Presenting at the Senate as part of the Coalition for Health AI (CHAI) "Day on the Hill" wasn't on my 2024 bingo card, but I'm feeling energized by the bipartisan discussions on healthcare AI. The numbers tell a compelling story: while nearly 2,000 healthcare AI startups have received funding over the last two decades, only 1 in 20 provider-facing companies reach late-stage maturity. Beyond natural market dynamics, two critical barriers are holding back AI's full potential in healthcare: unsustainable payment models and inadequate evaluation infrastructure – only 5% of research studies on LLMs in medicine utilize real patient care data, and studies advancing our understanding of performance beyond medical licensing exam benchmarks remain anemic at best. At CHAI, we're advocating for essential foundations: • Rigorous technical model performance evaluations • Comprehensive bias assessments across diverse patient populations • Thorough pre-deployment simulations and prospective studies • Robust ongoing local monitoring The path forward demands strong reporting infrastructure, advanced healthcare AI benchmarks, and meaningful public-private partnerships to drive real-world AI deployment in healthcare. One of the most important concepts we introduced in our recent Healthcare AI roadmap is the notion of “Modality—Business Model—Market fit.” This concept describes how the delivery method and business model of AI in healthcare directly shape its potential to create value. There are many possible combinations of modalities and business models in healthcare. AI can be delivered as software, copilots, agents, services, diagnostics, or therapeutics, supported by business models ranging from usage-based and volumetric pricing to performance-based and shared savings arrangements. Nailing selection of modality and business model is essential for AI companies seeking large market opportunities and profitable operations. As we describe in an example with computer vision-based diabetic retinopathy screening, modality and business model selection can drive a difference in TAM by upwards of 25x, and has serious implications for overall margin structures. Open-source models can be a strong lever for increasing gross margins at AI companies, especially those that have hefty cost structures (e.g., enterprise sales orgs that require significant customer success or implementation resources, as well forward-deployed AI services models common in healthcare and life sciences). One of many reasons to maintain a modular and flexible stack with minimal dependencies when building an AI company. Nearly half (if not more) of YC's F24 healthcare startups are building AI agent tools for doctors. Using agents to answer phones, write notes, prescreen patient intakes, etc. Sounds great right? Well, not exactly. That's because what YC and other investors are getting wrong about this generation of physicians versus those of the past (who did nothing to stop consolidation or electronic health records), is that they aren't just going to sit around and wait for someone else to build something for them anymore. They're builders who want to solve their own problems, and they're willing and eager to learn. Just look at telemedicine adoption data vs AI to explain what I mean: - Prior to COVID telemedicine was a <19% - During COVID it grew to > 90% - Post COVID it went back down to <40% But AI? - In Jan 24 it was 38% - Latest survey in Oct shows it >68% - By this time next year it should be near 100% This is the fastest adoption of any tech in healthcare in history, and it's only the beginning. Every day, new models, new features, and new tools are created for non-developers to build software using AI. What makes people believe that doctors can't or won't figure out how to use zero shot/few shot prompting in environments like cursor or claude just like everyone else? So if your bet is that AI doctors will make the same purchase decisions as their non-AI counterparts of the past, you're most likely making the wrong bet. Here’s an interesting read/concept to consider from TechCrunch! AI-powered chatbots like ChatGPT are incredible innovations that are worth being excited about — but it’s important to also remember that we haven’t perfected these platforms yet. When it comes to chatbot technology and inclusivity, there are still multiple obvious shortcomings. #ChatGPT and other AI tools of the same vein currently struggle to read cultural nuance, and the algorithms that power these services mostly approach things from a Eurocentric/Western lens. Basically, one of the most popular and widespread AI platforms we have today was not built with the perspectives of #POC #BIPOC in mind. As a result, we’re starting to see alternative chatbots/modifications of ChatGPT in an attempt to create #PersonalizedAI such as John Pasmore’s Latimer.AI, an AI language model that tailors its answers to the experiences of Black and brown people. See also Erin Reddick’s beta ChatBlackGPT, which adjusts the harsher language and rhetoric regarding Black historical events, was trained by Black authors, writings, and philosophers. So, do you think #culturallyspecific AI models are a good solution? Or eventually, should broader companies that own ChatGPT/chatbots have a responibility to make them as inclusive as possible? Could that even be achieved with the structural racism and persistent inequalities that prevail in the tech ecosystem today? I want your takes.

A new era of deep tech has emerged. First time funds will raise “unheard of” amounts of capital to fuel next gen deep tech startups - producing outsized, superior returns for LP’s compared to the rest. Prudent investors will act on this data and shift investment strategy as LP’s or risk being left behind savvy wealth managers and CIO’s / FO’s who saw this trend begin 4 years ago.

Did you know that YC founders get a secret weapon… and it’s not what you think. “Bookface” is YC’s internal social network. It was needed because YC had funded larger batches and the hardest part of a community is knowing who is in it and who you can trust and ask for help. On the platform, founders share real methods and advice that would break if it were shared widely It’s secret knowledge shared with a trust-first community - answering 80% of the questions a founder can have about building and selling a product, be it to clients, candidates or investors. Before the platform, if you had a B2B startup, you had to reach out cold to potential customers. Now there are over 1000 YC companies you can sell your service to and all of their contact information is in Bookface. As a result, YC alumni companies are often over 50% of your new sales during YC. It’s superpowers like this that make YC deals so attractive compared to the norm. You are investing in a founder with a 10x network already!

Some VC investors believe that brick and mortar - referring to a physical presence of some kind - should be avoided at all costs. Yet, the track record of clinic and hybrid care businesses in health care indicates otherwise. ==> One Medical (sold for $3.9 billion to Amazon), Oak Street (bought by CVS for $10.6 billion), Lifestance (IPO at $7.5 billion market cap), and PE-backed Premise Health (acquired by OMERS Private Equity for $1.1 billion) If a clinic is indicated as the optimal way to deliver care for that patient population, there are creative ways to build and operate them. Hot off the pre-print server – an important publication authored by the Atropos Health team exploring the performance of LLMs in answering real-world clinical questions. LLMs excel at summarizing and contextualizing existing literature, but cannot generate de novo medical evidence today. The results of the study highlight performance gains attributable to injecting novel, relevant, and up-to-date medical evidence into LLM-powered clinical decision support systems. GenAI is rapidly transforming industries. *Blink* and you’ll miss the next development. Having elderly and remote family members, I closely observe GenAI's impact on healthcare. Healthcare has long explored AI and data to improve patient care. LLMs accelerated this progress exponentially. Now, AI is no longer a 'nice to have'—it's a 'cannot ignore.' AI is improving patient outcomes in many ways. Three examples: 1️⃣ Faster, cheaper drug discovery AI models swiftly sift molecular data, shortlisting promising drugs and simulating disease effects. 2️⃣ Personalized treatment plans 20 years ago, IBS patients received nearly identical management plans. Now, AI combines patient data, medical histories, and latest research to generate tailored recommendations. 3️⃣ Enhanced medical imaging AI assists in detecting subtle patterns and anomalies, aiding diagnosis and suggesting further investigation. Beyond these, I see three crucial categories where AI is revolutionizing senior care specifically: → AI-powered vertical software for senior living → Monitoring and prediction for aging in place → Assistive technology within senior healthcare In these areas, AI can be present wherever a human can't, significantly improving seniors' safety and well-being. Medical professionals will always make the final call. But AI is becoming an invaluable healthcare assistant—supporting doctors in making the best choices for patients.
