Is there anything that our facility does extra well? Would you be willing to answer a quick survey after your visit? Would you like to join our monthly get-together for family members of our residents? For security: Have you recently traveled outside the US? Do you have a new cough or cold symptoms? Do you have any alcohol or tobacco products with you? Are you a family member of a resident? Are you part of a visiting group or organization? Customized questionnaires are one way to get feedback from the different groups within your facility – for improvement, strengthened security, and other areas you wish to focus on.
