National Health Service and personalized care


In order to improve the management of long-term diseases (LTCs) and encourage preventative
healthcare, the NHS in the UK has developed the NHS Health Check and Personalised Care
initiatives. The NHS Health Check program evaluates cardiovascular risk factors in people between
the ages of 40 and 74 in a methodical manner in order to promote lifestyle changes and enable early
intervention. Conversely, Personalized Care incorporates social prescribing to treat both medical and
non-medical elements of health, customizing healthcare regimens to each patient's unique preferences
and needs. Through the integration of these techniques, the healthcare system takes a proactive and
holistic stance, giving priority to early diagnosis, prevention, and tailored interventions in order to
improve long-term health outcomes. These tactics seek to lower health disparities, enhance general
health outcomes, and provide people the confidence to take an active role in their own well-being
(Bunten et al., 2020).
NHS Health Check-Up:
Aims
The NHS Health Check provides a preventative health assessment to those between the ages
of 40 and 74. Encouraging good living behaviours and reducing risk factors related with
cardiovascular illnesses are among its main objectives. Through the evaluation of important
health markers like body mass index, blood pressure, and cholesterol, the program seeks to
detect any health problems early on and facilitate prompt treatment and intervention (NHS
Health Check, n.d.). The goal of the NHS Health Check is to lower the prevalence of
preventable diseases and encourage proactive treatment, especially for the target age range
(McCracken et al., 2024).

Services/Components
Risk Assessment:
Individuals undergo a thorough evaluation to assess their risk factors for Long term conditions. This
assessment considers various factors such as age, gender, family medical history, existing health
conditions, and lifestyle choices. The goal is to identify potential risks early and tailor preventive
measures accordingly (Debiec et al., 2023)..

Lifestyle Advice:
Participants receive personalized guidance on lifestyle modifications aimed at reducing LTC risk. This
advice includes dietary adjustments to promote heart health, recommendations for increased physical
activity, strategies to quit smoking, and guidance on moderating alcohol consumption. These lifestyle
changes are crucial for managing and preventing cardiovascular issues (Debiec et al., 2023).

Referral and Follow-up:


Individuals identified with a higher risk are referred for further assessment or treatment. Routine
follow-ups are recommended to monitor progress and make necessary adjustments to treatment plans.
This ensures ongoing support and optimization of preventive strategies to effectively manage
cardiovascular health (NHS Health Check, n.d.).

Approaches to Delivery
Healthcare providers administer NHS Health Checks across multiple settings, including pharmacies,
community clinics, and general practices, facilitating accessibility for individuals. These checks
typically involve face-to-face consultations, allowing individuals to discuss their health concerns and
receive tailored advice. The in-person nature of these consultations enables a more personalized
approach to healthcare, where individuals can engage in meaningful discussions about their health and
receive guidance tailored to their specific needs and circumstances. This hands-on interaction fosters a
collaborative relationship between healthcare professionals and patients, empowering individuals to
take an active role in managing their health and well-being(Atkins et al., 2020).

Personalised Care
Aims
Personalised Care recognizes individuals as active participants in their healthcare journey,
emphasizing tailoring services to meet their unique needs, preferences, and goals, particularly for
those with chronic conditions. The main objectives of this approach include enhancing patient
outcomes, increasing satisfaction with healthcare experiences, and fostering collaborative partnerships
between patients and healthcare professionals. By acknowledging the individuality of each patient and
providing customized care plans, Personalised Care aims to promote patient-centered healthcare
delivery, ultimately leading to improved overall health outcomes and patient empowerment.

Services/Components
Personalised Care and Support Planning:
Involves patients and medical professionals working together to collaboratively make decisions in
order to create a care plan that takes the patient's preferences, objectives, and values into account.
This includes components of treatment that are not medical in nature (Mikkola et al., 2020).

Social Prescribing:
Integrates social factors alongside medical interventions. It involves connecting individuals with non-
medical resources, such as community groups, exercise programs, and mental health services, to
address their holistic needs (Tierney et al., 2020).

Personal Health Budgets:


Allows people with complex needs to choose how to spend their healthcare funds, giving them more
control over their costs.

Approaches to Delivery:
Social workers, medical officials, and patients must work together and communicate clearly in order
for this to happen. It stresses regular meetings and planning based on the person to make sure that
complete and individualized care is given.

