Despite playing an extraordinarily important role at the centre of Healthcare Delivery & Management, the 'Practice Management Software' industry seems to go by unnoticed within the VC / Startup community. I recently met (& had a great conversation) with a partner from a prominent German VC, he chucked and told me that he thought Saas was 'done', that all of the major industries had been disrupted already. Then someone drew his attention to 'Practice Management Software'. 👀 🧠 High-Level, what is it? 👨🏻‍⚕️👨🏻‍💻🤒 The software by which Healthcare Professionals within medical practices manage all patient administration & regulated processes. 🩺💊💰 Such as: Health Records, Medication Prescriptions, Medication Plans, Medical Forms, Insurance Management, Billing, Lab Data, Device Data, Waiting Room, Calendar, etc. 👩🏼‍⚖️✍🏽✅ The software is complex, highly Regulated & Mandatory: if a patient is state insured (90% of the german population) a doctor 'must' be using a regulated PMS in order to treat them. 🏥🇩🇪💰The German healthcare market is the 2nd biggest in the world (after the US) with a yearly spend of 13% of GDP. 🏥🏥🏥 There are a LOT of medical practices in Germany, over 110,000 of them in-fact (especially when you compare to a country with a similar population size, e.g. the UK, with 7,000 practices). What is the current state of affairs: 📠 The highly regulated PMS industry has remained relatively undisrupted over the past 30+ years. 📀 Most practices are still using software that was developed in the 80s/90s: DOS based, On-premise, Offline/No Cloud, No modern UX, No proper interoperability. 📉 By far the biggest player in Germany is CGM, who have the largest % of the doctors using one of their softwares. However, when you look at the stock price over the past few years, things do not appear to be going so well, but why? 🔮 📉While still turning over large amounts of revenue, the future does not appear so good. - Government & Regulators are pushing for meaningful nationwide digitisation. However, with PMS at the heart of how HCPs work & health data is created/stored, this digitisation feels like it's being blocked by the legacy PMS. - There is no modern cloud based system. - A lot of leadership/vision changes over the past years. - The general customer satisfaction is very low. The need for PMS is not reducing, but the likelihood/belief that this company will be the one to provide it in the long-term future is diminishing. Meanwhile: 🥇We at doctorly are the first (& only) VC backed tech-startup to be certified to sell Practice Software in Germany. 📈 With hundreds of HCPs already using doctorly to fully manage their practices, we aim to fill the evident & growing need for a modern PMS over the next years. 🤝 Beyond an efficiency boosting PMS for HCPs, we also intend to provide a platform for true interoperability & data flow across the healthcare industry. #healthcare #digitalhealth #healthtech

3 noteworthy acquisitions were announced in #DigitalHealth last week, moving the total to 76 acquisitions in 2024 till date. - On Tuesday, Switzerland-based Mediktor announced the acquisition of Sensely, a San Francisco-based digital healthcare provider that pioneered an empathy-driven conversation platform to support hospital systems and insurance services with world-class member engagement. - On Thursday, Carevīve was acquired by Health Catalyst. The acquisition strengthens Health Catalyst's position in the digital healthcare market, expanding its capabilities in patient engagement and care coordination. - On Friday, Doktor.se announced that it acquired competitor Doktor24. The combined entity will provide a comprehensive suite of digital healthcare services, including telemedicine consultations, patient engagement platforms, and data analytics capabilities. Compared to last year, the total number of acquisitions is still trending significantly lower. HLTH Europe is coming to a close today, and that is a good time summarize. 1. We are in an age of AI buzz at the max, almost every panel had AI mentioned multiple times. 2. Medical dat is key, but making it available and integrating with it is still not a simple task in every country, and in order to unlock the value, there is still a road ahead - Standardization, API's and also process. 3. Start ups have amazing concepts and tools, but because of point number two I think it will be time for platform integration approaches as it will become an issue managing so many elements in to health system IT infrastructure. EHR vendors have already started, but it needs to get easier, better and more user friendly. 4. Healthcare is a glocal system - while the research and human body is mostly the same across the global, the nuance and local systems are very different, and maybe in devices, medicines you can usually stick to a product, when it comes to digital and AI solutions the amount of customization, and business models might kill you on the road to commercial viability. In Oct 2020, in a bold move, Germany approved the first digital therapeutic (DTx) for use and reimbursement through its DiGA program. In lay terms, a mobile app can, when meeting clear criteria, serve as a medical device and as such be prescribed by physicians to patients. For that, the developer can collect money from insurance. Przemek Grzywa has neatly summarized how the program has fared so far. While still tiny compared to traditional therapeutics, I see clear success and tremendous potential for both human health and the medical technology sector!

