    NYS Triple Aim: New York Hospitals Are Improving Health, Enhancing Quality, and Reducing Costs.

    "Hospitals and nursing homes are implementing various initiatives to reduce
    hospitalizations and the Center for Nursing and Rehabilitation (CNR), part of the
    CenterLight Health System, is no exception. This facility, which serves the needs of
    residents with a variety of physical co-morbidities and cognitive challenges, was
    recently recognized by the American Medical Director's Association (AMDA) for its
    approach in reducing hospitalizations through value-based healthcare.
    CNR's comprehensive and interdisciplinary approach to reducing hospitalizations
    includes using the Stop and Watch communication tool to quickly notify the care team
    of any change in a patient's condition, review screening and assessment tools,
    evaluate staff competency, and provide weekly educational programs.
    As a result of these efforts, CNR reduced their short-term re-hospitalization rate by
    41% and long-term hospitalization rate by 52% during the study period. Their overall
    patient satisfaction rating of 95% and 97% recommendation rating also illustrate the
    quality of care their patients receive".

Fewer than 9% of Skilled Nursing Facilities in the country qualify to receive this award. Eligibility for the award is based on three years of skilled nursing facility survey data, including the Health, Fire (Life Safety), Complaint Surveys, as well as top quintile performance on designated Quality Measures. The criteria also…

    The Eli Pick Facility Leadership Award uses data driven criteria to objectively identify high-performing skilled nursing facilities and the leaders who make them excel. Fewer than 9% of Skilled Nursing Facilities in the country qualify to receive this award. Eligibility for the award is based on three years of skilled nursing facility survey data, including the Health, Fire (Life Safety), Complaint Surveys, as well as top quintile performance on designated Quality Measures. The criteria also included an 80% or greater facility occupancy and a three year avoidance of a Special Focus Facility status.
    This award was created in memory of Eli Pick who was a consummate member of ACHCA dedicated to advancing professionalism and leadership in long term care.
