"Why have you chosen to pivot into healthcare?" One of the greatest privileges I enjoy in Adopt-A-Doc is the opportunity to speak to a wide range of healthtech #founders. To note, over 90% of these founders come from a technical or commercial background - rather than clinical. One of the first questions that I always ask is: 𝐖𝐡𝐲 𝐡𝐚𝐯𝐞 𝐲𝐨𝐮 𝐜𝐡𝐨𝐬𝐞𝐧 𝐭𝐨 𝐞𝐧𝐭𝐞𝐫 𝐡𝐞𝐚𝐥𝐭𝐡𝐜𝐚𝐫𝐞 𝐨𝐟 𝐚𝐥𝐥 𝐟𝐢𝐞𝐥𝐝𝐬? Now bear in mind, many of these founders have actually left jobs that were: - In stable industries, - Very well compensated, - In larger, multinational corporations, 👉 To foray in healthcare which has its own sets of challenges. _______________________________ I have heard many great stories. They range from: - A personal or family experience with illness, and the deep dissatisfaction that came from it. - A desire to make things better having seen first hand how prevalent ill health is. - A strong motivation to use their skill set to build something impactful. 𝐇𝐞𝐚𝐥𝐭𝐡𝐜𝐚𝐫𝐞 𝐢𝐬 𝐢𝐧𝐝𝐞𝐞𝐝 𝐚𝐧 𝐚𝐫𝐞𝐚 𝐰𝐡𝐞𝐫𝐞 𝐭𝐡𝐞 𝐩𝐨𝐭𝐞𝐧𝐭𝐢𝐚𝐥 𝐭𝐨 𝐝𝐨 𝐠𝐨𝐨𝐝 𝐢𝐬 𝐢𝐦𝐦𝐞𝐧𝐬𝐞.  A successful #innovation scaled widely can help thousands, if not millions of people. _____________________________ I believe that #healthcare attracts people who are trying to help. It is the one reason why I am still hopeful about healthcare. Because there are brilliant innovators from all walks of life coming together to work on this important problem. Can clinicians that are still working clinically contribute significantly to the healthtech industry? 𝐖𝐞 𝐛𝐞𝐥𝐢𝐞𝐯𝐞 𝐭𝐡𝐚𝐭 𝐭𝐡𝐞 𝐚𝐧𝐬𝐰𝐞𝐫 𝐢𝐬 𝐚 𝐫𝐞𝐬𝐨𝐮𝐧𝐝𝐢𝐧𝐠 𝐘𝐄𝐒. Having now spoken to over 110 healthtech start-up founders via Adopt-A-Doc, we know first-hand that clinical experience and expertise is in great demand. 🤔 The common sentiment is that you have to leave medicine to be able to play a significant part in HealthTech. Latest #PitchBook data for #HealthIT investment shows a preference for later stage investment across healthIT that pre-dates the VC downturn and current interest rate environment. Even though healthIT has been more resilient to current headwinds compared to health care services and PPM plays, perhaps it speaks to a need to scale solutions and points of saturation for new entrants. "...In the lower middle market, we are beginning to see more examples of a phenomenon we have anticipated for a while: PE firms and PE-backed companies acquiring VC-backed startups. Lower-middle-market PE firms perennially complain that VC- backed companies expect too-high multiples, and these firms would generally prefer to invest on a multiple of EBITDA rather than a multiple of revenue..." I just spent 3 days in sunny Brisbane at the Australasian Institute of Digital Health (AIDH)’s HIC Conference, along with my Nous Group colleague Kaushal V, and I feel inspired, re-energised & positive about the future of digital health in Australia. Also got a good dose of much-needed Vitamin D 😎 We heard from fantastic speakers from within our own shores as well from the UK & US. We were healthily provoked, buoyed by the rapid evolution of technology, saw some impressive innovators & were re-assured that governing bodies still have the focus on the care providers and the people they care for. Well done to the AIDH, the Australian Digital Health Agency & the long list of sponsors for putting on an excellent event 👏 I also felt privileged to join a distinguished panel with Dr Simon Kos Duane Attree Stephanie Hodgson Scott Kitchener & chaired by consultmed's A/Prof Vikram Palit where we were asked to share our reflections on how we keep the focus on value as digital health quickly permeates into every part of our health ecosystem 🙏 Now that I’m a consultant 😉 I couldn’t help myself in creating a conceptual framework to bring together the themes from the conference to perhaps help organisations in ensuring improved outcomes & value for people & care providers are produced from our rapidly evolving digital health ecosystems (see screenshot below) 👇 This framework focuses on the crucial relationship between care providers (broad term for clinicians, healthcare professionals, carers, family & health services), the people these care providers serve (people includes patients, consumers, clients, residents etc) & the tools that make that care interface better, safer, faster & more human – I used tools to include all technology, AI/ML, digital systems so that we can remain grounded in first principles, no different to the stethoscope as a tool, or medical imaging or unimagined tech in the future 👍 Calling it a tool reminds us that digital tech (albeit bloody amazing!) is still a tool to enable & enhance the human interface of care, rather than the technology being the main focus or final destination 👩‍⚕️ I then considered different principles governing the interactions between those three entities & if all works well, the outcomes (short-medium term) & value/impact (long-term) to people & care providers should be optimised 🤰 Around the edges, we can’t forget the role of infrastructure, sensible policy & regulation, societal expectations, changing models of care, equity of access & of course a focus on continual learning, improving & sharing 🤝 Would love to hear your thoughts on this! 😊 Look forward to the next gathering of like-minded people with a passion in the evolution of digital health 💙 Tim Fazio George Margelis Raymond Wen Raj Verma Yael Cass Sudipto Sarkar Daryl Cheng Dr Monica Trujillo Bryan Tan Angela Ryan Wendy Chapman Christopher Bain Chris Boyd-Skinner Dr. Paul Cooper Michael Franco Sanka Amadoru

