The (Healthcare) Ground Beneath Our Feet… This year it is likely that over $10 billion will be invested in the digital health sector – a robust amount, no doubt, but inconsequential when compared to the amount of capital tied up in healthcare real estate assets, which is estimated to be $1.2 trillion by Jones Lang Lasalle. Of that amount, approximately $790 billion is in the hospital sector while the remaining $490 billion is in medical office assets. It is estimated that there are 48.6k facilities with 3.4 billion square feet of healthcare real estate, 42% of which are owned by REITs and another 33% held by other private investment vehicles. Much of the digital health investment is meant to make the physical healthcare infrastructure more productive: greater operating efficiencies, greater patient throughput, more relevant personalized care models with better outcomes. As the healthcare system is being pushed to be more distributed to lower cost sites of care, accelerated now by the rapid proliferation of automation and AI capabilities, the potential impact on healthcare infrastructure assets will be profound. I’ve had the chance to go deep with a number of digital health alum, healthcare mentors and #corporate innovation partners over the past few days. A few themes to share:   1. Digital will change healthcare. Period. How is hard. But it will happen and the best are continuing to push ahead. 2. What keeps companies in the S&P 500 for 50 years or more? Partnerships and innovation. This is why large systems must practice the hard art of both partnership and partnership towards innovation. For startups, partnership reps may be more important than brand. 3. There is a difference between needs and behaviors that startups need to understand, especially in Healthcare where incentives, process, practices are complex. Finding who pays early is not only essential but existential. 4. Don’t optimize for valuation, optimize for value. Over the long-run, this is what will matter. In Healthcare, today value will have to tie to ROI. Pharma, providers, payors are under pressure. As we reflect on the state of digital health in 2024, it's evident that the COVID-19 pandemic has fundamentally transformed healthcare delivery. The rapid adoption of telemedicine during the crisis has become a permanent fixture, with over 60% of physicians continuing to utilize this technology. The sprint towards smarter healthcare, catalyzed by necessity, has shattered preconceptions about digital-first health experiences. Virtual care is no longer a stopgap but an integral part of a seamless, anytime, anywhere health ecosystem. 🌍 We're witnessing the rise of B2B2C models in remote consultations, with companies like Teladoc Health and Amwell partnering with insurance providers to integrate their services into health plans. Public sector collaborations, like eConsult Health 's partnership with the NHS in the #UK, underscore the potential for remote consultations to enhance efficiency and accessibility at a national level. The digital health landscape is evolving rapidly, with advancements in remote consultations, mental health support, at-home diagnostics, and patient monitoring. AI and generative AI transform care delivery, making it more efficient, personalized, and patient-centric. 🤖 Yet, challenges remain. While venture capital funding for digital health has stabilized after the pandemic boom, it remains below its 2021 peak. Sectors like digital therapeutics face reimbursement hurdles, and we've seen major players like Walmart and Optum close telehealth businesses. Looking ahead, we anticipate further consolidation in digital health, driven by acquisitions to create broader ecosystems encompass telemedicine, mental health, and other specialized services. As healthcare leaders, we must leverage these digital advancements to improve access, enhance patient outcomes, and drive efficiency in our healthcare systems. I’m excited to share what we've been obsessing over for the last six months! Introducing Threshold Health, a unique patient engagement platform that puts healthcare operators in the driver's seat of modern care gap closure campaigns. Shockingly, only 1 in 20 US adults is up to date on their recommended preventive care, such as cancer screenings or blood pressure testing. This statistic is paradoxically low, especially given the increasingly direct financial incentives tied to increasing population-level care quality. When my longtime friend and now cofounder, Dr. Doug Murad, and I started exploring why patient engagement is so low, we quickly discovered a recurring pattern. Time and again, we heard from passionate operators across diverse healthcare organizations who struggled to mount effective patient engagement campaigns, often waiting until the last few months of the contract year to close important care gaps in their patient populations. In almost every case, these operators seemed to be building campaigns from scratch, deploying blunt tools like bulk SMS, physical gift cards, and manual call centers, and relying on lagging indicators of success. Unsurprisingly, these efforts were operationally expensive and often fell short of their targets. Dr. Doug and I aim to empower healthcare operators level up their patient engagement with campaigns that are cheaper, better, and faster than building something themselves. To do this, we’re building a self-serve platform that makes it easy for operators to rapidly launch and iterate on modern campaigns that integrate well with their existing care coordination workflows. If we’re successful in this first step, we will have a vehicle to deliver an increasingly sophisticated library of campaign templates and next-generation patient engagement technology. So far, we’ve developed a prototype that allows operators to go from zero integration to a modern deployed campaign in fewer than 10 clicks. We’re excited to run our first pilot in mid-July and continue developing the product with 2-3 launch partners through the rest of this year. There's a lot left to learn -- I plan to post regular updates and lessons here. Reaching this level of conviction took a lot of time—not just from me and Dr. Doug, but from numerous experts across various fields who have been extremely generous in sharing their expertise and feedback. I’m a lucky man - I get to talk about stuff I care about with people that feel the same way, but know how to approach it in fresh ways. This chat with Josh Au Yeung goes into another area I think needs much more attention - leadership. NHS providers recently published data showing only around a third of NHS CEOs have a clinical background: https://lnkd.in/givrbU_p . When evidence suggests that expert leadership is strongly associated with better outcomes, happier staff, and greater productivity, we need to work harder to identify, train, and support current and future clinical leaders. I’m optimistic about the potential of generative AI to reduce provider burnout and improve provider-patient interactions. However, that optimism is tempered by the data that indicates the technology is not quite ready yet. Bright Ventures is interested in digital health founders who understand the realities of provider-patient interactions, and the role technology can play in strengthening clinical outcomes. Had a blast diving into Amir Dan Rubin's extensive healthcare career on Vital Signs. Amir previously served as the CEO of One Medical and Stanford Health Care, was an EVP at United/Optum and is now the Founding Managing Partner at Healthier Capital. Our conversation spanned payer-provider integrations, lessons learned from leading One Medical, applications of generative AI, and more. Some highlights: ⚡Combining care models We talked a lot about the One Medical/Iora combination. Amir argues that just because two care delivery approaches may occupy the same physical location (e.g., a clinic), that doesn’t mean they necessarily need to conform to the same economic model. For instance, One Medical and Iora were able to combine effectively despite the former being a membership model and the latter being global capitation. Amir emphasizes that much of the overhead resources (e.g., front desk staff, phlebotomy) can be shared across models. ⚡How One Medical uses generative AI to improve efficiency One Medical allows members to message the care team, and then uses NLP to read the message, figure out its content, and triage it to the appropriate team member. Amir is excited about leveraging internal knowledge graphs to get around hallucinations. This approach can point to relevant internal data in helping compose responses, creating more trust and accuracy in the outputs. ⚡Behind the Amazon acquisition One Medical was interested in expanding its direct to consumer offering. However, they recognized that customer acquisition was costly, which is why partnering with Amazon Prime’s existing membership base made sense for them. Amir explains how at their core, both Amazon and One Medical are all about using technology and logistics to create an excellent customer experience.
