    1) A potential safety issue was identified where loss of gear selection feedback on certain Amphion drawworks control systems could result in over-travel of the traveling assembly and contact with personnel and equipment on the drill floor. 2) The issue was caused by loss or failure of the high gear selection proximity switch generating a Category 2 stop that may allow excessive travel depending on conditions. 3) NOV recommends immediately checking affected rigs listed in Appendix A and contacting a NOV service center to request a software update to address the issue while ensuring proper functioning of proximity sensors.

    Assemblies: Reference Appendix A – Affected Rigs

    Objective: To advise affected NOV customers of a potential safety concern in the event of
    loss of gear selection feedback.

    Issue: NOV has been advised of an incident where a Loss of Gear Selection feedback
    initiated a Category 2 Stop condition resulting in over-travel where the travelling
    assembly contacted the drill floor.

    Loss or Failure of the High Gear Selection Proximity Switch generates a


    Category 2 stop of the drawworks, which may result in excessive travel
    depending on the elevation and velocity of the traveling assembly while the
    drawworks control system takes action. It is possible that the travelling
    assemblies may come into contact with personnel and equipment on the drill
    floor in these circumstances.

    This condition may result in death, bodily injury or property damage if not
    immediately addressed.

    Solution: NOV recommends customers to immediately review the affected list of rigs
    within Appendix A. If affected, customers should contact their local National
    Oilwell Varco (NOV) Service Center to request an updated software release. In
    the interim period, NOV recommends that the high and low gear proximity
    sensors are checked for security, correct installation and functionality.
