Pharmacare, a single-payer pharmacy benefit system for Canada, has moved a big step closer to becoming a reality with the passage of Bill C-64 by the House of Commons. However, private health insurers, the Canadian Chamber of Commerce and the pharmaceutical industry have expressed misgivings about the proposals.   The intent of Bill C-64 is “to support the development of a national formulary of essential prescription drugs and related products and to provide for the development a national bulk purchasing strategy.” In the first instance, pharmacare will “provide universal, single-payer, first-dollar coverage—for specific prescription drugs and related products intended for contraception or the treatment of #diabetes.” The program could subsequently be expanded to all essential medications.   Critics express concern about the “universal, single-payer, first-dollar coverage.” Stephen Frank, President and Chief Executive of the Canadian Life and Health Insurance Association, said: “Late last month, the Minister of Health told MPs on the health committee that those who have an existing drug plan are going to continue to have access to those plans and the coverage they provide. If that’s the Minister's intent, it is not at all clear from Bill C-64.” Frank warns that a single-payer system could disrupt the employer-sponsored drug coverage currently received by most of the population. The Canadian Chamber of Commerce is similarly concerned that pharmacare could “jeopardize the private insurance of 27 million Canadians.”   Innovative Medicines Canada noted that, “already, 97.2% of Canadians have access to pharmaceutical coverage, and insurance gaps are typically small and geographically concentrated. This presents a clear and feasible opportunity to introduce a pharmacare system that takes care of uninsured and under-insured Canadians. While public statements made by the Minister of Health indicate there would be no duplication of existing coverage, the text of the bill clearly states otherwise. We are also concerned that the proposed approach would replace existing coverage models—that work well—with less effective treatment options. We urge you to take the time to allow for a comprehensive study of Bill C-64 in the Senate. Given the expedited process of the bill in the House of Commons, we believe that it is incumbent upon Senators to give the bill more careful and deliberate consideration, inclusive of all perspectives.”   C-64 states that “the Government of #Canada commits to maintaining long-term funding for the provinces, territories and Indigenous peoples to improve the accessibility and affordability of pharmaceutical products, beginning with those for rare diseases. The funding for provinces and territories must be provided primarily through agreements with their respective governments.” The success of #pharmacare will depend on support from regional governments, some of which also have continued reservations about the initiative. #drugbenefits

In my opinion, Mosaic AI Gateway is the most important part of the Databricks AI stack. Why? It provides a unified service to access, manage, and govern AI, enabling enterprises to incorporate multiple models (potentially from different providers) to build a compound system. Consider a pharmaceutical investor relations presentation... Creating an effective chatbot that incorporates such a document may require multiple specialized models including a vision-language model (for color-coded charts); a model that excels at deterministic calculations (for math tasks); and a domain-specific model (perhaps for biomarker abbreviations). AI Gateway eliminates the need for multiple systems, allows for rapid experimentation with quality evaluations, and incorporates comprehensive guardrails to enforce safety policies and protect sensitive information. This capability is allowing customers to leave behind narrow pilot projects and move into enterprise production.

Operations story: Inspiration for operationalizing delivery (in real sense!) On October 28th, 2015, my wife was admitted to the hospital for her first delivery. However, due to a last-minute her relative driven change in hospitals (a one-hour drive from our home), the situation became more complicated. After the baby was born, next day the baby was unnecessarily transferred to the neonatal ICU in another hospital. During this challenging time, I found myself at the center of everything—providing meals, handling daily hospitalization needs, and ensuring a smooth process. Learning from this experience, we decided to take a different approach during our second delivery. We eliminated the factors that had caused the last-minute hospital change, opting for a hospital closer to home with integrated neonatal care. On June 30, 2021, my wife was admitted for second delivery at 8 am. Thanks to our operationalized approach, everything ran smoothly without depending solely on me to the extent that I could work from the hospital room on that day. Since, at work, I faced a different challenge. On the same day, June 30, Quarter-end Incentive Compensation (IC) processing was underway, I and only a new associate were on the project. He was struggling with the excel workbook. Hence, I spent around an hour on a call with the associate from the hospital room, clarifying business rules to ensure the IC workbook could be processed efficiently. Reflecting on both experiences, I wondered: If we could make real-life delivery independent of my direct involvement, why not apply the same principle to IC processing? Inspired by this thought, we operationalized the IC process by creating a Tableau dashboard.

As I prepare to join you all this Tuesday, June 18, at the Pharma Brand the Age of AI, by Propeller Events and the Ontario Pharmaceutical Marketing Association Summer Party that follows, I wanted to reflect on my take on AI, given its rapid growth over the last 18 months. 1) Productivity ChatGPT has established itself as the logical evolution of the digital tools at our disposal, becoming both our personal assistant and an extension of our office tools, resulting in improved productivity. Its popularity has made it the number-one choice for many of us. We have learned how to interact with it effectively, bringing back the art of the brief (yes, what a great idea!)—context, parameters to consider, target audience, and expected results, among other things—to guide it for the best outcomes. Some of us even nickname ChatGPT, reflecting our inclination to humanize it. 2) Proprietary We have also come to understand the ethical issues surrounding AI usage, such as intellectual property protection and the risks of misuse like fraudulent use of our images or voices, or as a tool for spreading news—true or false. It is therefore crucial to understand that data quality, protection, and curation will be the cornerstone of the most effective AI systems. In this sense, the great ancient philosophers are making a significant return to remind us of the importance of developing critical thinking, avoiding the traps of biases, and steering clear of intellectual laziness. Our hunger for knowledge and curiosity should not be satisfied with pre-digested thoughts from a machine, no matter how amazing AI can be. Clinical trial stage signal detection is something that we often get asked about. Practices vary widely across companies, depending on the size of the company/ resource availability/ stage of clinical development/ molecule under development, etc. It can be a mine-field, and we often see over-engineered processes in place which can put huge strain on an often stretched to capacity internal resource pool. Here's how: Increased Patient Flow: With broader coverage for medications like contraceptives and diabetes drugs, pharmacies are likely to see an increase in patient visits. This could lead to busier pharmacies, particularly in areas where access to these medications was previously limited due to cost. Cost Management and Price Negotiation: The government's role in bulk purchasing could indeed lower drug prices, which might affect pharmacies' revenue models that were previously based on higher drug costs. However, this could also mean that pharmacies might need to adapt by focusing more on service fees rather than just the dispensing of medication. Shift in Service Delivery: As mentioned, there might be a shift towards pharmacies focusing more on patient consultation and health management services. Economic Implications: Challenges: Initially, there might be economic challenges as pharmacies adjust to potentially lower margins on drug sales. The transition might also involve costs related to upgrading technology and training staff for new roles. Long-term Benefits: Over time, the increased volume of patients, combined with government support, could lead to: Stabilization of income through service provision rather than just drug sales. As a Canadian mother, daughter, sister, aunt, professional, and friend - I welcome the opportunity to celebrate World Mental Health Day. While I now live in Basel, I remember clearly the year Bell Canada launched Bell Let's Talk. A Canadian telecommunications company working to support destigmitization of Mental health. What a critical turning point to help encourage us to talk openly about mental health and ensure that everyone is able to get the help they need and not feel alone. This is critical in our communities, and at work. The 4 pillars of a good strategy to tackle mental health: anti-stigma work including outreach, care and access, research, workplace leadership.
