Visit to Gohira Shantirdip Union Health Centre

Introduction
Union Health Centre is the union level public health facility for quality primary
health care services. There are 7 union sub centres . As a part of our RFST program
we got an opportunity to visit Gohira Shantirdip Union Health Centre in
Chittagoing with our assigned teachers Dr Sayeed Mahmud & Dr. Saeeda Raisa.
According to Schedule we reached in the Union health center at 9:45am. We were
warmly welcomed by medical assistant Mrs. Mithali Halder & MLSS Mrs. Jannatul
Ferdous. Mrs. Mithali Halder briefed us about the activities & facilities available
in the center. It remains open from 9am to 3pm (6 days in a week).

Community based health care (CBHC) and Union Health Centers

Bangladesh has achieved notable progress on different health parameters during the
last 2 decades in comparison to the neighboring countries and many other
developing countries of the globe. Bangladesh has achieved MDG- 4 before the
stipulated time and are very much in track on MDG-5 in spite of low health budget
and many other constraints. The country is now in SDG era. In Bangladesh 4th
HPN Sector Program is on board for a period from January.2017 to JUNE, 2022.
For the 1ST 5.5 Years of SDG, CBHC will contribute substantially in achieving
SDG as of MDG and will continue till the achievement of SDG by 2030. Under 4th
HPN Sector Program the dimension of CBHC has been extended further. In short
it consists of the activities of union health centers (the flagship program of the
present government) and ESD (essential health services from Union health centers
to all health facilities within the Union).

3. To observe the strengths of the organization
4. To know about the special services given by them
5. To observe the limitation of the organization

Function of the organization


1. They provide only out-door services
2. They give treatment to the patient with fever, common cold, diarrhea (mild)
etc.
3. (35-40) patients per day come to the health care for the treatment.
4. They provide & types of medicine with free of cost.
5. They give treatment to the patient of mild condition. In severe condition
they refer the patient to UHC
6. They provide treatment free of cost
7. They measure BP & temperature
Structure:
• 3 ROOMS
• One for MA and MLSS
• one for pharmacist
• One for MO

Drugs available:
1. Paracetamol
2. Salbutamol
3. Histacin tablet
4. Antacid tablet
5. Albendazole BP 400 mg
6. Amoxicillin Capsule 500 mg
7. metronidazole 400 mg
8. ORS
9. Vitamin B complex tablet

Environment
• Poor ventilation
• Poor Sanitation
• Insufficient water supply
• Area is small
• Total environmental condition is poor.

Limitations
• The patients are not admitted.
• There is no emergency bed available.
• They are unable to take care of patient in critical condition.
• No injectable medications are available.
• Domiciliary health services are not offered.
• No diagnostic facilities
• No indoor service is available.
• Inadequate manpower.
• There is no pharmacist present, and the MA is in charge.
• There is no ambulance service for emergencies.

Observation
• Poor Environmental condition.
• No boundary around the organization
• (35-40) patient come here for the treatment
• No bed for emergency purpose
• They register the name of the patients & give the treatment.
• No poster for health education & creating awareness of people.
• They provide the treatment with free of cost no need to buy the tickets.
• They provide service from 9:00am to 3pm.
• They don't provide the full course of all drugs to the patient except
antibiotics

Recommendations
• All facilities for emergency referral should be available.
• The environment should be upgraded.
• They should have at least one emergency bed available.
• There should be more manpower available.

Conclusion
The first and foremost goal of Union Health Centre is to provide primary health
care to the people of union. To maintain this the staff of the centre are facing many
obstacles and challenges. Local MP, community and political leaders, people of
administrative level all are trying to help overcome these challenges.

We hope that in near future the vacant post of the centre will be filled and its
activities will run smoothly overcoming the challenges.
