COMPLIANCE

GUIDE
REGISTRATION PACK
Please complete the following forms and bring these with you completed to
your interview:
Application Form
Tick once complete
Please complete ALL sections of the application form and sign all declarations.
CV dated to age 18.
Tick once complete
We require an up to date CV covering the last 18 years of your working history. Any
gaps of employment should be on your CV including dates and reason for gap i.e.
unemployment/holiday/maternity leave etc. All gaps must be explained. Please use a
MM/YYYY format throughout CV and please email to us in Word Format.

Skills checklist
Please ensure you complete the appropriate checklist for your speciality, remember
to sign the bottom of the checklist
Online DBS Form
Tick once complete
Please refer to the enclosed instructions and complete the online DBS form as soon
as possible. Once your compliance consultant has the necessary Identification
documents we will be able to process your Hays DBS.
Medical Questionnaire (Only required once offered) Tick once complete
Please find enclosed a Medical Questionnaire. This form only needs to be completed
and returned to your compliance consultant once you are offered your first placement
with Hays.
Induction Confirmation Form - Declaration

Please read the Induction Handbook and complete the Induction Confirmation form.
Only the Induction Confirmation form needs to be returned. The handbook is yours
to keep and refer to
Terms and Conditions
Tick once complete
There are 3 different terms and conditions enclosed in our registration pack. Please
complete the appropriate form depending on how you wish to be paid:
PAYE candidates (Pay as you earn): please complete the attachment with the titleTerms and Conditions PAYE
or
Candidates with an Umbrella company: please complete the attachment with the titleTerms and Conditions Umbrella Company
or
Candidates with their own Limited company: please complete the attachment with the
title Terms and Conditions Limited Company. You will also need to provide us with a
copy of your certificate of incorporation.
Referee Details (3 years)
Tick once complete
We require references from your current and previous position. Referees must have
supervised/ managed you. Please provide as much contact information as possible
for both referees eg ward/department, contact number, work email address.

References must cover the last 3 years employment history without any gaps

INTERVIEW GUIDE
Once your registration pack has been returned we will arrange for you to
attend an interview day.
Below are the documents that we require for your compliance, please bring
these to your interview to be verified and copied: (Please note that even if you
send us copies of the following before your interview date we will still need to see the
original documents on the day of your interview)
ID Documents
Passport & VISA
Tick once complete
We must take copies of your original passport and, if appropriate visa and supporting
home office letters.
If your passport has expired, please bring this with you to the interview. If you do not
renew your passport we will require additional ID documents such as your Birth
Certificate, Driving Licence (Counterpart and photo card required) and/or adoption
certificate. Please note that if all you have is your UK birth certificate we will also
require a copy of the back of your bankcard showing your signature.
National Insurance
Tick once complete
We require a proof of your National Insurance number, this can either be a copy of
your National Insurance Card, your P60 / P45 or a letter from HM Revenue and
Customs with your NI number on it. Please take the original proof to your interview.

Please provide us with documentary evidence of all name changes i.e. marriage
certificate, birth certificate or deed poll certificate (change of name deed)

Name Change

2 x Proof of Address Letters


Tick once complete
We require two proofs of address in order to process your DBS. Acceptable POAs
are: Utility Bills, Bank Statements, and letters from the government i.e. HMRC,
Council Tax. Please ensure the bill or letter is dated within the last three months.
For HMRC and Council tax it can be dated in the last year. Your DBS will not be
processed unless we have this information. The DBS will not accept general letters
as proof of ID. We can accept both parts of your Driving Licence as long as the
address matches your DBS application. This must include your photo card and
counterpart licence.
Qualifications Certificates
Tick once complete
We need to see and take copies of ALL original higher educational qualification
certificates that have been listed on your CV. Eg Nursing Diploma, ODP NVQ

NMC renewal letter and Original


Statement of Entry Letter (Nurses Only)

Tick once complete

We require a copy of your NMC card as well as your Statement of Entry (this would
have been sent to you when you first registered with the NMC). Indemnity insurance
is also advisable. Your original statement of entry can be requested from the NMC
free of charge. [email protected]

We require a copy of your current HPC certificate. Please make sure that new copies
are sent to us every time you renew your HPC registration.

HPC (AHP & ODP)

Basic Life Support, Manual Handling, Fire


Tick once complete
Safety and Safeguarding Children Training
This training must be completed annually, please take any original certificates you
have to your interview if they are dated within the last year. Please note Basic Life
support and Manual Handling must be practical sessions. If your training has expired
we will contact you once we have received all relevant paperwork to arrange training.
Our courses range between 40- 50 which covers all the mandatory training.

Immunisations

Tick once complete

Please provide evidence for the following:


HEP B
Your Hep B primary course date and:
If you have had a Hep B vaccination within the last 5 years we require evidence of
your titre level or
If you had your Hep B vaccination over 5 years ago we require proof of your Hep B
booster and a titre level of any year in-between the original vaccination and booster
or If you have not got proof of your Hep B booster please provide us with your titre
level within in the last year.
VARICELLA
Please complete the Varicella self-declaration form. (Please note Month and year are
both required) If you do not recall having chicken pox then we will require proof of
immunity to Varicella either through a blood test or evidence of two doses of the
vaccine
TB
Evidence of immunity to TB, either a BCG scar verification or a grade 2 Heaf Test or a
positive Mantoux test reading (6mm-15mm). Please note that all BCG scar
verifications must be completed by your GP or an occupational health Nurse to
confirm you have a BCG scar.
MMR
Proof of 2 MMR vaccinations or a serology showing immunity to Measles, Mumps
and Rubella
For EPP roles (Scrub nurses/ODPs, A&E nurses and Midwives)
We will require serology for HIV, Hepatitis C and Hepatitis B surface antigen dates
within the last 5 years. These blood test must be Identity validated samples (IVS)
and this must be stated somewhere on the result.
2 Passport Photos
Tick once complete
We require two passport-sized photographs so we can issue you with a Hays
Healthcare ID Badge.

Please bring as many of the above documents as possible to your interview. Failure
to do so may result in delays in the compliance process.
