times when the Manager may take the same investments for a Fund and one or more funds. This may create a conflict of interest if there is only a limited amount of the
investment available, or if the investment is purchased at different times or at different prices for different funds. If this happens, the Manager will attempt to allocate the
investment fairly between the Fund and other funds. Factors which the Manager considers in allocations include the size and timing of previous allocation, whether the
security meets the objectives of the respective portfolios, the relative portfolio size and the rate of growth of the portfolios.

ACKNOWLEDGEMENT OF VARIABILITY

I acknowledge that:

I have applied with Allianz PNB Life Insurance, Inc. for a Variable Unit-Linked (VUL) Life Insurance Policy, and have reviewed the illustration(s) that shows how a VUL life insurance policy
performs using company's assumptions and based on Insurance Commission's guidelines on interest rates.

I understand that I may apply to withdraw my funds in full or in part by filling out the prescribed form and submitting it to Allianz PNB Life Insurance, Inc., subject to the applicable rules
and guidelines set by the Company. Any values shown in this illustration will be affected if premiums and/or withdrawals are not followed as assumed in the illustrated schedule.

I understand that since the fund performance may vary, the values of my units are not guaranteed and will depend on the actual performance at that given period and that the value of my
policy could be less than the capital invested. The unit values of my VUL Life Insurance Policy are periodically published.

I understand that the investment risks under the policy are to be borne solely by me, as the policyholder.

ACKNOWLEDGEMENT OF SCHEDULED WITHDRAWALS PROGRAM

I acknowledge that:

I understand that the scheduled withdrawals would start and end on the years specified in the illustration of benefits coinciding with the anniversary of the chosen plan.

I understand that the specified amounts would be paid automatically by the plan from my chosen funds while the plan is inforce.

I understand that the amounts specified may vary depending on the actual performance of the chosen funds and would only be paid out while there is enough money in the chosen funds to
pay for the scheduled withdrawals.

I understand that if the amount in the funds would not be enough to pay for the scheduled withdrawals, the remaining fund balance at the time of scheduled withdrawal payments would be
automatically paid out in full.

I understand that the remaining amount in the funds after the payout of the scheduled withdrawals are still subject to the minimum requirements set by Allianz PNB Life and if the amount
in the chosen fund would fall below the minimum that the balance would be automatically paid out at the same time as the scheduled withdrawal payouts.

I understand that should all amount in the chosen funds be paid out due to the conditions of scheduled withdrawals described above, the chosen plan would automatically terminate,
including any life insurance coverage of the plan.

I understand that I can change or cancel the scheduled withdrawal program while the plan is still inforce by submitting a written request to Allianz PNB Life at least six (6) months prior to any
scheduled withdrawal payouts subject to the approval and within the set guidelines of Allianz PNB Life.

I understand that the investment risks associated with the scheduled withdrawals and fund performance are borne solely by me, as policyholder.
