Scenario 1;
- Universal Health Care or UHC is a means for all Filipinos to have a guaranteed equitable access to quality and
affordable health care goods and services and protected against financial risk. This is the main topic discussed in the
video: "KALUSUGAN AY KARAPATAN", where lot of questions regarding UHC are asked and answered. Some
of these questions being about on how to avail this universal health care rights especially for someone living in a
community where health care facilities are not yet ready to practice primary care, and we all know not all local
government units are equally as ready compared to others in terms of providing primary care because one may have
more resources than the other. As student nurses, we feel like this whole thing about some certain areas not ready in
providing primary care is a big hindrance to the goal of implementing UHC in all parts of the country. That is why I
agree that it should be a shared responsibility to have certain human resources (health care workers) from mega
cities to step in and give an aid to an areas with less resources. We need to diverse our health care workers, we need
for them to be available in all parts of the country, and not just cramped in one area just because the pay there is
higher. That is why i think all good facilities should be available also in all parts of the country, higher pay
regardless of the location (not just in mega cities). Because all people especially in this country should have the
rights to obtain health care and one that does not break the bank at that. Health is not a privilege, but a right.
2) What do you think is your role as a public health nurse (PHN) in the implementation of the PHC-UHC?
- Nurses in primary health care work in a variety of roles, including health promotion and education,
illness treatment and prevention, and community development, and they’re able to assist and identify the
needs of a client. They also plan, manage, control and assess client's perspective. Nurses will give the best
health care service to all people there are no rich or poor into the service quality because the PHC and the
UHC implements the service quality that even when you are poor you will be given a first-rate health
service. And another role is protecting and promoting the health of populations using the knowledge from
PHN and promoting health and disability for all the people that are through the creation of all conditions in
which the people will be healthy, and the PHN can collaborate with other health professions in order to
identify the specific problem focused on health care service. The PHN will consider the factors and cost on
the planning and delivery of the PHN programs and services to all people.
3) If applicable, should nurses/PHN undergo specialize training in relation to the
implementation of UHC?
- We think nurses or PHN doesn’t need to undergo specialize training in relation to the implementation
of UHC. According to Dr. Faraon, from the other video provided, education would begin with educators.
Like he said, new curriculum especially for medical courses are already revised to have focused onto
community health based. On that said, young people or aspiring medical practitioner have already acquired
the knowledge about the infrastructure of this health system while they are still studying. For nursing
students like us, as we talk this topic as of now, it is already implementing to us the important and how it is
done. So, in the future we only need to spread and implement the use of it to the people. Specialize training
is not necessarily needed because it is simple to use if you have enough knowledge about it. Probably many
nurse will not or can not participate because they already have loads of work to do than to do the training,
we think giving them guidelines or seminar to promote the system is way better than undergoing on
specialize training.
4) What is the role of the PHN in the individualized and population-based health needs?
- Community involvement and knowledge of the entire population are combined by public
health nurses with personal, clinical understandings of the health and illness experiences of
individuals and families within the population. They help members of the community express
their issues and ambitions by translating and articulating the health and illness experiences of
varied, often vulnerable individuals and families in the population to health planners and
policymakers. Public health nurses are aware about a variety of intervention tactics, including
those that apply to the overall population, the family, and the individual. Through focused
interventions, programs, and advocacy, public health nurses bring information from the health and
social sciences to individuals and population groups. A single public health nurse or a group of
public health nurses can practice public health nursing. The role of a public health nurse is to
promote public wellness. In this capacity, nurses focus on helping underserved and at-risk
individuals gain access to the care they need. The main goal of public health nurses is to help
prevent disease and reduce health risks at the population level through evidenced-based care and
education. This is achieved by promoting healthy lifestyles ,researching diseases, injury
prevention, detecting, preventing and responding to infectious diseases.

• Lack of doctors, nurses, and primary health care takers are another issue. This issue is inevitable since
great numbers of doctors and nurses are trying to go “abroad” since the pay here in our own country is so
low. But, by changing the curriculums, schools have produced much more great and excellent doctors
and nurses. Also, Universities and Med schools always have a big role to play in shifting the focus of
young medical students to understand the Universal Health Care and Primary Care systems.
• There has also been a certain wide discrimination towards generalist doctors (general practitioners).
Filipinos much prefer to see specialist doctors because they have much more longer years of training.