Benefits of Supporting People with LTC


Early identification and intervention plans are a big part of both the Personalised Care and NHS
Health Check methods. Health exams that find risk factors early allow quick action to be taken to
avoid problems, and personalized care ensures a proactive and individualized way of managing pre-
existing diseases (Christoffersen et al., 2022).
Personalized Care gives people more power by letting them have a say in choices about their care.
Taking part in activities like these builds self-efficacy and a sense of responsibility, which helps
people in long-term care homes better handle their illnesses.
Personalized care recognizes the importance of taking care of both the medical and non-medical parts
of health. To help people who need long-term care, you need to use a comprehensive approach that
considers the bigger social and living factors that impact their health.

Impact and Effectiveness:


 NHS Health Checks show improvements in detecting and managing cardiovascular risk
factors, but their overall impact may vary, and challenges like ensuring follow-up care and
increasing uptake rates need to be addressed (Alageel et al., 2020.
 Personalized Care has been linked to positive outcomes including enhanced patient
satisfaction, improved quality of life, and better self-management of illnesses, although
ongoing assessment and implementation enhancements are necessary to optimize
effectiveness.
 Both NHS Health Checks and Personalized Care aim to promote preventative healthcare and
support individuals with long-term care needs.
 Combining these approaches creates a more comprehensive and person-centered healthcare
system, ultimately enhancing quality of life and health outcomes (Halteren et al., 2020).

Case study
The 48-year-old cab driver Hasan lives in a low-income neighborhood of Bradford and faces
numerous health problems. His unhealthy eating habits, smoking habit, and lack of physical activity
worsen his Type 2 diabetes and hypertension. Hasan is eager to take charge of his health, but he is
unable to do so because of stress, financial hardship, and obligations to his family. The strain on the
family dynamic is exacerbated by Salima's mental health concerns, his wife. Significant hazards are
posed by Hasan's numerous health conditions as well as his high blood pressure. It is essential to
address his lifestyle determinants and assist his mental well-being as a social prescribing link worker
in order to mitigate the harmful consequences of his long-term problems.

Background to High Blood Pressure (Hypertension)


Chronic hypertension, also known as high blood pressure, is a common long-term condition in which
blood pressure numbers stay above normal. Heart diseases, like heart attacks and strokes, are more
likely to happen if you have this condition. Hypertension usually happens slowly over time and is
affected by many things, such as genes, living choices, and health problems that are already present.

Principles of Person-Centered Care for Managing Hypertension in


Hasan's Case
1. Collaborative Care Planning: Engage Hasan in developing a personalized care plan that
considers his lifestyle, preferences, and cultural background [4]. Collaboratively set
achievable goals for managing hypertension, such as dietary changes, smoking cessation, and
increasing physical activity.
2. Shared Decision Making: Involve Hasan in decisions regarding his hypertension
management, ensuring he understands the risks and benefits of treatment options. Discuss the
importance of adhering to prescribed medications, lifestyle modifications, and monitoring
blood pressure regularly (Ali et al., 2020).
3. Self-Care Management: Empower Hasan with self-management skills to monitor his blood
pressure at home, understand his medication regimen, and make informed decisions about his
health. Provide resources and education on healthy lifestyle habits, including dietary changes,
smoking cessation support, and stress management techniques (Ortiz & Fromer, 2011)
4. Health Literacy: Ensure Hasan and his family understand the significance of hypertension,
its complications, and the importance of treatment adherence (Andersson et al., 2022). Use
culturally sensitive and language-appropriate materials to enhance understanding and
engagement.
Benefits of Person-Centered Care Interventions
 Improved Treatment Adherence: It is more probable that Hasan will stick to treatment
regimens and lifestyle modifications if he is involved in the decision-making process and his
concerns are addressed. (Kalra et al., 2017)
 Enhanced Health Outcomes: Care that is concentrated on the individual encourages better
regulation of blood pressure, which in turn lowers the risk of cardiovascular problems and
improves overall health outcomes (Tisdale et al., 2021)
 Increased Patient Satisfaction: Hasan is more likely to feel supported and motivated to
manage his hypertension when he is actively involved in the planning and decision-making
process for his care.
By applying these principles, Hasan can receive comprehensive support that is tailored to his needs,
which will foster better hypertension management and overall well-being.

Conclusion
Implementing person-centered care principles, such as collaborative care planning, shared decision-
making, self-care management, and health literacy, can greatly enhance the management of Hasan's
high blood pressure. By actively involving Hasan in decision-making, customizing interventions
based on his preferences and requirements, and equipping him with self-management abilities and
understanding, healthcare providers can enhance Hasan's capacity to effectively control his
hypertension. This approach cultivates a supportive and empowering atmosphere for Hasan, which
ultimately contributes to improved health outcomes and a better quality of life.
