“The conventional wisdom is that the U.S. healthcare system is broken, but that assessment is misleading. After all, can a system that has never truly existed be called broken? All the components of fabulous healthcare exist in abundance in the United States, yet with few exceptions, they have never been organized to work together…A more useful assessment is ‘U.S. healthcare is fragmented,’ and the right follow-up question is ‘How do we organize those pieces into a system that can use its components to meet patient’s needs?’…In the absence of true organization into delivery systems, however, providers often do not communicate with each other or coordinate their efforts…physicians are divided into increasingly narrow subspecialities defined by organs and diseases…This fragmentation is familiar to patients who feel that the healthcare system has forgotten that they are people, not just diseases…in a single year, the typical Medicare enrollee sees 7 different physicians. These 7 physicians include 2 different primary care physicians and 5 specialists who collectively worked in 4 different practices…most physicians do not have a stable population of patients for whom they feel totally responsible.” Thomas H. “Pay-for-Performance is unlikely to be the road to salvation for the healthcare system, but it might be the entrance ramp to that road. In pay-for-performance, providers are paid on a fee-for-service basis, but with some portion in the payment, either through withholding or an additional incentive payment, contingent on quality- and efficiency-related performance targets…the limitations of this approach have become clear. Providers tend to focus their improvement efforts on the specific targets, with surprisingly little improvement in spillover in other areas…In order to pursue meaningful pay-for-performance incentives, providers must be organized…the relatively modest incentives that characterize most pay-for-performance programs will not be enough to motivate providers…Prospect Theory, the developers of which won the Nobel Prize for Economics in 2002, argues that relatively small incentives attract disproportionate attention, particularly when framed as potential losses instead of bonuses…the annoyance of ‘leaving money on the table’ is more important than the bonuses themselves.” Thomas H. In any leadership role, but particularly for a CHRO, it's essential to recognize the influence they have and the responsibility that comes with it. As we all know, Human Resources isn't just about managing policies or payroll, it's about stewarding the most valuable resource of all: people. CHROs must be thoughtful about how they use both their human and organizational resources. Using resources wisely is a critical responsibility. This includes, championing fairness, investing in people, and allocating training, development, and wellness resources wisely. Every dollar and hour invested in the workforce should be aimed at long-term growth and employee well-being, not short-term gains or budget cuts. The CHRO position has the ability to drive change, build cultures, and create lasting impact, but only if resources and authority are used responsibly. It's not about power; it's about stewardship. 🚨 Workplace incivility is on the rise, and SHRM’s recent research highlights some concerning trends. According to an article in SHRM’s quarterly magazine, workplace conflict is growing across all environments—whether on-site, hybrid, or fully remote—and if employers don’t take action, the consequences could be severe. A key insight from the research? Team members are twice as likely to leave their roles within a year if they believe their workplace is uncivil. But what does an uncivil workplace look like? It can be subtle but harmful: talking to people disrespectfully, interrupting or silencing others, and excessive monitoring or micromanaging—whether face-to-face or via virtual tools—are all common expressions of incivility. These behaviors damage the culture we’re building and undermine retention, team morale, and productivity in every type of workplace setting. As leaders, it’s crucial to address these issues head-on to create a respectful, supportive environment. “We have spent much of our resources and energy over the last 15 years creating the potential for such coordinated care-for example, getting all our physicians on the EMR (Electronic Medical Record), creating the ability for patients to communicate with us via secure email systems…Yet neither our doctors nor our patients are fully and effectively using these systems…Clinical information can improve adjustment for the severity of the illness, but there is no gold standard for how much risk adjustment is enough…the fear remains that public rankings of performance might produce perverse effects…Critics of public reporting worry that public rankings of individual physicians could influence decisions in ways that are not in patients’ best interest. The risk of dragging down ‘performance’ might inhibit physicians from attempting an audacious treatment strategy when audacity is exactly what a patient wants and needs…published data in 2008 concluded that public reporting does lead to quality improvement activities in hospitals-yet the impact on actual patient outcomes is not clear.” Thomas H. Strengthening staff competence and cultivating an inclusive work culture are the key benefits of DEI training in healthcare organizations. According to the 2023 State of Healthcare Training and Staff Development Report, 35% of workers noticed an enhancement in workplace culture following such training.
