INSTRUCTIONS- Fill one form out for EACH dog registered. Send the form and the ANNUAL FEE
of $10 for EACH dog submitted to: Tiffin Bark Park Registrations, 870 West Market Street, Suite 69,
Tiffin, OH 44883. Checks should be made payable to FRIENDS OF THE TIFFIN BARK PARK.
Requirements for registration into the Tiffin Bark Park are as follows: Current 1 yr. or 3 yr.
Rabies vaccination, current 1yr. DH(L)PP or 3 yr. DHP and have a fecal performed within the
past 3 months. If your dog cannot receive regular vaccines, please provide proof of protective
titer levels. Dogs MUST be vaccinated by a veterinarian to enter into the Tiffin Bark Park for
their own protection and for the protection of other dogs.

FREQUENTLY ASKED QUESTIONS ABOUT THE NEW SECURITY SYSTEM


How will the new system work? The Tiffin Bark Park Security System will be two fold. Security cameras will record any
movement in the park. In the event of a serious situation, we will have the ability to view the video file.
Who installed the system? The Friends of the Tiffin Bark Park funded the system and its installation. It was designed
and installed by Sound Solutions of Tiffin.
How do I get access to the Tiffin Bark Park in the future? During the initial roll-out period, the Tiffin Bark Park will
remain open, as it has been. Once the system is activated (DONT WORRY!- We will tell you when that is.) you must
have a Tiffin Bark Park Key Fob to unlock the gating system.
How do I register for a Tiffin Bark Park Key Fob? You will need to take the form on the reverse side of this sheet to
your veterinarian. You fill out your name and information. Your vet must verify and fill in when your dog had their last
vaccinations. Your vet MUST also sign that they feel your dog is socially prepared to be among other dogs off-leash in
the Tiffin Bark Park. You then mail the form and the $10 ANNUAL registration fee (per dog) to the Tiffin Bark Park Office
listed on this form. You will receive your registered Tiffin Bark Park Key Fob in the mail in approximately 5 business days
from the day we receive your form.
What if I have multiple dogs to register? You will need to fill out one sheet and pay $10 ANNUALLY for EACH dog you
register. You will receive only 1 Tiffin Bark Park Key Fob per household.
What if I lose my Tiffin Bark Park Key Fob? You will need to contact us to void your old fob and issue a new one.
There is a $10 replacement fee for each fob.
Can I give the Tiffin Bark Park Key Fob to someone else to use? The fob is registered to YOU and your dog(s). The
system keeps track of who uses the park and when they enter. Someone may bring your dog to the park with your fob.
But they may NOT bring unregistered dogs into the Tiffin Bark Park. If they do and use your key fob, your fob can be
deactivated and you will no longer be able to use the Tiffin Bark Park.
When will the locking system be activated? We will give plenty of notice as to when the system will be locked down. It
will be posted at the park.
Why does it take 5 business days from the day of receipt to get the Tiffin Bark Park key fob?
We need to enter your information into the system and encode your key fob. Every fob has a unique code that is
registered to YOU.
How do I volunteer to help out at the Tiffin Bark Park? We are in the beginning stages of organizing a Friends of the
Tiffin Bark Park volunteer group. Stand by for more information.
How do I contribute to the Friends of the Tiffin Bark Park? The Tiffin Bark Park is maintained and serviced with NO
CITY FUNDS. Only with your donations can we continue to maintain and upgrade the park. Feel free to send your