That doesn’t mean that the generalist doctors are nowhere near the skills of a specialist doctor, they are
all doctors, and they are all experts in a certain field. That’s why this is a mindset that needs to be
changed.
What challenges the operation of the Universal Health Care System is that there are hardly any family medicine or community
medicine doctors to provide the health care needed by the people in the community. The goal of the UHC is to provide affordable
health care system for those in the line of poverty, and in that case, necessary personnel such as GPs are needed to be produced in
order to accommodate a great number of client needing medical attention. Since a shortage of Physicians or General Practitioners,
most work were entitled to community nurses which cares and resolves issues pertaining to health concerns in lined with the nurse’s
scope of practice and could be labeled under one of the nurse’s roles and responsibilities.
Another challenge faced by the Implementation of UHC is regarding prejudices on medical practitioners resulting in imbalances
on concepts and standards of quality practice on the skills of medical professionals. The referral system aids to solve and procure a
steady mindset of Filipinos on the capabilities of every medical professional and avoid crowding in hospitals through a step-by-step
consultation and referrals. The mindset being discussed was the thinking that specialists have the upper ground on medical care,
resulting in the discrimination towards the general practitioners which had almost the same knowledge than the specialist. Although
the scope of the GP is way broader and not settling, with the referral system addressing the toxic mindset of the Filipinos, it designated
more tasks to non-specialist medical practitioners, especially on community health nurses who would serve as the initial care provider,
filtering out those with mild health concerns which can be addressed and treated by the nurse. And those who needed secondary care
services are then referred by the nurse to the attending physician within the municipal hospitals where they are given the needed
secondary care to be aided and resolved by the attending GP. For those with chronic illnesses who should be addressed by a specialist;
are referred to the corresponding specialist by a GP from the secondary health care provider.
In summary, more obligations and tasks were about to be handed-over to community health nurses. The scope of the roles and
responsibilities of a community health nurse may be expanded to cater heavier duties. Since the scope is expanded, a huge load of
patients would be consulting the primary health care providers such as the community nurses, for their health concerns. Fairly enough,
more concerns can be addressed by the primary care providers maintaining a healthy community.

- As we all know health is the most important aspect of a human being, but this comes with different issues about the healthcare system. There
are many problems in the Philippines that is related to the system such as the universal healthcare act and philhealth, lack of doctors, health literacy, and
health information. To begin with, we have the universal healthcare act. This act is passed in 2019 which expands the access of all Filipinos to health
services especially the primary health sector. The act also enrolls all Filipinos in philhealth, a national health program, which ensures the access to
health services without financial hardships because according to this RA 11223 healthcare is a progressive right. Now this program affects the
healthcare system in a both positive and negative way. Positive because it can provide healthcare services to the non-privileged population as well as
give them the chance to improve their health. On the other hand, it can negatively affect the nurses in the community because as the number of people
who needs intervention grow, the quality of care becomes low. It can also cause hardships toward the nurses because there are times where nurses,
especially community nurses, experience minimal funding which then affects the available referral systems. Next problem under the universal
healthcare act is that we are currently experiencing a lack in healthcare providers especially doctors as well as the toxic mindset associated to that.
These doctors are crucial in improving the quality of life of a patient or client because they are the one responsible for the proper diagnosis appropriate
for the condition of a patient or client. Together with that is the toxic mindset where being a specialized doctor has more benefit and knowledge than
those who are practicing medicine generally or also known as general practitioners. This mindset should be set aside or changed because all doctors,
specialized or not, followed a comprehensive curriculum that allowed them to acquire the needed knowledge in becoming a doctor and without these
doctors, nurses will be greatly affected because it is a known fact that nurses are cannot diagnose a patient or client thus it will also affect the education
and health promotion directed to the patient or client. Doing health promotion includes health literacy and health information. These are also
considered a problem under RA 11223 because when we say health literacy, it is the capacity of an individual to obtain, process, and understand basic
health information which is vital in health. While health information is the data related to a person's medical history like symptoms, diagnoses,
procedures, and outcomes which is needed to make a good decision not based on a “whim”. Without these, the nurse might not have a baseline data to
properly assess the patient and give the right intervention and that can hinder the role of being a nurse. In conclusion, this universal healthcare act is a
major problem that needs to be addressed and polished because if not, health care providers and practitioners will suffer that could result in providing
poor quality of care that will affect the individual’s quality of life.