It is only the first edition, but HLTH Europe has already established itself as the leading #HealthTech conference in Europe. Besides my 4 highlights, I also had the chance to moderate a panel on the topic "Last mile: Localising access for underserved areas" with 3 fantastic panelists: · Prathibha (President, Mayo Clinic Health System) · Minal (National Director of Primary Care and Community Transformation, NHS England) · Harmony (SVP, Chief Medical Officer & Head of Global Medical at ViiV Healthcare) PS: Amira (SVP Innovation at Siemens Healthineers): we missed you! Here are the key insights of the panel: Bringing healthcare #access to all, especially to underserved areas, has been a challenge since the inception of healthcare. With all the technology of our modern world, is #Healthcare access on the verge of being solved? The short answer is no. But there is hope. In the UK: ➝ There is a 15-year life expectancy gap between deprived areas, and wealthier neighbourhoods nearby. ➝ Funding struggles to keep up: In the next 15 years, a 37% increase in "ill-health costs" is expected, while the working population will increase by 4% over the same period. ➝ Increasing productivity is essential to solve the problem: A 1% decrease in productivity costs 1.5 billion pounds. In the US: ➝ The zip code is (still) a better determinant of health than the genetic code. ➝ Enabling access to underserved areas can also be seen as an opportunity rather than a problem. Tech adoption will be faster due to higher unmet needs, accelerating innovation that can later benefit well-served areas. Debunking one myth: ➝ For HiV, there is now a better access and coverage in Africa than in certain parts of developed countries (US and Europe). Pockets of innovation: ➝ To unlock HCP capacity, the NHS has a national program to move towards a modern General Practice, moving away from "first come first serve" to a new model based on population health and segmentation: the right care for the right person in the right way. First results show that 30% of GP appointments could be done via "messaging only" (instead of F2F visits), bringing massive efficiency gains. ➝ Mayo Clinic already has 75 specialties that are technology enabled, bringing the “Mayo medical care" in the rural areas via virtual and hybrid models, and also embedding the 50+ AI algorithms already active at Mayo. ➝ But increasing access is not only about Tech. Sometimes, it is about boldness: Viiv Healthcare, a pharma company specialised in HiV, gave the patent of its drugs away in 90+ underserved markets, enabling cheap and at-scale production. What does the future hold? Will someone living in an underserved area be able to visit the "Mayo Clinic in the metaverse with his / her digital twin?" (There are many private hospitals that operate on a fee-for-service basis, incentivising them to adopt innovations that improves client experience, reduce cost or provide them a competitive edge) 3. (Countries like Hong Kong, China, and the majority of ASEAN countries are far less stringent when it comes to regulations, making it easier to go to market, develop your products or generate clinical evidence) 4. 𝐓𝐡𝐞𝐫𝐞 𝐚𝐫𝐞 𝐥𝐞𝐬𝐬 𝐢𝐧𝐜𝐮𝐦𝐛𝐞𝐧𝐭𝐬 𝐜𝐮𝐫𝐫𝐞𝐧𝐭𝐥𝐲 𝐢𝐧 𝐩𝐥𝐚𝐜𝐞. (Meaning less competitions and potential to leapfrog older technologies in favour of new ones - i.e. Traditional EPRs vs Newer AI based transcription software) _________________________________ 🍃 But the grass is not always greener on the other side. Ability and willingness to pay will always be chief amongst all consideration when expanding into a new market. My personal tip is to pay close attention to Tech Brews that is being led by Dr Terence Tan and Alvin Tay for the inside scoop. Here a short breakdown of some of the more interesting we tracked in HealthTech Alpha: 1) AI-powered healthcare: Preferred Networks, Inc. (JP) and Samsung Electronics partnered to develop AI-based solutions for medical image analysis and diagnosis. 2) Data interoperability: Solace (US) and CommonWell Health Alliance joined forces to improve data exchange and interoperability in healthcare. 3) Cancer diagnostics: Lunit Oncology (US) and Rezolut collaborated on developing AI-based solutions for early-stage cancer detection. 4) Personalized health: Dr Diary (SK) and Fitness 101 partnered to offer personalized health and fitness plans based on individual data. 5) Rare disease research: Unravel Biosciences (US) and the CSNK2A1 Foundation joined forces to advance research into a rare genetic condition. 