In the field of cardiology, we understand that early intervention is key. In this video highlighting Phillips Foundation's partnership with the World Heart Federation, we see how access to ultrasound technologies can enable effective screening and early diagnosis, preventing life-threatening events and improving patient outcomes. It is heartening to see this in action in APAC, starting with the Philippines, helping children and young people with rheumatic heart disease, along with medical training and greater support for healthcare workers. The addition of dapagliflozin to standard care for critically ill patients and acute organ dysfunction did not improve clinical outcomes; however, confidence intervals were wide and could not exclude relevant benefits or harms for dapagliflozin. The primary outcome was a hierarchical composite of hospital mortality, initiation of kidney replacement therapy, and ICU length of stay through 28 days, analyzed using the win ratio method. Secondary outcomes included the individual components of the hierarchical outcome, duration of organ support–free days, ICU, and hospital stay, assessed using bayesian regression models.

Revisiting Race and the Benefit of RAS Blockade in Heart Failure A recent meta-analysis published in JAMA sheds light on the effectiveness of renin-angiotensin system (RAS) blockers in heart failure patients across different racial groups. Let's dive into the findings and their implications for patient care! Interactive Questions & Answers: Q1: What did the meta-analysis examine regarding RAS blockers? A1: The meta-analysis examined whether the benefit of RAS blockade in patients with heart failure with reduced ejection fraction differs between Black and non-Black patients. Q2: What were the key findings of the study? A2: Black patients had higher rates of hospitalization and death. The rate of reduction in the primary outcome (cardiovascular death or first hospitalization for heart failure) was comparable between Black and non-Black patients taking RAS blockers. However, Black patients had a smaller relative risk reduction in hospitalizations for heart failure. Case Study & Answer: Case Study: Maria, a 65-year-old Black woman with heart failure, was prescribed RAS blockers by her cardiologist. Despite her adherence to the medication, she experienced several hospitalizations. Her cardiologist reassured her that while her relative risk reduction in hospitalizations might be smaller, the absolute benefit of the medication is significant given her higher baseline risk. Q: How did the RAS blockers benefit Maria despite her higher hospitalization rates? A: The RAS blockers provided an absolute benefit by reducing the risk of cardiovascular death and hospitalizations, crucial for her given the higher baseline risk associated with her condition.
