Login to your patient portal at myhealth.push.purdue.edu or on the PUSH website at purdue.edu/push. Once logged in, click the forms tab to locate the immunization information form.

All students must pay a $35.00 processing fee regardless of where immunizations are received. This fee will be posted in the students bill. Please make a copy of this form for your records before returning it to Drexel University.
