    RELEASE WAIVER QUITCLAIM


    And UNDERTAKING

    I, __________________, of legal age, Filipino, single/married and a resident of


    ____________________________________________, Philippines, after being sworn to in
    accordance with law, deposes and states:

    1. That I am previously employed as __________________ of (name of company) located

    2. I voluntarily resigned and submit my resignation letter on ____________________ and


    the same was received by our Human Resource Manager;

    3. I have been paid all the wages, remunerations, and benefits due me by the

    4. Therefore, I am hereby waiving and hereby releasing (name of company) from


    any and all obligations due me and arising out of my employment with the said
    Corporation;

    5. In consideration of the foregoing, I, for myself, successors and assigns, do hereby


    release, waive and forever discharge (name of company) and/or any of its
    owners and staffs from any and all actions that may now exist or may hereafter
    exist or develop on account of said resignation/termination.

    6. As such, I finally make manifest that I have no further claim(s) or cause of action
    against (name of company), and/or any of its owners and staffs and forever
    release the latter from any and all liability.

    7. I hereby undertake that all sensitive and confidential information with regards
    to the operations of (name of company), whether directly given or otherwise
    obtain by virtue of my duties shall keep in strict confidence and shall not be
    disclosed to any third party;

    8. I am executing this affidavit to attest to the foregoing facts and for whatever
    legal purpose this may serve.
