Intent is good, but that's where these claim ends 🙂 Currently the use cases of AI in Indian Healthcare are very limited within Diagnostics (radiology, pathology), Insurance claims processing, Admin workflows, etc. We are far, far away from real clinical AI use cases where it would be used to prevent, predict, and manage care for patients. Truly assisting front-line doctors to better care for patients at scale. Things would change, but we need 2 things for AI to help Indian Healthcare: 1. Implementation + Clinical level adoption of HIMS and EMRs in Hospitals  2. Interoperability of these products within themselves and with other products --- --- 𝗛𝗼𝘄𝗲𝘃𝗲𝗿 𝗛𝗜𝗠𝗦 / 𝗘𝗠𝗥 𝗺𝗮𝗿𝗸𝗲𝘁 𝗶𝗻 𝗜𝗻𝗱𝗶𝗮 𝗵𝗮𝘀 𝗯𝗲𝗲𝗻 𝘁𝗼𝘂𝗴𝗵: > Small market with fragmented vendors > In such a fragmented product quality takes a back seat > Hospitals & Clinics still sees this as a cost. So implementation and adoption is a big issue Indian Hospitals HIMS adoption is ~20-35%. But for most hospitals, HIMS primarily means Billing, Admin, Inventory, Patient Registrations at best. ❓ What is the solution for B2B Health Tech Distribution? Currently, 12+ startups offer no-cost EMI and interest-free medical loans. These startups can forego the interest because of the inherent business model, which includes below revenue streams (one or more) 🔷 One-time partnership or subscription fees from hospitals. 🔷 Late payment penalties on EMIs 🔷 Revenue from cross-selling products like insurance or teleconsultations 🔷 Monetize the patient data (after reaching certain scale) 🔷 Transaction fees charged to hospitals, typically 20-25% of revenue. In this article, 'industry experts' highlight that the main challenge for medical lending firms isn't just technology or market reach but also maintaining credit quality.   IMO, from a larger societal viewpoint, the main challenge is not about credit quality but reducing the overall cost of healthcare to make it more affordable #medicallending

APPT - My pillars for entrepreneurial success. For me, key reasons for whatever success I have achieved as an entrepreneur is due to these key principals - Adaptability, Persistence, Patience and Team. I have built, scaled and successfully exited from multiple businesses - each one of them pivoted significantly from the original idea. So, for me, being successful at entrepreneurship is not about having some unique idea. It's rather about ability to listen to market feedback, to accept that initial Excel sheet is not working, to accept changing market conditions - in short, being able to adapt for me is one of the key pillars to success. Next for me is the ability to persist - to remain focused on defined goals, to keep executing the plans, and to not let noise around you distract you from moving forward. Then, a realization that entrepreneurial journey is not a sprint, but a marathon, and that too one of the toughest one that is out there. So being patient through the chaos is the only way one can survive and succeed in this journey. And the biggest pillar for success - team! Having people who are aligned with your vision, take ownership, can absorb chaos and go beyond their KRAs for organization's growth - without this key ingredient, success is simply not possible in my view. It's in this context that I welcome Vikas to Healic! Vikas joined us when we started Hush in 2017. His key role was site expansion for Hush's F&B outlets. Everyone slogged together while business went through initial pivots and then we started tasting success. And then Covid stuck, and Hush had to temporarily shut down operations and Vikas had to unfortunately move out. I however always regretted Vikas leaving, because, he is the kind of person who I would always want around me in any of my entrepreneurial journey. And now, with Healic, after going through pivots and after stabilizing the business model, I was looking for a person who could work on expanding Healic across India, and Vikas is the first name that came to my mind! And he too readily agreed to join back! Healic has an amazing set of people now and each one of us is now working towards the vision of making Healic the largest neighborhood hospital chain of India, of positively impacting lives of millions across the nation, and to bring a noble disruption to the sector! In a government hospital in India, the doctor will only see you for 2 minutes. According to a 2017 study by the British Medical Journal, doctors in India spend an average of two minutes with each patient. This is one of the problems that Parchaa is solving and Saumya Rawat Pandey, MD, MRCP is leading the charge. She described grueling 36-hour rotating shifts, coming home for just 12 hours every two days. It really highlighted the intense demands placed on healthcare professionals, especially during crises. What struck me most was how Dr. Pandey's background as a practicing physician informs her work in health tech. She has a unique perspective, understanding both the clinical needs and the technological possibilities. When talking about the topic of building personal brands, Dr. Pandey's insight that anyone not actively showcasing their expertise on social media today is leaving money on the table, really hits home. Single Speciality Hospitals <> Startups of Hospitals In 2023, Hospitals in India inked funding deals worth $5.5 billion. 