Buying your medication online is very convenient, but we have to be careful. The National Association of Boards of Pharmacy (NABP) found that: 💭Around 95% of websites selling prescription drugs might not be operating legally. Some online pharmacies could be scams, which is why it’s good to stay informed. Here are some helpful tips to keep in mind before making a purchase: 1. Know Your Seller Always check the seller's physical address and licensing status. A quick search can reveal negative reviews. 2. Shop Licensed U.S. Pharmacies Only Ensure the pharmacy requires a prescription and has licensed pharmacists available. Avoid sites that don't meet these standards, as they may sell counterfeit or expired products. 3. Trust Reputable Sources Rely on government (.gov) and nonprofit (.org) websites for reliable health information. 4. Watch for Fraud If a deal seems too good to be true, it likely is. With many pharmacies, especially big-box retail chains, closing down, it’s more important than ever to be vigilant about your medications. For those taking multiple medications, drug interactions are a serious concern. In fact, one of the most common causes of death or medical complications in the U.S. is due to harmful drug interactions. We’ve seen clients who are on 8, 12, 14, or even 20 different medications prescribed by six or seven different providers. While these prescriptions are well-intentioned, they can sometimes work against each other. Unfortunately, doctors often don’t have the time to thoroughly review potential contraindications, and electronic medical records may not always catch them. If you’re on multiple medications, make sure to visit a physical pharmacy and have the pharmacist review your prescriptions for any potential contraindications. An interesting map from The New York Times shows how many pharmacy deserts – zip codes with no pharmacy – have been created since 2015. Apparently, nearly 800 zip codes that had at least one pharmacy in 2015 now have none. A lot of the closures are from independent pharmacies. One of the principal problems on the prescription side is that they are, increasingly, reimbursed at too low a rate by pharmacy benefit managers. Sometimes the reimbursements do not even cover the wholesale cost of the drugs. Faced with this, many drugstores have simply opted to close. The New York Times does a great job of breaking down the issues and problems with PBMs and their impact, especially on poorer and more rural areas. The FDA has issued a small business exemption for small dispensers (Pharmacies) with less than 25 full-time Pharmacists and Pharm Technicians. These small dispensers have until November 27th, 2026, to comply with the enhanced drug supply chain security requirements. If you are a Small Dispenser that qualifies for the exemption, you may not have to Collect and receive Serialized data (EPCIS) Conduct a Product verification on the Suspect product (VRS) Conduct a Product Tracing on a suspect product. However, the rest of the DSCSA regulations will still be enforced by November 27th, 2024. Those requirements include: Collecting and saving all Transactional (T3) data Reconciling all Drugs with your Transactional (T3) data Saving Transactional (T3) data for at least six years Verifying Trading Partners Require Procedures and Policy documentation Identifying and investigating any suspect product Reporting Illegitimate Products to the FDA with Form 3911 Obtaining GLNs Confirming all smallest salable products have Product Identifiers Overall, pharmacies must be prepared for the complex regulations and strive for full compliance by the November 27th, 2024, deadline. upnrx.com

Why drug prices keep rising . Over the past decade, the cost to employers for mail order prescriptions has risen to more than $200 billion. This is another instance where a plan to cut prices leads to the opposite effect. Employers now pay 3-4 X the cost of generic drugs sent to employees by mail, compared to getting the drugs at brick and mortar pharmacy stores. It was assumed that filling prescriptions by mail using generic drugs and 90 day supply would cut costs. No one saw markups coming
