Malayattoor Yearly report

Mehac Foundation is a non-profit organisation registered as a charitable trust registered

under the Act Of registration of deeds. We are committed to improving the quality of life of
people with mental illness and their families, by providing active total care, delivered by a
highly skilled multi-disciplinary team. We provide comprehensive psychosocial care to
people from disadvantaged backgrounds through community clinics and build strategic
partnerships with local partners and train a wide network of community mental health
volunteers. Currently providing clinical service in four Districts of Kerala where majority of
our beneficiaries are from Ernakulam & Alappuzha district. Mehac has been able to create a
space for mental health at the grassroots level, take a proactive role in advocacy and
awareness issues and enable preventive care, treatment and rehabilitation.

Mehac started providing clinic services in association with Malayattoor Grama Panchayath
and Primary Health Centre from 2019 onwards. A total of 94 patients have registered with us
since 2019, of which, a number of patients have dropped out. The reasons for the dropout
included difficulty in accessibility to the clinic, death of the patient, shifting patients to
rehabilitation centres and poor treatment adherence. Currently we have 51 patients in active
follow up. The clinics are conducted every 2nd Friday of the month. At present we are
conducting one homecare every month along with a palliative care team from the health
centre. We also have 3 DHC once every 4 months. Currently our home care services are
only need based and we are providing home care for 20 patients. However, we also do
frequent tele follow ups. Continuing partnerships with LSG members, different community
stakeholders, hospital staff and ASHA workers help us to facilitate timely intervention for the
beneficiaries.

ACTIVITIES DONE

1.CLINICAL SERVICES

As mental health remains a sector where individuals often hesitate to seek assistance, and
where services are frequently scarce, delivering appropriate medical help can be
challenging. Recognizing this need, the clinic was established in collaboration with the
panchayat, with the primary aim of providing free and easily accessible mental health
services to the general public. While initial reluctance stemmed from stigma and lack of
awareness, significant transformation has occurred over time. The increasing number of
individuals availing the clinic's services stands as a testament to this positive shift.

The Mehac clinic comprises a multidisciplinary team including psychiatrists, psychologists,

psychiatric social workers, and social workers, all adept in providing holistic care.
Each month the clinic is organised on the 2nd Friday with prior discussion with palliative
nurses and hospital staff by the clinic coordinator. Subsequently the clinic coordinator
informs each patient about the time and date for the clinic. From April 2023 to March 2024
we conducted 11 clinics and did 250 consultations.

2. TELE FOLLOW UP

Tele follow up is done by our team, where each patient is followed up at least two times in a
month. Through this follow up we enquire with the patient and their family members about
the present condition of the patient. If there is any assistance needed for the patient or
family, the team provides timely services for them.

3. HOME CARE SERVICES

Many individuals face barriers in accessing healthcare services, often due to physical
limitations, especially prevalent among the elderly population. Additionally, those with mental
illness may encounter challenges, such as social anxiety or depression, which hinder their
ability to seek assistance in unfamiliar environments.

Recognizing these obstacles, the Mehac team has implemented regular home care
initiatives facilitated by palliative nurses, ASHA workers, and doctors . This proactive
approach has proven invaluable, providing essential mental health services directly to
individuals in need within the comfort of their homes. Not only does this benefit the patients
themselves, but it also alleviates the burdens placed on their families.

4.CAREGIVERS SUPPORTING GROUP

A caregiver support group offers a vital network of emotional and practical assistance for
individuals caring for loved ones. These groups provide a safe space to share experiences,
exchange advice, and find solace among peers facing similar challenges. By fostering
understanding and solidarity, caregiver support groups empower participants to navigate the
complexities of caregiving with greater resilience and confidence. Mehac team has
conducted regular Caregiver support in the clinics on different relevant topics such as Anger
management, Relaxation techniques, Side effects and usage of medicine, Illness and its
effects on the patient, Stress management, Sleep hygiene, Behavioural modification of
patients and Crisis Management.

5.COMMUNITY PROGRAM and PARTNERSHIPS

Community programs play a vital role in promoting mental health and well-being within
society. These programs offer essential support, education, and resources to individuals
facing mental health challenges. By fostering a sense of belonging and understanding, they
reduce stigma and isolation associated with mental illness. Moreover, community programs
provide accessible avenues for early intervention and treatment, ultimately improving overall
mental health outcomes for individuals and communities alike. Through collaboration and
outreach, they create a supportive environment where individuals can thrive and lead
fulfilling lives despite mental health struggles. Mehac team has donated an Oxygen
concentrator for the palliative patients of Malayattoor, which was handed over to the Medical
Officer. We conducted different awareness sessions for the ASHA workers as they are the
primary point of communication from the community and clients. The sessions were on
Elderly Mental health and Disability Welfare benefits. We also celebrate Christmas and New
Year with the community by distributing Cakes for clients and staff at clinics and while
conducting Home care visits.

Future Plans

● Conduct monthly Field Visits with the palliative nurse.

● Mental Health awareness sessions for ward members.
● More awareness programs for different groups/stakeholders.
● Plan to create support groups for different categories of people coming to
the clinic.
