Honored to share the results of Joslin Insight's work creating a positive work environment at Los Angeles General Medical Center with Nancy Blake, Daryl Joslin, and Ria Sangalang. Thank you to the entire team and LA General's wonderful directors of nursing and nurse managers. Our recent #NurseLeader article, “Enhancing Nurse Satisfaction and Retention Through Real-Time Feedback: A Case Study at Los Angeles General Medical Center,” highlights how real-time, anonymous feedback can drive cultural change and improve nurse engagement. The print version will appear in the October edition of Nurse Leader. Our IRB-approved study reveals that LA General saw a 9% reduction in nurse turnover and improved clinical outcomes. I want to commend Nancy Blake for the work she is doing. While multiple factors contributed to this success, I’m humbled that a key factor was the Joslin Insight App, which provided leadership with the feedback needed for informed decision-making. Changing culture isn’t easy, especially in health systems, where the focus can often shift toward compliance rather than genuine engagement. However, our national studies show that truly listening to nurses and addressing their needs is not only a simple and vital solution but also a fundamentally human one. The ANA Workplace Survey, which we helped conduct, revealed that 11,442 nurses ranked this among the top three for improving work satisfaction, alongside increasing pay and staffing. Grief and loss are a personal and sometimes confusing experience. Most people are familiar with the 5-stages of grief as described by Elizabeth Kübler-Ross, but did you know this has since evolved to 7-stages? The 7-stage model is widely accepted as more inclusive and accurate to what people experience. Physicians devote their careers to improving and preserving their patients’ health and longevity. But along with the success and fulfillment physicians often derive from practicing medicine, grief and loss permeate the profession as well. In the traditional culture of medicine, the latter emotions are not just undiscussed, but also poorly understood. Congratulations to Rhafia Bucoy, DBA, MSN, RN, RN-BC, Regional Clinical Practice Consultant, Perioperative Services, NCAL Kaiser Permanente, recipient of The DAISY Foundation DAISY Nurse Leader Award.   Rhafia Bucoy is NCAL Perioperative Services’ clinical practice and financial performance consultant. She serves as an OR nurse informaticist and analyst, and she is definitely everyone’s “go-to person” for everything Perioperative data, clinical staffing, OpTime build, and all other things. Rhafia is the nurse in the background who silently gets things done, from report build to data mining, analysis, and presentations. She is the clinical liaison to the national teams for KPHC, national product council, and revenue cycle. She is responsible for the clinical testing and auditing of electronic med record documentation.   Despite the overwhelming demand for reports and other competing priorities, Rhafia manages them well and always willingly helps others. She has collaborated with revenue cycle in closing out unposted surgical logs and ensuring accurate implant documentation. She has also recently agreed to close out records related to bedside procedure recovery to ensure accurate capture of workload and patient throughout in the post operative space.   Rhafia serves as the clinical lead and resource for all of NCAL’s surgical system administrators as she lends her expert consulting hat in the OpTime domain from case creation to case closure and posting. Rhafia partnered with supply chain on the implementation of Case Pick App in 19 NCAL facilities, 11 of which went live in 2023. This application helps accurate accounting of supplies used in each surgical case leading to cost valuation and eventual supply standardization. With a few more sites going live in 2024, procedure card maintenance is key and Rhafia recognized this gap in some locations. With this information, she took the initiative to reinvigorate and develop training modules on procedure card updates. She partnered with the national training team and launched the first facilitator-led training in January of 2024.   Another important work that Rhafia is leading is the implementation of the Preop and PACU bay assignment to capture both medical and surgical workload in the perioperative spaces to inform staffing decisions. Her collaboration with the workforce and finance teams is critical in this important work.   Rhafia is a self-directed and a silent nurse leader in the Perioperative space. Her contributions to KP NCAL nursing can easily go unnoticed but definitely worthy of a recognition. In a busy senior care facility, staff members faced a challenging incident‼️ They were renowned for their dedication to the well-being of their residents, not only by employing dedicated staff, but by leveraging technology to ensure safety, communication, and personalized care – but early one morning an emergency alert echoed through the facility. A cherished resident was experiencing a sudden health scare. “It is about patients, but the dollars follow the patients,” #OHSU struggles with a 47% drop in transfers from other #hospitals which are typically #patients seeking more lucrative specialized #procedures. #Hospital beds are really precious in Oregon (49th in the country per capita supply). #ED clogged with non-urgent non-lucrative cases. Better access to #UrgentCare & #CaseManagement might help. It’s a community-wide issue bigger than OHSU alone. What happened to that cadre of patients who were not transferred to OHSU over the last 5 years? Got acceptable care elsewhere? Waited longer in the OHSU specialty inpatient queue? Suffered more from shunted or delayed access? Does anyone know?

Reducing Hospital Readmissions Many patients discharged from a hospital return within 30 days for the same or related health conditions. This can rack up unnecessary expenses and cause further health issues - both of which are preventable. It's helpful to meet with the hospital discharge planner shortly before you are set to leave the hospital to make sure you have everything you need for a safe transition home. This includes all necessary medications, directions on how the medications are to be taken, specific instructions on physical limitations and dietary recommendations. Make sure to obtain information regarding follow-up physician visits, so nothing is overlooked. Patients often have trouble performing the regular activities of daily living following a hospital discharge. Injuries and ailments may make it difficult just to get out of bed and take care of personal hygiene and nutritional needs. It might be necessary to get help with these needs and other home care tasks following discharge. Some daily living activities caregivers can assist with following hospital discharge include: Bathing and dressing Going to the bathroom or incontinence care Laundry and linens Dusting and vacuuming Meal planning and preparation Errand assistance Attending follow-up appointments Extra care will allow you to focus on resting and recovering, rather than home and personal care tasks. Receiving extra care is also advantageous after fall injuries, as many people strain themselves and prolong the amount of time it takes to recover. Having assistance with tasks can greatly factor into reducing hospital readmissions and ease your mind, as everything is still being taken care of.
