A new op-ed from Rivka Friedman, Head of Health Care Innovation at Morgan Health, and Daniel Stein, MD, MBA, CEO of our portfolio company, Embold Health, outlines why employers need a more comprehensive view of data on the quality of individual physicians and care facilities. The lack of clean, consistent data in this area has been a real challenge in commercial insurance – often fueling the misconception that more expensive care is higher quality. Common standards to assess care quality offer a solution here. Research shows that when employers can successfully connect their members to high-performing clinicians, they’re more likely to receive the most appropriate care at a more efficient price cost. Rivka and Dan S. share orthopedics as a help case study. For both spine care and orthopedic joint care, costly surgeries are more likely to happen when patients see low-performing providers. In contrast, high-performing clinicians tend to be more conservative in recommending surgery and use physical therapy and other non-surgical interventions to achieve good functional outcomes (at a lower cost) for their patients. Came back from #HLTH2024 - excited and energized. I am now convinced that dog petting, rock singing, and hair grooming should be part of every conference 🙂 On a bit more serious note, it is rewarding to see how AI is evolving and moving into the mainstream addressing important use cases. Yet, there seems to be a gap that keeps expanding as more AI solutions become available. As powerful as they are, the AI solutions have limited utility unless they are deeply integrated into the clinical systems and user workflows. This integration is far deeper and more complex than traditional systems integration. If done right, the AI solutions are bound not only to make provider/patient/payor workflows more efficient, but fundamentally transform and revolutionize them. Yet, in our many conversations at HLTH, we clearly observed a lack of understanding of this complexity or knowledge of how to address it. There is another challenge for integrating AI solutions - the variability and quality of the data they rely on. The outcomes that AI models produce are heavily dependent on the context we supply to them (or the data we use to train or fine-tune them). This context/data is largely represented by patient data. It should be to no one's surprise that the patent record the provider sees on the screen is different from what is exposed through interfaces. And the data exposed through the same interfaces for an identical patient record in two different EHRs is likely to be somewhat different. This means that, for example, patient summaries from two EHRs for the identical patient record are going to be somewhat different. Extensive expertise and knowledge of clinical workflows, ability to extract, reconcile, and harmonize clinical data, and strong integration capabilities are rarely part of the core competence of the young and emerging AI solution development organizations.  It seems that a new era of partnerships is emerging where AI-focused vendors team up with seasoned systems integrators, healthcare experts, and medical informaticians to seamlessly "lace" AI solutions into user and systems workflows. #HLTH2024 #HealthcareAI #GenerativeAI #PatientExperience #FutureOfHealthcare #IntegratedHealth

As promised here are my thoughts from Day 2 at the FDA's Digital Health Advisory Committee meeting on "Total Product Lifecycle Considerations for Generative-AI Enabled Devices." I have pretty much focused my thoughts on the talk and remarks presented by Dr. Keith J. Dreyer as he summarized it all very eloquently. He was truly inspiring! Here are my top 5 takeaways: 1️⃣ Continuous Clinical Validation is a Must: AI devices, especially in radiology, need ongoing validation to ensure they remain accurate and effective in real-world settings. 2️⃣ Regulating Generative AI for Patient Safety: There's a pressing need for clear regulations to guide the safe deployment of GenAI in healthcare, focusing on transparency and human oversight. 3️⃣ Expanding the FDA's Role: Keith emphasized that the FDA should look beyond manufacturers to include device users for better monitoring. As he put it, "We have to keep the data pristine to make sure these devices are working, independent of what the manufacturer makes." 4️⃣ Cost-Effective Monitoring Solutions: Recognizing that monitoring and validation can be costly, we need innovative solutions that make this feasible for all healthcare settings, big or small. 