One of the most important concepts we introduced in our recent Healthcare AI roadmap is the notion of “Modality—Business Model—Market fit.” This concept describes how the delivery method and business model of AI in healthcare directly shape its potential to create value. There are many possible combinations of modalities and business models in healthcare. AI can be delivered as software, copilots, agents, services, diagnostics, or therapeutics, supported by business models ranging from usage-based and volumetric pricing to performance-based and shared savings arrangements. Nailing selection of modality and business model is essential for AI companies seeking large market opportunities and profitable operations. As we describe in an example with computer vision-based diabetic retinopathy screening, modality and business model selection can drive a difference in TAM by upwards of 25x, and has serious implications for overall margin structures. The (Healthcare) Ground Beneath Our Feet… This year it is likely that over $10 billion will be invested in the digital health sector – a robust amount, no doubt, but inconsequential when compared to the amount of capital tied up in healthcare real estate assets, which is estimated to be $1.2 trillion by Jones Lang Lasalle. Of that amount, approximately $790 billion is in the hospital sector while the remaining $490 billion is in medical office assets. It is estimated that there are 48.6k facilities with 3.4 billion square feet of healthcare real estate, 42% of which are owned by REITs and another 33% held by other private investment vehicles. Much of the digital health investment is meant to make the physical healthcare infrastructure more productive: greater operating efficiencies, greater patient throughput, more relevant personalized care models with better outcomes. As the healthcare system is being pushed to be more distributed to lower cost sites of care, accelerated now by the rapid proliferation of automation and AI capabilities, the potential impact on healthcare infrastructure assets will be profound. According to the latest report from Rock Health, digital health funding in Q1 2024 totaled $2.7B across 133 deals, with an average deal size of $20.6M. It doesn’t break out device-related investments, but most medtech founders I meet these days are being forced to chase tiny rounds made up of endless little checks, consuming valuable energy and slowing their progress. The benign neglect of this next generation of diagnostic and therapeutic devices is counterproductive and surprising. Given the crowded virtual care market, for example, I’d expect more such companies would be exploring medtech partnerships, investments or acquisitions that'd enable them to back integrate into delivering sharper, and more differentiated, care and treatment. This is exactly what Maverix Medical is doing from the other direction: Reshaping the entire spectrum of lung cancer care by integrating more advanced + effective approaches to screening and diagnosis, treatment and therapeutics, and even palliative care…And putting it all to work through an innovative provider network. One notable investor exception is Y Combinator partner Surbhi Sarna who’s been backing great teams like Selera Medical (YC W24) doing foundational work on ambitious medical devices. Without more support from investors like her, we’re going to increasingly find ourselves stuck with the incremental portfolio improvements from the incumbents, and that’s just not nearly enough.

Science-fiction is inherently speculative. Author George #Orwell of ‘#1984’ predicted concepts such as doublethink, little earbuds and society’s obsession with the black mirror screens that dominate our lives — and according to the BBC, novelist and short story author JG Ballard, of “High-Rise” and “Crash” fame, has predicted the current tool and cultural fixation of #GenerativeAI in the modern society — only he didn’t just speculate them within a story, he is actually one of the first use-cases of a generative AI used in a creative context — Ballard was so fascinated with the growing technological advancement of computers that he used one to compose poems, essentially the precursor to what we have now with ChatGPT. Today, the debate about the creative use of generative AI and its value on creative industries is an extremely fraught and controversial topic, but if controlled in a space that doesn’t replace human labor or try to equate itself to traditional art forms, it is interesting to see what computers can do and how they are creatively limited in comparison. Ballard’s two poems, The Yellow Back Novels and Machine Gun City are generated, and both include notes on how Ballard achieved producing them. Regardless of quality (Ballard himself probably wouldn’t describe these two poems as his best work), it is interesting to see primitive versions of the technology that we utilize on the daily today, and how these moral conversations that concern them would continuously evolve.

As a past board member, I'm honored to celebrate the outstanding quarterfinalists. As we reflect on the state of digital health in 2024, it's evident that the COVID-19 pandemic has fundamentally transformed healthcare delivery. The rapid adoption of telemedicine during the crisis has become a permanent fixture, with over 60% of physicians continuing to utilize this technology. The sprint towards smarter healthcare, catalyzed by necessity, has shattered preconceptions about digital-first health experiences. Virtual care is no longer a stopgap but an integral part of a seamless, anytime, anywhere health ecosystem. 🌍 We're witnessing the rise of B2B2C models in remote consultations, with companies like Teladoc Health and Amwell partnering with insurance providers to integrate their services into health plans. Public sector collaborations, like eConsult Health 's partnership with the NHS in the #UK, underscore the potential for remote consultations to enhance efficiency and accessibility at a national level. The digital health landscape is evolving rapidly, with advancements in remote consultations, mental health support, at-home diagnostics, and patient monitoring. AI and generative AI transform care delivery, making it more efficient, personalized, and patient-centric. 🤖 Yet, challenges remain. While venture capital funding for digital health has stabilized after the pandemic boom, it remains below its 2021 peak. Sectors like digital therapeutics face reimbursement hurdles, and we've seen major players like Walmart and Optum close telehealth businesses. Looking ahead, we anticipate further consolidation in digital health, driven by acquisitions to create broader ecosystems encompass telemedicine, mental health, and other specialized services. As healthcare leaders, we must leverage these digital advancements to improve access, enhance patient outcomes, and drive efficiency in our healthcare systems. The future of healthcare is digital, and the time to embrace this transformation is now. Analysts Melissa Incera and Dan Thompson return to the podcast to dig into the data and discuss the ramifications for enterprises, datacenter builders and operators and those putting AI to work with host Eric Hanselman. The computational requirements of generative AI are unique, taking demands similar to high performance computing or cryptomining and spreading them across a vast new community of users. The demands for GPU-based capacity are consuming more power, sometimes with order of magnitude increases. It’s also driving retrofitting of existing datacenters. The need for higher capacity and innovations like liquid cooling are causing a new surge. We’re at the beginning of the shift from a focus on model training to greater amounts of inferencing, changing where capacity is needed.  Looking at money flows in this market offers additional nuance. Investments in AI startups have been massive. Capital investments from hyperscalers in their own infrastructure is also massive, but it remains to be seen if they’ll pay off in equivalent amounts of revenue. Is all of this sustainable? It’s an complicated question in the many aspects of the word."

Mental health issues are a growing concern in America, with approximately 50 million Americans currently grappling with a mental health condition— a number increasing by 1.5 million every year. At the same time, the US is experiencing a massive shortage of 30,000 mental health clinicians in 2024, a situation costing the US health system $300 billion annually due to delayed treatment. Yet, clinicians are spending only 50% of their day with patients, while the rest of their time is consumed by admin-heavy work, delaying access to mental health care even further. With mdhub, mental health clinicians become 10x more efficient by automating tasks such as clinical documentation, drafting patient after-visit reports, pre-charting, and treatment planning. Generally surprised to not see more hardware vendors in healthcare and life sciences partnering with AI-first companies. When the zone of genius of a company is hardware, it's difficult to imagine that same team achieving parity on an AI product – Apple may be the greatest example of success in this realm, and still has secured many AI capabilities via targeted M&A (Siri, Turi, Xnor.ai, Voysis, Inductiv). Given the importance of leveraged distribution models in the ossified industries of healthcare and life sciences, it seems there is untapped opportunity for more hardware manufacturers to partner and co-develop AI products with startups. Control over the way data is captured and processed is critical for building high quality training data sets, which creates an interesting feedback loop between hardware companies and potential AI-first startup partners. Interested to explore this further with folks in healthcare and life sciences hardware!
