This approach is cost-effective and aligns with current healthcare trends towards community-based care. Additionally, it offers regulatory and financial incentives, such as those under CMMI’s Primary Care First initiative, making it a strategic move for hospices looking to improve value-based, comprehensive care and reduce hospital admissions. starting today, we are implementing "Martha's Rule" in several of our inpatient wards to enhance patient care and safety. This initiative allows patients or their family members to request an additional medical review if they are concerned about a patient's health condition deteriorating and feel that their worries are not being adequately addressed by their care team. This policy is designed to empower patients and their loved ones to have a more active role in healthcare decisions, ensuring that their voices are heard and their concerns are taken seriously. Studies have shown that patient involvement in healthcare can lead to improved outcomes and increased patient satisfaction. By introducing "Martha's Rule," we aim to create a more responsive and patient-centered healthcare environment.

National Healthcare Decisions Day is a perfect opportunity to celebrate and honor the importance of proactive healthcare decision-making. This day serves as a reminder for individuals to consider their healthcare wishes and preferences, especially in terms of medication adherence. By actively participating, individuals can take steps to ensure that their healthcare decisions align with their values and goals, including decisions related to medication management. Medication adherence is a critical aspect of healthcare decision-making, as it directly impacts treatment effectiveness and overall health outcomes. We encourage individuals to reflect on their medication adherence habits and consider how they can improve their self-management practices. This may involve developing strategies to stay organized with medication schedules, seeking support from healthcare professionals or caregivers, or exploring new technologies or resources to enhance adherence. For individuals who may have difficulty managing their own medications, this special day also provides an opportunity to discuss and plan for caregiver support. Identifying trusted caregivers who can assist with medication management ensures that loved ones receive the necessary support to adhere to their treatment plans effectively. Healthcare Decisions Day encourages open and honest communication among family members, caregivers, and healthcare providers to ensure that everyone involved is informed and empowered to make decisions that promote optimal health and well-being. So lets all stop and take the time to reflect on our loved ones and how we can be proactive in completing advanced directives to remove the burden from our loved ones. #healthcaredecisionsday #medicationadherence
