Advanced Healthcare Administration assists individuals in finding long-term housing with stability, providing a secure residential environment that acts as a bridge to permanent housing. We offer a continuum of support and resources to ensure sustainable living arrangements for those we serve. AHA is a vital partner that fulfills Department of Healthcare Services (DHCS) needs and assists Managed Care Providers (MCPs), clinics, correctional facilities, hospitals, nursing homes, and housing management organizations in identifying enrollee health and social needs. We collaborate with you to deliver Community Support Services by referral, ensuring that DHCS members receive the specific services they require with the utmost respect and care, while collecting Social Determinants of Health data that aid Partners in care planning and contribute to MCPs’ population needs assessment. Our journey to support begins with safe and reliable housing, but doesn’t stop there. AHA connects people in need to the following Community Support Services: - Health Insurance Enrollment - Food Insecurity - Financial Security - Social Support - Transportation - Legal Aid - Employment Services - Physical Support
