The SHORT Case Of The Day! Tooth #28 - VRF (Vertical Root Fracture) or Missed Canal? Patient came in with pain on tooth #28. It had a crown with a previous root canal. The 2D image shows the root canal was done well. However, the 3D image shows otherwise. There was a missed lingual canal (LC) and periapical pathology. Endodontic retreatment was performed in one visit using bioceramic sealer and single cone condensation. Patient returned asymptomatic since retreatment with osseous healing 6 months later. This was not a vertical root fracture. This was a missed lingual canal. The SHORT Case Of The Day! Tooth #6 - One Visit Internal Resorption Case Patient came in with pain on tooth #6. The diagnosis was a necrotic pulp with acute apical periodontitis and internal resorption. Internal resorption is an uncommon resorption of the tooth, which starts from the root canal and destroys the surrounding tooth structure. Normally the tooth is vital but not in this case. Tooth #7 will need retreatment due to PAP present. A LFOV 3D CBCT was taken to make sure there was no perforation of the defect on tooth #6. Endodontic treatment was performed in one visit. The irrigation solution of choice in the case was Triton by Brasseler USA. Triton is the first irrigant to deliver all of the benefits of NaOCl, EDTA, and CHX in a single-step all-in-one irrigation solution. It's also effective against E. Faecalis and kills bacteria quicker than any other solution. I also activated the Triton with the EndoUltra Vista Apex Dental Products Ultrasonic unit for 2 minutes prior to obturation to make sure I was able to clean the internal resorptive defect. (I use to use calcium hydoxide for 4 weeks in these cases - but now I perform these in one visit). The cone fit radiograph was take to make sure everything was perfect. Obturation was performed with warm vertical condensation gutta percha using ZenSeal Bioceramic Sealer by Kerr Dental. Some bioceramic sealers are versatile and can be used safely with warm vertical condensation. I chose this technique to make sure I was able to 3-dimensionally fill the internal resorptive defect. Normally I use single cone (cold) technique with bioceramic sealer. The benefits of bioceramic sealer are biocompatibility, non toxicity, antibacterial, effective against E. Faecalis, non staining, dimensional stable and bio-inert. They have a similarity to Hydroxyapatite, an intrinsic osteo conductive activity and have an ability to induce regenerative responses in the body. I talk more in detail about bioceramics in my textbook "Endodontic Keys And Cases" on Amazon. Notice the phenominal result. The canal was cleaned and shaped and filled 3 dimensionally in one visit. This saves patient time and the clinician chair time. It's a win on both ends.
