The sessions provided valuable insights and updates on key topics such as standard precautions, pain management, patient rights, and much more. We take pride in equipping our caregivers with the tools and knowledge the need to provide the highest standard of care. By staying educated and informed, our team continues to demonstrate unwavering commitment to the well-being and dignity of our clients. Some thoughts on Aidan Fowler’s comments (reported in HSJ today) re major patient safety inquiries:- Yes, let’s keep moving towards a system where learning & responding to the needs of those harmed through patient safety problems happens in real time…. …but we aren’t there yet & until we are, inquiries into major problems where the system has failed to intervene & where patients, families & staff are left without answers remain essential… However, are repeated inquires into individual service failures (and the frameworks currently used) effective? I’d think the answer is clearly not. I’d argue that a different model of national level investigation is needed - a model that incorporates restorative approaches to listen & respond to the needs of patients, families and staff - one that looks at best practice (nationally and internationally), and where there is a clear pre-established methodology and framework that means the process can be undertaken much more rapidly that currently is the case. Finally (I’ve said this before) - the model should incorporate ongoing independent oversight & scrutiny on the implementation of recommends for change. None of this would be hard to do if the will was there.

A new consultation on a framework for patient safety opens today. The need for transparency and data driven decision making are notable. I would have liked to see something on event analysis, victim handling and incorporation of human factor practice, but perhaps the principles will evolve after feedback.
The Patient Safety Commissioner has launched a public consultation on a draft of "seven principles of better patient safety" They seem very sensible and it's hard to disagree with any of them. The Clinical Human factors Group have responded to the consultation supporting the principles while stressing the importance of human factors science and systems thinking to ensure they don't just remain as principles but become part of the way work is done and the way care is delivered.