Scaling vs. secret sauce. Healthcare is a mix of pixie dust and bricks & mortar. More bricks and mortar centric healthcare is not the cure. It's just going to bankrupt us faster. A startup approached us at BP2 Health earlier this year and asked us to build out their business model - their concept sounded exciting so we said yes. Unfortunately, there wasn't enough pixie dust and they were focused on becoming another Oak Street Health in terms of valuation more than creating something special. There are so many lessons to be learned from CVS Health and particularly CVS and the Oak Street acquisition - not just about transactions but about healthcare at large (all the way from care delivery to insurance benefits). -Losing sight of the pixie dust needed in care delivery -Buying an asset doesn't guarantee assimilation -Investing too heavily in expansion at the cost of margin -Scaling faster than you can ingest and still create a culture -Doubling down in a segment with intense headwinds -Creating a primary care model that is profitable under % of premium (Gloria Gates CARE Zane Gates Colby Schaeffer) -Missing the opportunity for N of 1 healthcare and non-bricks and mortar care delivery methods that can only be deployed in value based contracts like those in which Oak Street participates I'm also sorry for those individuals at Oak Street that didn't get paid out as a founder, but have significant skin in the game and a vision for the future that no longer align with each other. Equity is a game few win at but so many play in healthcare because it appears such a clear path to capital these days. I've been in your shoes and it stinks (better words but I'm keeping it clean for LinkedIn).

Cate Swannell for the Health Services Daily Private health insurers, notably Medibank, are using vertical integration, backed by self-commissioned reports, to recommend home-care services. But they are only paying for the services they run themselves, a move dubbed by the one peak body as “a threat to clinician autonomy”. #disruption

Companies like Grove Health are really demonstrating how asymmetrical the playing field is when it comes to Telehealth in Australia. This asymmetry is across both access to rebates for consumers and information asymmetry given to consumers. So #1 - asymmetry of access: Grove advertises free/bulk billed Telehealth consults for consumers. Straight off the bat this is strange - I spend a good chunk of my day telling patients they are outside of the 12 month face to face criteria the government puts on GPs to allow patients to access a Telehealth rebate. How do they do it? Nurse practitioners…… So it turns out the government never put the same restrictions on patients accessing nurse practitioner rebates that they did to GPs….and the Telehealth only companies have been hammering this! So now we have a situation where I can have a long term relationship providing care to a patient but if it’s been 366 days since our last Medicare billable face to face consult then a Telehealth rebate won’t be available BUT the patient CAN receive a rebate from a nurse practitioner providing Telehealth only services shilling medications online. Just bizarre. #2 asymmetry of information for consumers So Grove Health sells natural medicines - whatever, not my jam but consumers gonna do what they do….. But Grove Health is also the supplier of said medications. And Grove Health also charges patients $49 to use scripts provided through these bulk billed (otherwise known as government funded) consultations anywhere except their pharmacy. Seems pretty unfair to me. So what’s the take away? Play the rebates right and you can have the government fund - a nurse practitioner hired by a company and providing incidental fragmented consults - funnel care to the issuing of scripts that have a financial penalty if used anywhere but the pharmacy owned by the company. At the exact same time my patients receive no rebate for my unbiased Telehealth care based on long term knowledge of their health. And I have no product beyond that unbiased advice to sell.

