(i.e you are in trouble) Too ascertain what actions/procedures were breached. A clear understanding of what was done wrong and why you cannot do things this way. Seek a commitment from the employee to rectify their actions in the interest or a safer workplace for all. A warning that they will be watched going forward and a return to work assessment of the task breached. Promulgate the incident to the wider workforce as a reminder why we do things and why it is an unexpected practice. It is NOT a problem to make a mistake. It IS a problem not to learn from the mistakes. KISS (keep it simple/safe)