5️⃣ AI's Potential to Boost Efficiency: Generative AI could greatly enhance efficiency in radiology. Keith noted, "If there was a way to improve our efficiency through AI, and I think Gen AI has a way to be able to do that, then you're going to see availability, much broader availability of even specialists, to do more work and to work beyond the scope of what they can do today." If you are over 45, you should be screened for colorectal cancer. I know. It's not as fun as it sounds. But caught early, it can be dealt with and you can move on with your life. Caught late, and it's going to affect your life. Today, the FDA approved a blood test, which is another option for screening. Talk to your doc about getting screened. Healthcare organizations must build AI solutions through a health equity lens from day one. Failing to do so places underserved communities at risk. To understand what's at stake, consider the harm caused by a biased equation for prioritizing kidney transplant patients. The test overestimated Black patients' kidney function, ranking them as healthier than non-Black patients. As a result, thousands of Black patients ended up farther down the transplant list than their conditions warranted — waiting years for the care they deserved. One woman who secured a spot on the list in 2019 discovered she should've earned her place in 2015, the Associated Press reported. More than 14,000 Black people have had their wait time for a kidney transplant adjusted to address the bias. But healthcare can't stop there. We must seize this moment to commit to prioritizing health equity in AI solutions. Equations like the one used to evaluate kidney function already support countless care decisions for payers and providers. As AI solutions proliferate, the algorithms will only become more common and complicated. If organizations don't prioritize health equity in the AI development process, they risk building vast networks of solutions that actually harm communities. Once these solutions are in place, it may take years before experts can trace harmful practices back to lines of biased code. Building AI solutions is highly technical, but reducing bias requires a human touch. Including the voices, cultures, and lived experiences of diverse groups throughout development — from data gathering to deployment — will help ensure solutions respect everyone's needs. Assembling diverse teams to monitor AI once it's operational will help keep the technology from veering off course. Healthcare is poised to adopt AI in a big way. Manatt, Phelps & Phillips, LLP's take got me: "CMS is recognizing the role of digitally enabled care in improving access and outcomes." They also propose these new models "present health tech companies with opportunities to target new markets" because of the new/extra funding—such as viewing state Medicaid offices as new partner opportunities, which I've been seeing a lot of talk about recently in my feed. What do you think? I guess being in New Mexico, where almost half of our state is covered by Medicaid, Medicaid and MCOs have been a primary strategic partner/target market for 15+ years, and a somewhat omnipotent source driving most investment and initiatives. ------------------ More about the models: The Transforming Maternal Health (TMaH) model will issue agreements to up to 15 state Medicaid agencies to develop and implement a whole-person approach to pregnancy, childbirth, and postpartum care. The Innovation in Behavioral Health (IBH) model will develop and implement state-administered approaches which integrate behavioral, physical, and social supports to coordinate and manage care for Medicare and Medicaid beneficiaries with moderate-to-severe mental health conditions or substance-use disorders (SUD). I thought to myself, "wait, aren't we already doing that?" But they clarified: through the new models, CMS is encouraging states and providers to invest in technology and supporting infrastructure. ------------------ 🎉 What I'm excited about: Continued support for entities to get connected and the continued case for HIEs to expand their capabilities. Specifically, EHRs, HIEs, and remote patient monitoring (RPM). AND.... 🎉 "Additionally, CMS will support states with implementing data infrastructure to enable collection of Health-Related Social Needs (HRSN) data and referral to support resources." ------------------ 🤯 What I didn't realize: "It is estimated that only 6% of behavioral health facilities and 29% of substance use disorder treatment centers in the U.S. utilize EHRs, whereas nearly 4 in 5 office-based physicians (78%) leverage an EHR platform today 🤯 I'm having a hard time following along with all the new models coming out of CMMI, though 😉 Interesting piece all around, good info.

One of the most compelling takeaways from our conversation is that patients today are no longer passive recipients of care; they are active, empowered partners in their health journey. This paradigm shift underscores the need for advanced, omnichannel solutions that not only support but also elevate the individualized patient experience. Value based care (VBC) 1.0 language was heavily oriented to language like "disruption". This language was like a punch in the mouth and was effective at building awareness of new opportunity - but disruptive punches to the jaw don't sound like a very attractive end state. It sounds pretty painful, and maybe that is one reason why VBC adoption has stagnated. VBC 2.0 needs to be about integration and partnership so that we can start to bring real value to patients and the provider organizations who serve them - and do that at scale. The pre-eminent leader in health plan clinical guidelines coming together with the most innovative AI provider of utilization management technology is a perfect example of this new paradigm. Kudos to the MCG Health and Cohere Health teams taking a leadership role to make the health care system work better for all.