My 58 minuts conversation with Elizabeth Holmes, Theranos. Back in 2016, curiosity about how technology would revolutionise healthcare led me to a fascinating conversation with Elizabeth Holmes of Theranos, arranged by a friend working in a VC firm on Sand Hill Road. What was scheduled as a brief 30-minute chat extended to nearly an hour of deep discussion about the future of healthcare. Elizabeth was unwavering in her belief that Theranos was on its way to becoming a major player. Today, as I look back, that meeting serves as a powerful reminder of a critical lesson in business leadership. Despite the very dire outcomes for Theranos, the scenario exemplifies a larger truth: the top leadership can’t shift a company’s trajectory unless they address the core business fundamentals. No matter who was at the helm, without addressing the underlying issues, the result would have been the same. Unfortunately, five years ago, it was often enough to hire great people who knew how to interpret data. Today, that’s no longer sufficient. We need individuals who can not only decipher the data but also act decisively on it to steer the company in the right direction amidst rapidly changing industry landscapes. Now, as I talk with CEOs, board members, and entrepreneurs across various industries facing rapid change, I see how vital it is to surround ourselves with people who are not only straightforward about what the data tells us but also skilled in adapting business strategies to upcoming industry trends. This experience has reinforced my belief in the importance of being relentless in data analysis to ensure our business strategies have real "lift power." As leaders, we must prioritize data-driven decisions and foster a culture that embraces honest and dynamic adaptation. Let’s continue to lead with both vision and rigor, ensuring our strategies are robust enough to thrive in an ever-evolving landscape.

Today, I attended a dynamic workshop in Sydney focused on how health technology assessment (HTA) should account for climate change. The discussions were lively, informative, and, most importantly, crucial for our future. Thanks for the invitation to be part of the conversation Humanise Health. Rising temperatures, increased pollution, and extreme weather events lead to a spike in respiratory issues, heart problems, mental health crises, and infectious diseases. These aren't distant threats; they're happening now. It was great to hear Assistant Health Minister Ged Kearney talk about how important it is to proactively address the impact of climate change on health. HTA, for the uninitiated, is where health experts look at new tools and treatments, like medicines, machines, or tests, to see if they work well, are safe to use, and are a good value for the money. Like climate change, its technical, complex and essential to understand. Our approach to HTA must evolve. Sustainability, meeting today's needs without sacrificing tomorrow's, is key. By evaluating the environmental impact of health technologies, alongside their clinical and economic benefits, we can foster a healthcare system that's vital, robust and green. Empowering people to have a say is essential. Australians want to understand these complex subjects so they can participate actively in decisions that affect their health and the environment. True empowerment means knowing the jargon and feeling safe and confident to sit at the important tables. It's not only the big decisions where you can make a climate impact. Every decision counts. Did you know that there are some asthma inhalers that use your breath to work, while others use a special gas to spray the medicine into your lungs? Dry powder inhalers are a more sustainable choice. Some folks will need the gas-powered inhalers but not everyone. I certainly don't know the climate impact of many of the health choices that I make. And I would like to be able to factor this into many of my choices. Today’s workshop wasn't just informative; it was a call to action. By enhancing HTA processes to include sustainability, we can address the long-term effects of climate change on health. Let's educate, engage, and empower our communities. Who doesn't love these strategy maps from CB Insights? We need to bring these maps into the Kidneyverse to better understand the strategies of kidney care market movers including: 🏪 retailers 🏥 providers 💰 payers 💊 pharma 💉 manufacturers ⚙ VBC / enablers 🤖 tech companies Here are a few perspectives I think this exercise will help us better understand heading into the second half of 2024 and beyond: 1. How retailers think about primary care 2. How primary thinks about polychronic care 3. How polychronic thinks about value-based care 4. How VBC thinks about payer growth 5. How payers think about provider growth 6. How providers think about tech / services 7. How services companies think about tech 8. How tech thinks about pharma partners 9. How pharma thinks about tech / pipeline 10. ??? Within each of these perspectives we find groups of key players, many millions of impacted lives, and many billions of invested capital. This type of deep dive into a single company strategy is important. So far in our Kidney Capital series we've spent a lot of time talking about the most active invest-ors in the space (see link in comments). Now it's time to talk about invest-ments.
