"The Big Elephant in Gastroenterology: Mental Health Refocus" In the realm of gastroenterology, numerous clinical challenges intertwine with psychosocial elements. These factors not only impact patient prognosis but also play a significant role in shaping the doctor-patient dynamic, often introducing tensions. Embracing psychosomatic gastroenterology doesn't entail gastroenterologists becoming therapists or psychiatrists. Instead, it encourages us to transcend the confines of the biomedical or traditional psychiatric models. By incorporating insights from psychology and psychiatry, we can enhance our clinical approaches, fostering better outcomes for our patients. It's imperative to shift away from a purely biological perspective and embrace a more holistic approach that integrates biological, social, and psychological facets into gastroenterological practice. You can substantially lower your chances of having a heart attack or stroke by taking the following steps to keep your heart and blood vessels healthy. 𝙈𝙖𝙣𝙖𝙜𝙚 𝙮𝙤𝙪𝙧 𝙙𝙞𝙖𝙗𝙚𝙩𝙚𝙨 𝘼𝘽𝘾𝙨: Know your diabetes ABCs to help you manage your blood glucose, blood pressure, and cholesterol. Stop smoking if you have diabetes to lower your chances of developing heart disease. 𝗔 𝗶𝘀 𝗳𝗼𝗿 𝘁𝗵𝗲 𝗔𝟭𝗖 𝘁𝗲𝘀𝘁: 👉 The A1C test shows your average blood glucose level over the past 3 months. 👉 This is different from the blood glucose checks you do every day. The higher your A1C number, the higher your blood glucose levels have been during the past 3 months. High levels of blood glucose can harm your heart, blood vessels, kidneys, feet, and eyes. 👉 The A1C goal for many people with diabetes is below 7%. Some people may do better with a slightly higher A1C goal. 👉 Your A1C goals may also change as you get older and your lifestyle changes. Ask your health care team what your goal should be. 𝗕 𝗶𝘀 𝗳𝗼𝗿 𝗕𝗹𝗼𝗼𝗱 𝗣𝗿𝗲𝘀𝘀𝘂𝗿𝗲: 👉 Blood pressure is the force of your blood against the wall of your blood vessels. 👉 If your blood pressure gets too high, it makes your heart work too hard. High blood pressure can cause a heart attack or stroke and damage your kidneys and eyes. The blood pressure goal for most people with diabetes is below 135/85 mm Hg. Ask what your goal should be. 𝗖 𝗶𝘀 𝗳𝗼𝗿 𝗖𝗵𝗼𝗹𝗲𝘀𝘁𝗲𝗿𝗼𝗹: 👉 A buildup of cholesterol, a form of fat found in your blood, can cause a heart attack or stroke. 👉 Ask your health care team what your cholesterol numbers should be and the risk of developing heart disease based on your cholesterol profile. 𝗦 𝗶𝘀 𝗳𝗼𝗿 𝗦𝘁𝗼𝗽 𝗦𝗺𝗼𝗸𝗶𝗻𝗴: 👉 Quitting smoking is especially important for people with diabetes because both smoking and diabetes damage blood vessels, so your heart has to work harder. 𝗘-𝗰𝗶𝗴𝗮𝗿𝗲𝘁𝘁𝗲𝘀 𝗮𝗿𝗲𝗻’𝘁 𝗮 𝘀𝗮𝗳𝗲 𝗼𝗽𝘁𝗶𝗼𝗻 𝗲𝗶𝘁𝗵𝗲𝗿. 👉 If you have Diabetes and are worried about heart disease, go consult your nearest cardiologist and get screened for risk factor for heart disease. Julius Goh was diagnosed with hearing loss at birth through the Universal Newborn Hearing Screening Programme at KK Women's and Children's Hospital. Following consultations with multiple doctors, his parents proceeded with his first cochlear implant surgery at 20 months old and a second implant surgery at 10 years old. In 2020, with the aid of government subsidies, Julius upgraded his cochlear implant equipment, enabling him to hear and communicate better, even in noisy environments. The Bluetooth function directly streams calls to his implants, boosting his confidence in answering telephone calls. Based on ACE’s evaluation, the MOH Medical Technology Advisory Committee recommended bilateral cochlear implants (BCI) for the management of children (<18 years old) with severe-to-profound sensorineural hearing loss in both ears for inclusion on the MOH Implant Subsidy List (ISL). This subsidy helped to ease the financial burden on Julius’ family. 📣 Research of the month 👏 Pulmonary hypertension (PH) causes high blood pressure in the lungs due to changes in the muscularization of blood vessels, making them stiffer and harder for blood to flow through. Untreated, this can eventually lead to heart failure. CPI researchers Xuran Chu and colleagues from the Justus-Liebig-Universität Giessen found an important therapeutic target for the treatment of PH. Vascular smooth muscle cells (VSMCs) increase pulmonary vascular resistance and stiffness as they emerge around blood vessels during PH. Until today it is assumed, that new VSCMs derive from already existing VSMCs that expand distally along the vascular tree. With the use of two independent models of PH in mice (Hypoxia and cigarette smoke exposure) Chu et al. show a major additional source of VSMCs during PH pathogenesis. Selective ablation of mesenchymal cells expressing the sonic hedgehog target gene glioma-associated oncogene 1 (Gil1) attenuates PH development in mice. Chu et al. show evidence that their findings are translatable to human Group 1 and Group 3 PH and therefore open a new avenue for testing novel therapeutic interventions. Self care is key to optimal management of hypertension, which ranks as among the top medical conditions treated in SingHealth Polyclinics and leading cause of vascular mortality and morbidity in Singapore. Delighted to share with clinicians from the Asia Pacific region on how we can motivate and nudge patients in self management. Afterall they only see their doctors once every few weeks to several months. In between consultation patients have to look after themselves well daily which is indeed difficult. As family doctors we are in a position to elevate their self efficacy and self-care, be it choosing healthier food options, moderate indulgence in rich food, exercise more frequently, self-monitoring of home blood pressure, stop smoking and reduce alcohol intake.
