This week the report ‘A Digital Health Record for Every Citizen’ was published by the team at the Tony Blair Institute for Global Change (TBI) following significant contribution by Feedback Medical. The report aligns to our vision of the future NHS and outlines a vision for a single patient record which unlocks transformational models of care with the potential to move service delivery out of acute providers and into the community, as called for by Rt. Hon. Wes Streeting MP, Secretary of State for Health and Social Care, when he announced that the NHS needs to transform into a #NeighbourhoodHealthService.   FEEDBACK PLC strongly champions the position that the Digital Health Record should be built out from the current core record for the patient, namely the primary care record. Our product Bleepa can augment the primary care record with diagnostic information, making it a one-stop-resource for information relating to the patient. Bleepa’s collaboration and pathway management capabilities further allow the record to be opened up to multiple care settings, ensuring that each provider can work off a common record in a fully informed and connected way.   This builds off the diagnostic pathway model that Bleepa already delivers in the community diagnostic centre (CDC) space, leveraging Feedback’s connections into primary care, secondary care and diagnostic facilities. Bleepa is already a digital glue that brings these providers together around the patient and their journey.   We believe the vision outlined in the TBI paper can be made into a national reality and we look forward to engaging with NHS colleagues and potential primary care partners to take this to patients in a broad range of settings including community care, mental health, social care and ambulance services. All you need is an explanation? 🗣 ❓   I enjoyed the latest joint RSM Digital Health Section and @Dev&Doc podcast featuring Dr Annabelle Painter, Dr Josh Au Yeung, Zeljko K. on #XAI.   𝐓𝐡𝐢𝐧𝐠𝐬 𝐦𝐞𝐧𝐭𝐢𝐨𝐧𝐞𝐝 𝐭𝐡𝐚𝐭 𝐈 𝐟𝐨𝐮𝐧𝐝 𝐢𝐧𝐭𝐞𝐫𝐞𝐬𝐭𝐢𝐧𝐠: 🧠 We need to trust a solution to accept it's decisions, and part of trust is understanding why - therein lies why we find explainability so important but actually it really boils down to our ability/willingness to trust?   🧠 We frequently use the terms ‘#explainability’ and ‘#interpretability’ with respect to AI as interchangeable, but they mean 𝘥𝘪𝘧𝘧𝘦𝘳𝘦𝘯𝘵 things (see: https://lnkd.in/eKuNMbXx.)   🧠 When we talk about explainable AI – do we mean explainable for each decision or how the system as a whole is working or both? And are they equally important? Also, could we even understand it or find it relevant (e.g. complex mathematical reasoning).   🧠 Whilst we expect AI to be explainable, is this a double standard when we don’t even know sometimes how humans make decisions? 🤷‍♀️ When referring for specialists/tests sometimes we have ☑ that says ‘clinical concern’ for when we can’t explain why a patent needs help – so if we can’t always explain our own intuition and make allowances for that, maybe we need to for AI? 🤔   💡 Fascinating point re: whether having an explanation makes doctors more likely to accept a false result. So could this actually worsen our over-reliance to AI? 😨  Something I saw raised in a Nature letter here published on explainable AI in dermatology. Lauren Oakden-Rayner gave a lecture several years back and discussed a 2015 study about how pigeons 🐦 could detect breast cancer on pathology images (https://lnkd.in/eD3beHN2 😂 ) and argued how they could be seen as a kind of ‘model’ outputting binary classification with high accuracy but we’d never dream of thinking of a 🐦 = 👩, yet we do this comparison with AI.   🤖 The topic of human-AI interaction and decision making cropped up a few times as an area of much needed research – although I’m certain there must be a lot out there given all the courses/degrees/jobs related to #HCI and #UX? No?   Anyway - lovely episode! Would enjoy hearing from anyone working on exciting projects that cross behavioural & social science / medtech. Always a fan of Freakonomics and how predictably unpredictable we can be with decision making…!