20% of it went to Single Speciality Hospitals (SSH), mainly from growth focused funds. Angel investments in hospitals has grown exponentially in last 8 years. Some of the prominent investments are: 1. Quadria Capital invested $102 million in NephroPlus. 2. AHH invested$75 million in AIN&U 3. EQT invested in Indira IVF 4. Centre for Sight acquires Laxmi Eye Hospitals 5. CX Partners acquires Sabine Hospital Benefits of Single Speciality Hospital (SSH) vis-a-vis Multi Speciality Hospitals (MSH): 1. Multi-speciality hospital with 300 beds takes 5 years to breakeven 2. Scalable - Indira IVF, Motherhood, Center for Sight 3. High ROCE - SSH has ROCE (return on capital employed) of 25% whereas MSH has 12% with exceptions like Max which has 35% 4. Less Space - SSH needs 5000 sq feet against MSH 25000 sq feet 5. Convenient - Hospitals are usually started by individual doctor specialised in single, therefore convenient to start SSH. 6. Quality Care - with growing purchasing power capacity Indian middle class is ready to pay premium price for quality care. Challenges: 1. SSH has Low ARPOB (average revenue per occupied bed) INR 40,000 against MSH ARPOB of INR 60,000 2. Less Experienced Doctors on roll 3. Focus on Premium services which are mostly not covered under Health Insurance therefore high OOPE 4. SSH has struggled to hold their Focus area. eg. Orthopaedics focused Shalby Hospitals & gastroenerology focused speciality hospital AIG are now MSH (multi-specaility hospitals). Strengths & weakness of SSH are similar to startups. However, unlike startups which are tech based & can give exponential returns, SSH has life-cycle of 7 years before it becomes MSH. Rules remains the same - start small, make a sustainable business & then grow. With new medical colleges, institutional funding, ABDM, HCX, the healthcare in India is strengthening its infrastructure & institutional ecosystem so that the growth can be fast-tracked & grow like Highway's in India has grown in last 10 years. So, Koo shut down. There are three schools of thought that I can see. ✳ I see Nishant Mittal and Harsh Shethia being upset about the amount of capital wasted. ✳ The broad majority subscribe to the opinion which we know as, "What goes my father?" Basically, not their headache. ✳ Then there are a couple of entrepreneurs who I was speaking with yesterday who told me that they think such failures should be celebrated. 👉 VC capital is meant to be risky. LPs who invest in VC funds invest with the goal of outsized returns to balance their portfolio. Remember, if VC funds didn't take risky bets, we wouldn't have any of the iconic companies of today. Bessemer Venture Partners records their misses. Did you know they passed on Google, Apple, Airbnb and many such companies early because they felt that the valuation of the company was too high? You don't know as a VC whether a company will succeed or not because there are way too many variables at play. So, you take a bet on what you know, who you're familiar with and you use proxies for the rest. Otherwise, most funds will need a year of thorough DD with a company to identify their real weaknesses. That is not viable. Don't we use proxies to make decisions in the absence of enough data in our daily lives? So do VCs. They use IIT/IIM founders, trending topics, winds of change, competitive landscape etc as indicators. Btw, that's what entrepreneurs use when hiring and building their companies as well. 👉 Are a lot of deserving companies not getting funded while risky bets like Koo waste money? Traditional capital such as banking and HNIs used to be reserved for certain communities with access to the network. VC funds have kind of created an equalization. Not completely. 👉 So, we can't eliminate proxies in an opaque market. What does a founder with a good idea and no network do? Speaking for myself, I knew no one and yet raised a decent amount of money. What did I do? Worked years building a team, building a network, building connections, and, most importantly, building expertise and knowledge. With Darwin at play, entrepreneurship isn't equitable in any way. You learn to use your skills, learn to rise above the canopy for sunlight through sheer hard work built over many years. Does that mean that we should celebrate Koo's demise? Never. We should analyse like Nishant has done. He's done an excellent job in his analysis (I would've used less vitriol). Low differentiation, small untested TAM, inability to monetize - the top three reasons why Koo failed. Could Koo have succeeded? Again, it's Darwin at play. They didn't need to follow Twitter's playbook. After all, Twitter hasn't found their own sweet spot yet. Should we care about it? Yes. The amount of capital in the market is limited. Ultimately, each failure contributes to the evolution of the startup ecosystem, shaping the path for future founders and innovations. Ce la vie. To the next failure. Do you agree?
