Administering Oral Medications
PURPOSES:
1. To provide a medication that has systemic effects or local effects on the GI tract or both.
EQUIPMENT:
● Medication cart
● Disposable medication cups:
small paper or plastic cups for tablets and capsules, or waxed or plastic calibrated medication cups for
liquids
● Medication administration record (MAR), or computer printout
● Pill crusher
● Straws to administer medications that may discolor the teeth, or to facilitate the ingestion of liquid
medication for certain clients
● Drinking glass and water or juice

Administering Oral Medications
1 Gather equipment. Check accuracy and completeness of the MAR or
computer printout with prescriber’s original medication order. Check
client’s name, medication name and dosage, route of administration,
and time of administration.
Check the client’s chart for allergies.
2 Assess risk for aspiration using a dysphagia screening tool if
available. Protect client from aspiration by assessing swallowing
ability.
3 Discuss the procedure with the client.
4 Perform hand hygiene and put on PPE, if indicated.
5 Prepare medications for one client at a time.
6 Read the MAR and select the required medication. Check the expiry
date and perform calculations.
Lock the medication cart before leaving it.
7 Empty the required dose into a medicine container. Avoid touching
the preparation.
8 Prepare the required medications:
1). Unit dose packages: Place unit dose-packaged
medications in a disposable cup. Do not open the wrapper
until at the bedside. Keep narcotics and medications that

require special nursing assessments in a separate container.
2). Multidose containers: When removing tablets or capsules
from a multidose bottle, pour the necessary number into
the bottle cap and then place the tablets or capsules in a
medication cup. Break only scored tablets, if necessary, to
obtain the proper dosage. Do not touch tablets or capsules
with hands.
