Shifting the financial model alone is not sufficient for the transition - the care delivery, workforce, and many other pieces of the ecosystem with which PCPs interact also need to adjust accordingly. Funding to allow for the startup costs of transformation must be made available. This is an interesting talk that challenges a basic assumption that more spending in healthcare leads to better outcomes. They defined health value by looking at mortality rates across 6 diaseases and healthcare spending per capital. Florida ranked 3rd and Vermont 46th. While healthcare spending was correlated with lower heart disease it was not correlated with opioid use disorder. This sheds light upon the fact that social determinants of health have a big impact on outcomes. #healthcare #healthspending

The new Enhancing Oncology Model (EOM) is a significant upgrade on the Oncology Care Model (OCM) in terms of both opportunity and complexity. The primary complaint I heard from participants during the original OCM was the "randomness" of payments. Many participants were not able to build additional revenues into their forecasts and budgets because the payments were intermittent and no one was able to explain what they had done to generate the successful payment. The EOM is designed to address this by adding additional adjustments to the payment methodology. This is a critical factor given the two-sided risk embedded in the EOM. It was one thing to get random bonus payments during the original OCM, it will be entirely different to get seemingly random bills from CMS during EOM. The new payment methodology is going to generate a less random result than the original due to the significant clinical and regional adjustments built into the algorithm. Participants will need to understand the complexity of the payment mechanism in order to anticipate how they will perform under this new model. As with BPCI-A analytics will play a key role in forecasting performance in the EOM. Key factors to analyze include: *Episode Attribution - Are you capturing all of your own episodes? Did you suffer from leakage during the baseline? *Baseline Expenditure - CMS will provide your baseline, but you will need to understand the components. How much of your baseline was impacted by other CMS programs (e.g. PC+, BPCI-A, et.c)? What is the impact of the trend adjustment? *Baseline Price - How does the Price Prediction model work? How is this affecting your baseline over time? What is the impact of the Experience Adjuster and what are the key components? What is the impact of the Clinical Adjusters? How are you poised to capture these in your clinical data? All of these factors (and others) are true enhancements to the program and will result in a more stable and predictable revenue stream for participants. To take advantage of these improvements to the payment methodology participants must work with their analytic teams to understand each component and start designing key reporting assets to track these as the program moves forward.

Information Systems Research (ISR) has recently published a new article to the Articles in Advance page:   Article Title "How Hospitals Differentiate Health Information Technology Portfolios for Clinical Care Efficiency: Insights from the HITECH Act"   Authors Jessica Pye (deceased), Arizona State University Arun Rai, Georgia State University John Qi Dong, National Technological University, Singapore   Problem definition We examine how hospitals differentiate their HIT portfolios in response to cost inefficiencies in clinical care processes and HIT policy uncertainty.   Relevance Adapting HIT portfolios to address cost-based shortfalls and policy uncertainty is crucial in shaping service delivery and impacting care costs, making it a key concern for policymakers and hospital decision-makers. Theoretical foundations We combine behavioral theory and institutional theory to explain how hospitals differentiate their HIT portfolios through idiosyncratic learning in response to cost-based performance shortfalls and shifting isomorphic pressures from changes in HIT policy uncertainty.   Methodology We constructed a panel dataset of 11,316 hospital-year observations from 3,319 U.S. hospitals over 2007–2014 to investigate how hospitals’ HIT search differentiation is influenced by cost-based performance shortfalls and policy uncertainty during the phases of the HITECH Act, from conceptualization to enactment to enforcement.   Results Our research provides a key insight: Moderate policy uncertainty, particularly during the HITECH Act's phases, significantly shapes hospitals' search for novel HIT solutions to address performance shortfalls. Hospitals enhance their search differentiation when uncertainty decreases from conceptualization to enactment but reduce it as uncertainty diminishes from enactment to enforcement.   Contributions Our findings surface the utility of combining the behavioral theory of the firm with institutional theory to explain the interdependence among a hospital’s cost-based performance shortfalls, HIT policy uncertainty, and HIT search differentiation.
