The connection between social needs and health outcomes has never been clearer. 🌟 The latest Accountable Health Communities Model evaluation from #CMS found a clear link between addressing health-related social needs (HRSNs) and reducing cost of care. In particular, beneficiaries who were screened for HRSNs and eligible for service navigation experienced the following benefits, compared to a control group: ⬇️ 3-4% reduction in total Medicaid/Medicare expenditures ⬇️ 4% reduction in inpatient admissions ⬇️ 5% reduction in emergency department visits At Papa, we see these same trends in our population—at an even higher rate. By connecting members with in-person Papa Pals, we help address the social needs that can drive members to over-utilize healthcare services and ensure they have the means to follow through on referrals and recommendations. CMS’s findings reinforce what we’ve long known: social health is health, and addressing it as such can help us move the needle on some of health care’s biggest challenges. November is National Family Caregivers Month—a time to recognize the challenges caregivers face and celebrate the vital role they play in our society and healthcare system. At Papa, we’re here to support caregivers by connecting them and their loved ones with Papa Pals—trusted companions who provide in-person help, like running errands, driving to appointments, and spending time together to ward off isolation and loneliness. For the many caregivers who need someone physically present with their loved one, this kind of support can be life-changing. It was for Desiree, a full-time working mom who lives an hour away from her family, to whom she is an only child and grandchild. Papa Pal Jessica started visiting Desiree’s mother and grandmother, taking them to appointments and simply offering friendship and companionship. It didn’t take long for her to become a part of the family. As Desiree said, “Jessica has become my mother’s best friend.