Given the positivity is greater for non clinical use cases, this is where we should start. 🌱 Fascinating that younger people feel AI may make care quality worse, we need to be co designing and engaging with diverse groups for this to work best. 🖥 Concerns around decision making are valid and we need more research on human AI interaction in healthcare. Theres loads more so do read the article. The UK's delicate industrial strategy balancing act: our service industry must embrace GenAI, but the risk is that we hand our future to US BigTech. My article in Bright Blue's new collection of essays on the impact of Generative AI, alongside such luminaries as Kenneth Cukier Rose Luckin Gerard Grech Tom Watson. "It is easy to be cynical about endless promises surrounding the transformative potential of AI. However, even if the current break-neck pace of technological development were to slow down, we are already seeing the first stirrings of a productivity revolution, especially in the services industry. This matters because the UK is the second-biggest exporter of services in the world, and, at present, we face a conundrum. If we do not embrace the AI opportunity, then we risk falling behind and losing our competitive edge. But, if we do fully embrace AI, we risk handing the keys to our economy over to the US tech giants on whose AI platforms our technological future depends." Let's hope that the next Government has the bandwidth and vision to focus on both aspects of this challenge. I've been reading interesting posts on the recent papers looking at the use of Generative AI to assist with clinicians responding to patient queries. First up we have "The effect of using a large language model to respond to patient messages" in Lancet Digital Health (24 April 24), and then "AI-Generated Draft Replies Integrated Into Health Records and Physicians’ Electronic Communication" in JAMA Network (15 April 24) - links in comment. Both are well worth a read, and indicate some significant shortcomings in the current implementations. LLMs appear to generate replies that can cause harm, or are implemented in such a way as to increase the amount of time needed to respond. Neither is helpful! There are positive findings in each study though, so something to build on. HOWEVER: the prompts are either not shared in the papers, or where they are appear to be woefully concise and lacking in detail or structure. It's the equivalent of finding someone and saying "can you reply to these please?". Sure, examples are given, but we know how important instruction quality is to the final outcomes of any model. Is it any surprise that they underperform when operated this way?

Raindrops are a great metaphor for the #digitaltransformation of Healthcare. What do I mean? The promises of the digital health revolution are many. Decreased workload, improved diagnostics, improved efficiency, greater access, enabling technologies etc. Furthermore, their wide adoption seems imminent! Not only does it make sense, but in every other industry, those same technologies are in place and delivering on their promise. In fact, the digial revolution should hit healthcare like a tsunami and allow us to start over on some version of the medical bay on the Starship Enterprise. It is a cruel experiment: your car drives you to work but when you get there, you are faxing patient referrals. A patient can book an international flight but when they try to book a clinic appointment, the task is insanely complicated. But not everywhere. At Kaiser Permanenet in NCAL, it is faster to book an online appointment than to do it on the phone (I did it). Some patients have the benefit of a sci-fi avatar driven all knowing virtual assistant to help them recover from surgery. A couple of posts back, I talked about the speed at which smaller hospital groups are able to adopt new technology efficiently. Unburdened by legacy tech stacks and outdated leadership models, they can actually identify pain points for which there are mature digital solutions and implement them. Done. Each of these smaller groups, or digitally enabled private practices, represents a rain drop of technological adoption. Over time, adjacent raindrops will coalesce, competition will lead to curiosity, curiosity will lead to familiarity, familiarity to trust, and trust to broad adoption. In the 1990s, many proclaimed that sequencing the human genome would usher in a wave of cancer cures within a decade. Nearly 40 years later, we are starting to see some raindrops. Today, it's AI. We hoped that a computer science tsunami would wipe the healthcare slate clean, but instead, we are getting raindrops of success and people are either surprised or disappointed. We should not be. For each raindrop, in that location, the job is done. Eventually, others join in. The network effect grows. And it grows quickly. So: go ahead, be a raindrop. Lead the pack. At the Seventy-seventh World Health Assembly, a resolution was endorsed to embed regular and meaningful social participation in decision-making processes for health. This is one of the critical components for leveraging #AI in the healthcare context. (Link to full article in comments) To Achieve Meaningful Social Participation we need to transform at 3 different levels: Individual Level: Cultivate a #mindset of service and a desire for truth. Engage sincerely and transparently, prioritizing collective well-being. Community Level: Promote behaviors consistent with agreed-upon values. Ensure diverse voices are heard, and AI technologies are designed with community input. Institutional Level: Evolve policies to support continuous engagement. Thinking about using ePRO in France for older Oncology patients? You might hit some speed bumps. 🛑 "Conclusion Our multicenter study showed a limited feasibility rate (26%) of remote ePROs monitoring for older patients with cancer, mainly because of technology barriers. Yet, among the patients who did have Internet access, most of them indeed used ePROs (66%). " 👀 The only thing I can bring to ePRO's defense is that this hospital group probably didn't consider offline data capture for ePRO, but there are other barriers as well. Just shows once again the answer isnt <<always>> ePRO. But it usually is :). PS. “According to Pitchbook Q1 2024, funding for telehealth and digital therapeutics was $1bn for the quarter, similar to the prior three quarters. I find interesting that the smaller VCs are the most active. Alumni Ventures was the most active digital health VC investor in the first quarter with four deals, followed by Connecticut Innovations with three deals. Since 2020, General Catalyst, Alumni Ventures, Gaingels, Optum Ventures, and City Light Capital have led digital health VC investments. Stay tuned this month for my blog post that shares ideas of new business models (despite their current modest size), and the emerging funds who invest in them.
