"Hospitals and nursing homes are implementing various initiatives to reduce
hospitalizations and the Center for Nursing and Rehabilitation (CNR), part of the
CenterLight Health System, is no exception. This facility, which serves the needs of
residents with a variety of physical co-morbidities and cognitive challenges, was
recently recognized by the American Medical Director's Association (AMDA) for its
approach in reducing hospitalizations through value-based healthcare.
CNR's comprehensive and interdisciplinary approach to reducing hospitalizations
includes using the Stop and Watch communication tool to quickly notify the care team
of any change in a patient's condition, review screening and assessment tools,
evaluate staff competency, and provide weekly educational programs.
As a result of these efforts, CNR reduced their short-term re-hospitalization rate by
41% and long-term hospitalization rate by 52% during the study period. Their overall
patient satisfaction rating of 95% and 97% recommendation rating also illustrate the
quality of care their patients receive". -

This award was created in memory of Eli Pick who was a consummate member of ACHCA dedicated to advancing professionalism and leadership in long term care. -
