How do you learn from your mistakes and failures as a leader?
As a Medical Director I gave a presentation that was a walk through my medical career, describing error, and unintended harm. A story about me, my actions, my contribution, in each building I worked in. I thought it would raise confidence in my colleagues to engage more openly with learning from error, and harm. A colleague said I was being brave. On interview panels for potential consultant posts I always asked the candidate to recount a mistake during their training. It seems they all had perfect work and training experiences…but they knew someone somewhere who… In a profession where we know patients get harmed, it isn’t about being humble - it is about getting to a point where it is safe to be open and honest. It is life or death.

Awarded after passing the requisite exams. Did not continue the College subscription once the FRCPCH was established, hence the (MRCP) annotation. The MRCP is the only way to denote the exam success and diploma award. It’s an ancient guild thing.

Improving a clinical investigation process - almost eliminating error

The median error rate for the DMSA investigation in children was 6 per 30 (20%). Using a systems improvement approach, we have reached a median error rate of 1 per 30 (3%) - a sustained shift change evidenced by the run chart. This equates to a more efficient use of £300,000 per year. We are aiming for a zero error rate with the next PDSA cycle.