6) Genomic analysis: Gencurix, Inc. and Hitachi partnered to develop AI-powered genomic analysis solutions. 7) Cardiovascular health: JLK (JP) and Heart Scan Screening Center collaborated on cardiovascular screening programs. 8) Sports medicine: Craif Inc. and Nagoya Grampus partnered to improve sports medicine and injury prevention. 9) Chronic disease management: Modivcare and the American College of Cardiology partnered to improve care for patients with chronic conditions. 10) Cardiac devices: PaceMate® and Medtronic collaborated on developing innovative cardiac devices. I’m expecting no drama since EMR data is limited inaccurate, (physicians counldnt care less for Emr accuracy, we want to treat the patient and not the computer), and represents only a shallow and narrow picture of a round figure - the patient with needs, culture, wants, needs, risk aversion profile and so much more. Medicine can’t be one size fits all. 2. AI will make headway in “simple rule” “limited options” fields like imaging and pathology. It will also add so much demands to the workload of pathologies and radiologists through significant incidental finding systems under development. These fields will evolve and demand for good pathologists and radiologists will probably grow. 3. IOT, wearables, home monitoring, telehealth, is evolving but not speaking to the EMR. Most offerings are speaking to propriety systems and clinicians needing to interact with yet another system. This will not help scale of home care and monitoring. 4. TMI we are making too much information and desperatly need good systems to summarise and alert clinicians only when needed. 5. Medical devices - many encouraging developments that empower primary care. From Pocus and pulmonary test to colonic polyp screening our offices will see more and more technologies that will maintain the high value patients are gaining from primary care. 6. EMR’s are the root of most evil and this needs to change. Most are closed gardens expensive and limited. Open EMR and FIRE initiatives are not moving fast enough and we are stuck with suboptimal systems that contribute to burnout and limit our capacity to predict and be proactive to our patients. 7. Israel’s health-tech scene is surprisingly developed and with our excellent healthcare system we are at a place we can be a global leader in the field. Any thoughts?

Exciting news from the healthcare innovation front, for those of us passionate about improving access to and the quality of #dementiacare. Mass General Brigham Ventures portfolio company Rippl has announced the acquisition of Kinto Care, a move that promises to enhance dementia care significantly. As a medical speech-language pathologist and dementia care specialist, this fills me with hope, knowing that our patients and their caregivers will have a chance at better access to high-quality care. Also excited to see the solutions being offered by Harmonic Health and Neuroglee Health in this space. I’ve seen first hand how difficult it is for individuals with dementia and their caregivers to manage every aspect of their care, from diagnosis to evidence-based treatments, navigating the compexities of our healthcare and reimbursement systems, and receiving adequate education and support throughout the neurodegenerative process - right until the end of life. It’s challenging and isolating and debiltating. The integration of #generativeAI #healthcaretechnology and digital health solutions for dementia care look like they will not only support patients but also caregivers, families, and clinicians. It's a step forward in accelerating all our collective missions to provide specialized and personalized access to care to more adults and families in the comfort of their homes. How compared to many digital health companies, Mykare’s metrics will be among the best in most areas✨🔥 One area I always want to focus 🧘‍♂️ on as a founder is maintaining the spirit and motivation of both myself and the team 🌟. Trust me, it can be challenging. There are times when you might be fully energised ⚡️ while the team isn't, and vice versa. But over time, I've learned one crucial thing: if the team truly understands what you’re aiming for— what you want to achieve, how you plan to get there, and how everyone will benefit, along with our customers—they'll be more aligned. ✨ To make that happen, the problem we’re solving needs to be clearly communicated, discussed, and understood by everyone. If they don’t understand it, they won’t grasp why it matters or how they can contribute to the solution.
