What causes hamate hook fractures in baseball players? Most literature suggests direct contact from the knob of the bat fractures the hook. Is this true though? A recent article questions this mechanism. Researchers reviewed 50 cases of hamate hook fracture and divided the patients into 4 groups based on injury mechanism. Group A: Baseball (24) Group B: Other sports (8) Group C: Falls (16) Group D: Crush (2) What did they find? Groups A and B had similar patterns. Both had a fracture line that started on the radial side at the mid section of the hook. Group C mostly had transverse fractures at the base of the hook. Group D had fragments at the tip of the hook. The fracture pattern in baseball batters suggests an indirect mechanism--repetitive force from flexor tendons pulling across the hook. Not direct impact from the knob. How does this apply clinically? Players may ask about axe handle bats for: 1) Injury prevention 2) Rehab after hook excision My thoughts: The knob isn't the issue. Is BFR a game changer to your rehab, outcomes and expedited return to sport post ACL reconstruction? Absolutely. Colombo et al AM J Sport Med 24 Comparison of blood flow restriction interventions to standard rehabilitation after ACL reconstruction. Although this systematic review showed there was a benefit to strength development, muscle size and PROM, the evidence was not that strong. As a BFR certification instructor who has been teaching this and using it for 8 years I can tell you 90% of the time it is used wrong. Yet we wonder why we have outcomes like this in studies. For best results: ✅ start early ✅ use the right rep and set sequences - you must introduce a hypoxic state to the muscle ✅ add BFR cardio routine - get that extra GH and IGF-1 load ✅ don’t ignore the need for proper hydration with ✅ don’t ignore the importance of proper nutrition with ✅ push with CKC and single limb training ✅ leverage as a tool for every phase of rehab Questions let me know or check out my BFR certification course through @neseminars
