cost—The Medicare Advantage Paradox This is a must-read article for all those in healthcare as well as all taxpayers who fund Medicare. The following quote in this article published in JAMA sums up why Medicare Disadvantage plans should be abolished, devoting efforts to strengthening traditional Medicare. It won't be too long before I am eligible for Medicare, I know I am not going to subject myself to the hassles of MA plans We think the time has come to declare MA a failed experiment and abolish it. That would allow redeploying the $88 billion taxpayers will overpay MA this year to upgrade benefits for all Medicare beneficiaries.2 But we recognize that even with that infusion of funds, constraints on low-value (and particularly no-value) care in Medicare might still be needed, and appropriate; no insurer should pay for harmful or useless treatments. And Medicare should have a preferred formulary covering all beneficiaries; when multiple equally safe and effective drugs are available, the least expensive should be the default option, with opportunities for off-formulary coverage when clinical needs (eg, allergies) dictate. However, coverage decisions should be driven by science, not corporate bottom lines, and they should be implemented fairly, consistently, and transparently—something that, by nearly all accounts, MA plans fail to do. Moreover, healthcare network limitations are not necessary; free choice of hospital and physician is something all Canadians already have. Health Care Professional Feedback Health Problem and Rationale Healthcare data breaches reveal a troubling trend in the industry. The incidents have an impact on patients, organizations, and professionals at alarming rates despite efforts to adopt and upgrade data security solutions. According to Bohn and Schiereck (2022), the demand for data-driven clinical practices requires hospitals to keep pace with the influx of sophisticated threats. The healthcare sector reported 337 data breaches in the first half of 2022 (McKneon, 2022). Similarly, there were over 19 million records implicated in healthcare data breaches within the first six months of 2022. The average cost of a breach is about $10.1 million, which reveals the extent of the problem and its implications on the future of health care (McKneon, 2022). The problem highlights the need for robust administrative, physical, and technical programs to identify and intercept risks. Boh and Schiereck (2022) indicated that current and former employees trigger increased breaches due to unauthorized access to databases and sharing of sensitive information with third parties. The targeted files contain details of patients’ identification, emails, appointment information, social security number, health insurance, billing information, and addresses (Lee & Choi, 2021). Failure to respond to incidents has serious implications on reputation, financial stability, and patient satisfaction. Enhancing security and monitoring capabilities is a strategic priority appropriate for the organization's commitment to maintaining a superior competitive advantage. Assessing and Measuring Issues for Quality Improvement The measures of health data breaches include frequency of occurrence, the common types in terms of percentages, and cost to patients and the organization. The metrics guide organizations to complete a risk assessment to understand the type of data affected, the number of people affected, and the harm caused by the breach (Argaw et al., 2020). The benchmarks for measuring performance include hiring adequate skilled IT security professionals, enhanced employee responsibility and accountability when handling patient information, and significant investment in technologies such as firewalls to mitigate data breaches. The goal is to ensure that the organization has enough resources to identify and resolve data breaches involving unauthorized access, loss, and theft of patient data (Lee & Choi, 2021). Assessment includes discovering processes necessary to prevent an incident from happening in the future. Blood Conservation: ASCs are adopting blood strategies to minimize intraoperative blood loss. Techniques such as controlled hypotension, which lowers blood pressure to reduce bleeding, and the use of antifibrinolytics, drugs that prevent the breakdown of blood clots, are common. Additionally, surgical techniques and the use of advanced devices help surgeons operate with precision and less bleeding. Preoperative Assessment: A preoperative assessment is vital in identifying patients at risk of excessive bleeding. ASCs utilize detailed medical histories and laboratory tests to gauge a patient's hemoglobin levels and clotting status. By doing so, they can tailor their approach to each individual, optimizing blood management before the patient enters the OR.
